As a new graduate nurse in the ER this is the BEST video I have ever seen to help me understand the pathophysiology of of DKA as well as intervention prioritization that goes along with its management. THANK YOU!
This was a very clear and concise explanation of how to medically address DKA for a hospitalized patient. As an RN who has had limited exposure to DKA medical emergencies, I thank you, Dr. Fazal, for your expertise in educating me on the fundamentals as well as appropriate treatment options.
Throughout my medical career i was struggling to understand the administration of insulin and Rx of DKA. I dont know how you made it so simple to understand. Now i got the confidence to understand and treat the patient myself ! Thank u Dr for such a wonderful video !
Thank you so much for these my child has been dealing with this multiple times in the past 6 months. A couple of times he sick due to a bug and food poisoning. I never really understood until you mentioned ketones in blood.
Don't stop of making these kind of videos which your are doing. it's my sincere request sir It's extremely helpful for nurses as well as doctors. Please make more videos. In this video's you explanation, and your are elaborating the things that's amazing which that enters directly to the 🧠 🙏👍🙏🙏🙏😁
what i've learned from this video: 1- diabetic ketoacidosis is an extreme metabolic state caused by insulin deficiency it's acute state of severe uncontrolled blood glucose 2- if the cause is dehydration then give fluids, if the cause is hyperglycemia give insulin 3- pass 2 large iv canula and start fluid 0.9% saline over 1 hour if the systolic blood pressure is less than 90mmHg give 50ml in 15mins 4- ask patients for test labs as: ABGs, blood glucose and blood kelones, electrolytes, CBC,ECG and CRP 5- start insulin and aim for drop of blood ketones 0.5mmol/hr and blood glucose by 54mg/dl aim for rise of venous bicarb by 3mmol/L/hr
Sir u explain every concept so perfectly kash hamare med school k teachers bhi aise hi sikhate hume ....you are the best sir tysm for all the amazing vds plz keep sharing more
Thank you doctor for making it so simple.. Things change with time, I did not see using D10W in DKA any more in my hospital, but overall excellent video, looking forward to watch most of your videos. I am your new student now.. thank you for helping students ...
Was never taught in medical college by professor in such a conceptual way...keep it up sir and male more such videos to clear our concepts in such topics..May God bless you
Very well explained doc.as I was always confused about complex treatment and why bicarbs are not given..you made it clear every doubt of mine.I have started watching your videos I totally liked them.Keep the good work doc.you have special power to teach all of us here.
Thank you Melese Mengesha. Hospital duties leave me with lesser time for videos. But I am working on some very interesting topics that I will upload very soon. 😊
My fiance passed in 2020,Travis was 48,Diabetic 2. He had been feeling sick. Had a fever. My pug also bit his 👋 thumb, I think she knew he was sick. HE had direah, we had a 900am.Dr appointment. HE passed as I was giving him CPR from 911-916am. I couldn't get air into his chest no matter how hard- I tried, I kept sweeping his mouth, A clear mucus like alien slime was so thick. It was blocking 🚫 the breath I was trying to give him. He was talking to me- Then he starry eyed leaned back, and we told one another we loved one another ♡ Thank you, God for that. But we stayed calm ,Justified his symptoms. Rationalizing and, convinced that we were OK 👍 to get to Dr. In am and he would be OK. But we were scared. I thought he had Covid. It was only April 6th 2020 and at that time - We Had zero cases reported in Connecticut. However NYC was climbing in infected patients. He was a child social worker. He worked with Father's and Children. He started the first Father Daughter dance here in Connecticut, He also independently started a diaper drive. He was a good man. We never made it to that 9am Dr appt. He was having a fever and direah. But we rationalizing and Justified symptoms 😢 bc we were scared. He never said, I can't breathe.... His sugar level was in 80"s! So his keytones were prob high according to what I'm listening. He passed in route at 9:25am. I gave CPR from 911-916am... I have a fire station across my street. Bf they got my door open I was yelling he needs sugar! I THOUGHT what I was seeing ,Low sugar pre coma symptoms. I never seen someone talking and so calm.😕 I think he was at peace, with going home. He said I love you. And those were his last words. Again I am grateful 🙏 for this.. But I still don't know what happened to him. In 2020, Drs werw saying Covid didn't have direah as a symptom. But he was hiding that 🤔 from me. I found a big mess in the bathroom the next day. He missed the toilet. Yet somehow had the strength to shower, and get dressed for Dr appt. Little did I know that was the last time ,I would see him. My point 👉 is GET TO ER. Travis was prob scared he would end up dying on a ventilator alone. As that was the only treatment in the early days of this plague. But again- Take your loved one to the Emergency room. It could save their lives! 💞💞💞💞💞💞 Thank u ,Doctor for explaining this to me. I'm not sure what happened. But he did say his lower back was hurting that night before.. Again his sugar level was in the 60 to 80 range, so the fire ,and EMTs weren't saying much. They were asking me ,If he's ever had a heart attack before? And @48 ,I said no. I saw he was tracking his heart rate on his phone - 3 weeks bf he passed. But he never told me. Again, I think he knew. And his sprit left his body,Before the Emts came in the house. I knew he was gone. I prayed 🙏 they could get him back. But the Heart machine they had on him . Said DO not shock. He had a low heart beat, My CPR worked. But, knew deep down he was gone. ' Ty for helping others understand 💓 ' Rest easy Travis West 🙏 Connecticut, USA 🇺🇸 I miss you.
Beware, there is a new form of DKA called euglycemic DKA (normoglycemic DKA) which happens rarely if a T2D patient takes an SGLT2 inhibitor. This DKA is characterized by normoglycemia i.e. BG < 200 or 250 mg/dl
