I have never before seen ambulances with air horns like that, nor have I seen any modern ones with any form of Q siren, electronic or otherwise. Those are some serious badass ambulances indeed!

EQ sirens are very expensive and have a tendency to burn out after a year and a half. But I think they are the best electronic siren going. My next favorite is the Carson Mech Tone.

At least one of Los Angeles Fire Department's ambulances has a mechanical siren. You can see this on rescue911.de Also, the Japanese sometimes use a mechanical siren on their ambulances as a backup to their standard hi-lo.
