    I had a 2.5cm rupture. Two weeks post surgery now and jeez, it doesn’t take long to lose that muscle mass

      @@dustinkeeton4193 I’m 7 months since injury now. Still not back to full strength and flexibility. walking down hill and stairs are still challenging. It’s all about calf raise reps for me. Hope your surgery went well.

      @@winslowwong9719 I think the surgery went well but yea I'm still in the phase where I'm just looking forward to being able to walk again.
      Hoping (read: desperate) to at some point get back to sports...

      @@dustinkeeton4193 hey man Same. Ruptured Achilles in October was able to walk in December and re-ruptured it again. Now I’m looking forward as well to finally after 4 months walk again. Hope everything will go well for you! Hope so to be able to do sports eventually

    Fully ruptured my Achilles yesterday. Didn’t get much sleep. Trying to figure out if surgery is a good option for me. Found this video very informative. Long road to recovery but this video and many of the comments have been helpful. I hope you all recover fully and get back to normal life soon 🙏🏽

    I did mine 2 weeks ago.... the most vile feeling I've ever experienced... what shocked me was how easily it went... in a boot for 6/8 weeks and they said I'd be driving and walking correctly in 10 to 12.... so frustrating as I've only been in my new job a month and now I've got to have 3 months off.... gutted!

      @jorgerosales6646 so I was off work about 14 weeks ... went back and still couldn't walk correctly and was still experiencing pain... but I cracked on and was able to stay at work even though it was difficult at times... I ruptured mine by the way it wasn't completely snapped (felt like it) 😂

    This is a great explanation.
    I fully ruptured my Achilles six weeks ago. Went non-op. Spent 2.5 weeks in a splint and 2.5 in a cast in planar flexion. I’m on day 5 of a boot with three wedges.
    Surgeon didn’t give me much instructions to be honest. Just told me he’d see me in 4 weeks. Just looked at it for a few seconds and said it seems to be healing well. He didn’t even tell me if I should walk on my boot right away or not.
    When he left the room the person who applied the boot said I shouldn’t put weight on, which I thought was weird. No talk of a schedule for removing wedges either.
    Thankfully YT videos and research has been my biggest guide to learning about range of motion / partial weight bearing and all the important things to help the process. So I appreciate the info.
    Was able to move around with one crutch by day one of the boot and dropped the crutches completely on the second day. It’s so great to have my hands back. Doing gentle exercises like those you mentioned just to get activation in the muscles. The swelling is the biggest issue now.
    I’ve got my first PT appointment in a little over a week so looking forward to get a bit more info on exercises I should do.

      Hey, I'm 6 weeks into my recovery, 5 in a bit with 3 wedges. I also felt like the info I was given was lacking and was just told today in my second physio that I can walk on it but to take it easy. Problem is one foot has a 3" lift and walking with a normal shoe on the other foot stains my knees and hips. I have a pair of River boots that are quite tall and I'm walking almost like normal.
      That said, goes is your recovery going since you've posted here 8 weeks ago?

