Asherman's syndrome (uterine synechiae) एक ऐसी condition होती है , जिसमे uterus की lining एक दूसरे से चिपकी हुई होती है। इसके काफी सारे कारण है , पर सबसे मुख्य कारन है Endometrial Tuberculosis, बार-बार गर्भपात होना जिसके कारन Dilation and curettage (D&C) करना पड़ता है और endometrial lining infection जिसके कारन Asherman's syndrome होता है।

Asherman's syndrome का diagnosis पे ध्यान दे तो पेशेंट history जानना बोहोत जरुरी है। Asherman's syndrome की वजह से महिलाओ के menstrual cycle पे बोहोत असर पड़ता है क्योकि हर menstrual cycle में endometrium बढ़ता रहता है और period के वक्त यह endometrium , blood के रूप में निकल जाता है। पर Asherman's syndrome में क्युकी ये wall एक दूसरे से चिपकी हुई होती है इसीलिए सारे hormones ठीक होने के बावजूद भी lining ठीक से develop नहीं होती क्योकि वह एक दूसरे से चिपक चुकी है। इसके वजहसे period का flow काफी affect हो जाता है (less bleeding during periods reason) और यह काफी महत्वपूर्ण diagnosis है।
Asherman's syndrome diagnosis
काफी बार patient अपनी history बताते वक्त बोलते है की उनके period cycle काफी normal थे पर abortion होने के बाद उसमे काफी बदलाव आगये है और सिर्फ spotting हो रहा है। जिसको amenorrhea कहा जाता है। इस चीज़ के कारन डॉक्टर्स को थोड़ा अंदाजा आ जाता है की शायद patient को Asherman's syndrome हो सकता है तो ये साबित करने के लिए डॉक्टर्स 3D ultrasound या 3D sonography करते है जहा पे internal sonography या trans vaginal ultrasound किया जाता है। इसमें uterus की wall की जांच की जाती है। उसका एक protocol होता है और यह protocol करने से uterine cavity के volume के बारेमें पता चलता है और uterine adhesions या कहा चिपका हुआ है यह समझमे आता है। यह होने के बाद sonography के bases पे इसका classification किया जा सकता है की यह point adhesion है या ये complete adhesion है। यह सारी चीज़े 3D sonography पे दिखाई देती है। यह करने केलिए २०-२५ मिनट लगते है। एक बार wall की thickness और adhesions के बारेमें पता चले तो उसके बाद uterus के अंदर के bloodflow के बारेमें जांच की जाती है क्योकि इसके ऊपर उसकी treatment निर्भर होती है।
जिस महिला को Asherman's syndrome होता है उसको infertility भी होती है क्योकि गर्भरोपण करने की जगह ही नहीं होती। इसकेलिए ट्रीटमेंट होती है hysteroscopy जिसके बारेमें हम अगले वीडियो में जानेंगे।