      @@davidhamm2613 Hey David, so it's been 18 weeks since I first went into a splint on June 7. 2 days after the injury, since i was injured on a weekend and at there wasn't an ortho to see me yet.
      I spent about 4 weeks in the boot with three wedges (7/12 - 8/9). Never removed any wedges incrementally which i found odd. Just straight to flat after the 4 weeks. I was re-fit with a new boot. It actually hurt for like a day and a half to walk in the boot do to the extra stretch on my tendon. Walked out of the hospital VERY slowly that day. But within the week I had dropped the boot completely. Which I had been doing partially before indoors for about 1.5 weeks to move around with weight on my heel. So by 10wks from injury I was moving around without a boot but being cautious.
      I used an "uplift" brand attachment around my shoe on my good leg and that helped even up my stance. Which I highly recommend since it makes it easier to walk and keep your boot as forward pointing as possible than say a side skew do to the peg leg walk when uneven. Because this is one of the tendencies that you have to get rid of once you are out of the boot. Your mind defaults to a limp in order to protect itself but messes with how your muscles are used. You can find them on amazon.
      I started going to PT every two weeks at that 9 week mark, right after i got refit with the boot, and that was quite helpful. One of the first things I was asked to do was walk forward without my boot and then walk backwards. No limp when you walk backwards, so that become something to practice. By my second PT appointment I had dropped the boot entirely as mentioned.
      I'd say that during the first two weeks out of the boot I was doing some of those mild stretches to see if I could get my knee to bend forward beyond my toes. I couldn't yet and it was a bit painful but I could see I was gaining cm's every week. By week 11-12 I was able to get my knee as far as my toes.
      I still favored stepping doing with my healing leg first when going down the stairs due to the dorsal flexion required on my healing foot if I stepped down with the other. But I was gaining flexibility pretty quickly.
      In terms of walking I'm pretty much back to normal. There is always some tightness in the morning when going down steps but after a couple of hours it's fine. It's just a matter of tight tendons I suppose. At week 14 I was walking 10K steps a day at a camp retreat I was out for 4 days. But still had a bit of swelling. Though not much of an issue the last 2 weeks.
      I've been biking since I got out of the boot. Haven't been cleared to run yet, or jumping, but i've done some jogging after the kiddos without feeling to worried. Everything seems attached and strong, but it's really just about rebuilding flexibility and strength at this point. My calves feel similar-ish to me size wise now but the ease with doing a calf-raise on my good leg vs the healing one is a reminder that it's not 100% yet. But I can do a calf-raise on the healing foot now just hard to balance so i usually do it against a table / wall. So I think that's pretty good for 4mos. I thought I was doing them right a few weeks back but my therapist said i was bending my knee and that i need to try to do them without bending my knee.
      I also asked my therapist if i can go to a climbing gym near me now, which i was hoping to do before my injury. He said i should be good and it will actually help my calf strength. PT is usually stuff like leg presses, leg curls, side plank holds and balancing on these discs / boards that create a bit of circular motion / stretches around my ankle. A good chunk of the PT is focused around overall strength of other leg muscles I wasn't using as much during the ordeal.

      One last thing, I found sujitdey81 or SJ's channel on his journey extremely helpful to follow along in terms of his exercises and progress.

      @@fgarciaish Thanks for the reply. It's strange hearing how different everyone's experience is with the healing process and what it's done in physio. For the first week I had a plaster half-cast moulded to my calf and run down to my toes, held on by tensor bands. That causes a lot of pain as it settled into the space left by the tendon. Then into an air boot with 3 wedges. I'm still in that now with the 3 wedges. My first PT was at week 4 and I was told I could start progressing to weight bearing and unassisted walking over the next week. I'm just about at week 6 not and I've been walking in the boot for 2 days.
      I've been watching some videos of various physiotherapists who are far more aggressive in their return to weight bearing with reasons backed by good scientific literature so I'm more confident but also still really worried I'll do damage if I go faster than my PT. It seems the PT is slower than some but sounds like you were put into 90° really early with PT later than I would have expected. I'm going once a week from week 4 and twice a week from week 8. So far just basic toe movements, ankle inversion and eversion and large muscle groups, no calf or shin muscle work.
      I bought a cheap bike trainer to stay my bike to for some simple cardio.
      Hearing from so many people about successful recoveries has definately raised my spirits over the weeks. Just being able to walk again was a huge boost.

    I just ruptured mine a week ago and I’m on the none surgery route in the boot. It’s quite a daunting thought but your videos are helping me stay positive and are very informative. Thanks kindly

      @@benwilliams787 Hey! Sorry to hear about your rupture and hope you are doing well.
      My consultant put me in a boot a couple of days after it happened. I had an ultrasound a week after which confirmed full rupture, however the tendon ends were very close together and there was no gap. He recommended non-surgical.
      For the first 2 weeks in the boot I needed to get around in crutches, however now I can walk pretty much fine, and from today I can get up and down the stairs no problem. I still have 4 wedges, and I'm due a follow up on Monday to have one of them removed and start my physio.
      It's hard to tell how I'm recovering as I can't really use it yet, but I can put all my weight on it (with boot), there's no pain, and swelling has massively reduced. I'll keep you updated!

    I have a full rupture and all the doctors I saw said they recommend I do the non surgical route which was a relief since I don't like anything surgical. I'm going to be week 5 tomorrow (Nov 17, 2023) but I am going through the Veterans Affair hospital and they are always fully booked for PT. I'm finally going to be seen Monday at the week 5 time frame but I have been weight bearing perfectly fine with my boot since about week 2 with the three wedges in. I have seen and researched that weight bearing early on helps the fibers form stronger/better if I'm not mistaken. I try to feel the "scar tissue ball" my doctor mentioned would form but I'm not sure if I'm healing, I guess im a bit impatient but I am going into this blindly since again I am going through the Veteran Affairs hospital, I really hope I am somewhat healing since I am very active and this has put me at a stand still from a lot of things.

      @@zazabaldtv2313 I'm 8 months in since my initial rupture and my tendons fully healed with no ridge from the tear. only thing is my calves are still weak and its sore in the mornings which is my fault, I've been slacking on PT. However I'm able to somewhat run and jump I'm just careful with it

      @@zazabaldtv2313 I'm 8mo out from initial rupture. It's fully healed and connected but I usually am sore in the morning but that's my fault. I've been doing PT but kinda slacking on it on my own. I'm able to somewhat jog/run and hop but it's still very weak. So far the conservative(non surgical) process have been good to me

      @@bashirhammad5940 So I had a full tear and I am now a year out from my recovery journey, I went non-surgical but I am doing great. I'm running, working out and pretty much feel back to as I was prior to my injury. Only thing is my right calf where I had my tear feels weak obviously from not having been able to workout for a while. From my understanding the re-rupture rate between surgical and non-surgical is only a 2% difference. You will recover faster going under the knife but only by 2/3mo earlier, with non-surgical it takes a little longer but you don't risk infections.

    I'm on week 8 LH ankle. At week 6 my ankle was feeling after hydrotherapy and the boot and looking fine but then I had a small pop so I went back to week 4 and a boot again. I've taken the wedges out now at week 8 and am using a wedged sport shoe and doing massage with Vit E cream and light stretches. I've now got about 50% of my tendon back at the rupture site. I'm waiting to see a physion.

    Had a full rupture about 3 weeks ago. The MRI showed a 1cm gap. I am in a cast and crutches and the doctor told me to not put ANY weight on it at all. I want to know why i didn't get a boot! I have 2 kids and I'm relying on everyone to help me. I hate it! I want the boot!

      Let me guess you're either in a splint or cast?I was put in a splint at first and just got out of my cast yesterday..Are you choosing to go op or non op?I ruptured my right Achilles March 6th this year

    I had a 30% tear in my Achilles, they wanted to put me in a cast but I refused and had a boot with 4 wedges so I was able to walk on it straight away. I did a lot of walking and reduced the wedges and after week 5 discarded the boot. I walked carefully with a limp and did daily exercises. I’m now on week 8 I’m walking almost normally. I’ve been able to do step ups with both legs about a week now, but at the moment cannot stand on my tiptoe with my bad leg. Work hard on the exercises and listen to your body.

    Great video. I did mine playing footy 5 weeks ago and have opted for non-surgical route. I am slightly worried about progress, more so mentally. I was in a cast for 4 weeks and now in a boot with wedges. Hopefully this works. I'm extremely active, run a lot and cycle, MTB. But your video series are giving me some hope. I'm not sure when I'll be weight bearing but praying I'm able to start ROM in a week or so. Paying for years of overuse I guess.

    Fully ruptured mine August 1. Had surgery Aug 4. I'm non-weight bearing for 4 weeks, then get to go to the boot. Wasn't really given an option of non-surgical. Was just told that I needed surgery. Sometimes I wonder if surgeons suggest surgery like a car salesman says you need to buy a car from them. I don't know what my gap was. Maybe it was too big.

    Great video and explanation, thanks a lot - I have a full rupture 1 cm wide - I was scared and perplex when the surgeon opted for non-surgery, now I'm relieved to see that this approach is shared and valued world-wide. Still, I'm afraid not to recover full power and also for the risk of recurrence.

      i feel your fear bro. both of my knees acl reconstructed. 1st knee on my right was operated 2003 then the other knee was 2014. i have learned my lessons well not to push my self to the limit and be reminded thant we can ruin our lives by not being mindful of our physical welfare. God bless

      @@tekkenmaster1235 I am going to need a physiotherapist but I’ve been walking better each day and I have regain a lot of mobility and flexibility in just nine days. I am walking without my crutches and the claudication I had had been gone for 2 days

    I just raptured my achilles and i had a cast on for 2 weeks, then i got the boot on with three «levels» for the heel.
    Now i will use the boot for two weeks before i go down 1 level. Then 2 more weeks and down antother level. 1 week more, no levels and after one week from there i will take the boot. Hopefully :)

    Hi Tim, have to make a decision on op/no op on a retracted full thickness tear to my left achilles 60mm above the calcaneal insertion - I am a 58 year old male and reasonably fit and not that keen on surgery - any thoughts on this? Thanks.

    Can you please explain why PTs don't want you to put full weight on the bad leg? I can understand walking with full weight but STANDING with full weight? Example: Brushing your teeth or shaving. I understand its supposed to be a precaution against falling but with crutches you have the chance of breaking your hip.

    I fully ruptured mine on Wednesday night at Touch Football. I'm not looking forward to all the time off work and life as I knew it goes out the window. 😢. Has any one heard of a certain heat treatment that helps complement the healing process ?.

    I have / had a 2.5 cm break, it was 5 days before I was 1st diagnosed as partial tear, went into a boot (one heel lift) while waiting for MRI, 3 weeks after accident finally had MRI results and visited Podiatrist surgeon who gave me choice of surgery or conservative treatment, no mention of whether or not a 2.5 cm break would have repaired better with surgery, especially after 3 weeks before MRI and Dr diagnosis. I chose nonsurgical. Would I have been better off going with surgery? I went into a cast for 3 weeks, now back into boot for 2 more weeks NWB. Does that sound right, no physical therapy was considered until after a full 8 weeks. :(

    hi, question. Mine is a bad one, rupture more than 5 cm. having surgery on jan 2, question , is it okay to ambulate w mobile scooter or free walk device from amazon w the bent knee
    ? concerned about poor blood flow if I bend the knee to rest on the padded mobile scooter or Walk free device, please advise . Thanks Sebastian

    Great watch my rupture is quite high up my calf so no surgery was in a cast for 5 weeks have now been in a moon boot for a month with 3 wedges started physio and acupuncture moving onto a shoe 👍

    I had a motorcycle crash which almost killed me, I fractured my femur, tib & fib, open fracture ankle and open fracture heel. I've had a skin graft on my right ankle now going round the back of my foot and 1 of my calf muscles (side ones) have been cut and put into my foot to fill the hole. I crashed in July and my leg was fixated with a metal frame from July to October, I was then put in a normal bandage (as im not allowed a cast over skin graft), then I got the all clear to start putting pressure on my foot on the 29th of October, however I can't flat foot standing upstraight... I'm trying my best, just feels like my bones wont allow it even though consultant said its all fine. I've read my notes an it mentions the Achilles Tendon Lengthening surgery, but really dont like the idea of it, as it was never torn or ruptured it was just my leg being immobile for months.. (now 6 months with no weight.. I've tried to put some weight, but I can't flat foot if i put my leg next to my other.. I've also forgot how to walk etc too.
    What exercises are best to flat foot, as i know everything is tight, ive also got a lot of swelling inside my foot, its squishy hard kind of feeling. You can see on my channel "why i havent uploaded in 3 months" for my injuries..

    Hi! This comment is from China! What a great explanation of the the recovery process. I‘ve been really unfortunate as I ruptured both of my Achilles’ tendon. I had surgery for my right foot on January 20, 2021, I went through rehab for around 6 months then I transferred to another city so I stopped. 9 months in and I started playing basketball like nothing happened and I broke my left Achilles’ tendon as well the same way I broke the other, without physical contact.... I’m only 20 years old and I feel like my basketball career has been terminated already.. anyways, I had my surgery for my left Achilles on December 7, 2021 and I just had my first rehab session yesterday. I have a question regarding walking with the boot. 3 weeks have been past after the surgery, the walking boot has 12 wedges in it making sure my foot is at a 30 degree plantar flexion, can I walk with the boot without wearing crutches? Or I have to wait longer? Again, loved your video, it has been really helpful ;)

    Sr I ruptured mines two inches before you get too the heel I had too have surgery it was very pain for my leg felt like a piece of rubber wood for about a year . I still have swelling and at times when it gets cold It be very stiff but at least I can walk ❤

      The shoulder ‘grinder’ machine in the gym is a popular option. Low load so you can donut for longer and get the cardio benefit

    Hi, thanks for the video! I ruptured mine this week and ultrasound found a 6mm tear. Im
    33 years old, play sports and my consultant thinks I should have open surgery. I’d like a faster recovery time and lessen my chances of re-rupture, is this the right thing to do?

      A 6mm is pretty small in the scheme of things and usually that would go well with non-surgical management and being in a boot for nine weeks. I would chat to you about no one‘s schedule management and then rehab. Have a look at my Achilles rehab program at physiorehab.com to guide you further.

    I got a boot with 3 wedges but I felt a bit too flat so after 4 days I added 4th wedge. Will I ruin my recovery?

    Is it bad to start doing to strengthening in a boot at 5-6 months? Cleared to 100% weight bear. Cycling, hamstring curls, quad extensions too risky?

    Great video! Just fully ruptured mine on Super Bowl Sunday morning playing racquetball. Opted for the recommended surgery so I could get back to the game with less chance of re-rupture. Braided suture used and cast and stitches being removed on March 8th. Then PT begins that evening. Hoping for full recovery and fearful of rupturing the other one ☹️. Should I give up racquetball?

    Hi Tim, Great videos, very clear and informative without being overwhelming to a layperson - thanks. Ruptured my Achilles on Sunday playing cricket setting off for a run and went to hospital yesterday where I had ultrasound and was fitted with a boot, given crutches and some blood thinners. Quick couple of questions about the crutches if I may - how much weight can I put on the boot, do I walk as normally as possible and how long am I likely to be on crutches (and could I use a stick instead perhaps)? Ta

    Hi Tim ..I'm in the 10 week of post op Im able to walk very very slowly with out any support ....but if I take rest for 2-5mints I'm not getting the constant walk like I have done before 2mints??
    How can I get rid of these thing

    Im going for my ultra sound tomorrow but they put me in a boot 2 weeks ago and ive been walking in the boot for 2 weeks and its not hurting too much only my calf and tendon when i walk sometimes should i be concerned?

      Oh man i literally just tore mine last night playing football, i didnt get an mri or an ultrasound but I’m seeing the specialists this week for the boot so hopefully we can do some additional imaging.

    Thank you, perfect vid!!! I'm exactly at 6 wks also w a tear and 2 wedges in my boot, was just searching on youtube why around my inside ankle I have that fluid. I actually had to add a thick work foot sole on top of the hard plastic wedges because the wedges have a thin pad that wore down, making it sore where the front of heel and my inner foot arch meet. Question: do you advice knee-hi compression socks? if so, light or medium compression?

    Hi.. great video. I need your suggestion, I had my ruptured 3 weeks ago. They did X-rays, Ultrasound and finally MRI and was told the gap is 3.9 cm. They put a cast on my foot. My surgery is due coming Wednesday. I am diabetic and am not comfortable with surgery. In your expert opinion, can this be treated without surgery ? My doctor told me that with surgery the chances of recurring is 5% and without it will be 30%, meaning I could do without surgery. What do you advise ? Would it need lot of physiotherapy ? I am also thinking of asking for the boot instead of the cast as I can not hop around in the crutches all day.. is it safe to remove the cast at this stage and get the boot?. I will appreciate greatly if you could give me your suggestion. Thanks

      In Australia usually anything >2cm gap requires surgery

    Do you think PRP injections would be a good idea? I've watched some YT videos where patients who ruptured their Achilles and didn't have surgery got very good results from PRP.

    Hi Tim, great video. I am in France with a 1.4 cm complete rupture. The ER surgeon put me in plaster and after two weeks a second ER surgeon at the hospital said ' we will let it heal itself' and put me back in another cast for a month. I am supposed to book physiotherapy for then but those I have spoken to are astounded that they did not operate and do not see how it can heal. However, on the web it seems to be 50/50 these days op or no op. Do I just keep looking for a physio who has had a better/more recent training?

    I'm in the UK and have a partial tear 2mm caused during a game of football ... have spent 6 weeks in a plaster non-weight bearing and today had my cast removed and placed in a boot. Docs are stipulating 4 weeks of non weight bearing with 4 wedges and then ANOTHER 4 weeks in the boot with a wedge removed per week - so EIGHT weeks in a boot total :(
    I've asked the doc about early weight bearing and my concern that this was a very conservative route I was on ... his response was that this is the policy of the hospital/head of orthopaedics
    This feels so cautious to me but would love to know what you think Tim?

      Weight bearing in boot should be able to do at this stage however I haven’t seen you Achilles so can’t advise you in tour case. The more healing the better remember

    My achilles tendon injury annoyance; i was pretty well at the 4week interval, 2weeks into boot and unfortunately I re-stretched or partially tore it again whilst walking around on holiday slipping. Full details of my injury are discontinuity of the achilles tendon approx 7cm from the calcaneal insertion. There is a 3mm gap consistent with either high grade partial thickness or complete tear. Complex fluid (likely haemoorhage) within tendon at the tear site is noted.
    When i did have an accident on holiday it felt more like a pull then a pop this time round.
    My biggest concern here is should I go back to get an ultra sound to see if there is more damage done. I did visit the ED again on the day and the doc just said take it easy let it heal more in the boot - but not sure if I should get a 2nd opinion and get a rescan to see if that's worsened the gap?
    Still yet to see physio even though I've been referred as outpatient to another hospital but concerned this has set back in the healing process etc. There is noticeable swelling and dull ache pain. But i can walk and load bare on the injured leg.
    Should I go back to get an ultra sound to see if there is more damage done?

    Hi, I'm now at my 7th week post-op and will begin PT next week. For now my took me off the casting and said to do some light dorsiflexion movement. It is normal to feel like the stretch it provokes will tear your Achilles repair? when I try to do those exercises I feel the stretch, even with very low pain associated, and fear for rupture. I'm able to do inversion and eversion without any pain or restriction, but that dorsiflexion is panicking . Thank you for the feedback

    Hasn't even been a week (5 days now). Similar story to @f1Clarky, tendon tore from the calf muscle (2cm) and consultant would rather run with the boot than surgery. 3 plates under the heel and my new space boot is with me all the time... Using crutches for the most part as 2cm difference between legs is pretty awkward however I have wellies that have a high heel. Balances up my right leg to walk around without the need for crutches. Hence I cut the lawn today! I presumed I could do some weight-bearing (some walking) while wearing the boot or am I making a big mistake?

      Avoid all that waking around. Listen to the surgeon and take it easy. It’s a long road and you want to do it right

    I’m at 6 weeks as well. I went for surgery tho, mine has healed but unfortunately the bottom of my feet are numb. It feels like its sleep all the time, they say its slight nerve damage

    I’m also at the 6 week stage now 😮‍💨, my rupture was slightly higher up the Achilles’ tendon where the tendon attaches to the calf. Is there any difference between recovery times depending on how high the rupture is ? Also thankyou for these videos!

      I believe the bigger the better if that helps. Healing time is healing time you have to hang in there. 12 weeks and then it’s strengthening time

      I had the same situation. I tore my achilles tendon where it joins achilles muscle. I am at 6 week now with air boot on.

      @@krnvirsingh I’m doing good thanks. I’m walking fine and just gradually building strength. I can do a single calf raise now which is great but still need to build a lot more strength. There is also still stiffness in my ankle and I need to continue stretching. Hope you’re recovery goes well.
