  • Multiple sclerosis is a neurological disorder that affects the myelin sheath of the neurons in the central nervous system. This video will discuss the pathophysiology of multiple sclerosis, signs and symptoms, nursing interventions, and medications used to treat MS.
    Multiple sclerosis tends to affect women more than men and tends to occur between the ages of 20-40 years. It is an autoimmune condition where the immune system cells are actually attacking the myelin sheath on the axon of the neuron.
    What is the function of the myelin sheath? To protect and insulate the axon so nerve transmission can occur. However, if the myelin sheath is damaged this causes inflammation and scarring of the neuron, which will lead to a decrease in nerve transmission. This is why sensory and motor symptoms can present in this condition.
    Signs and symptoms of multiple sclerosis vary among patients because lesions (demyelination of the myelin sheath) can occur in various places. Therefore, typical signs and symptoms include: fatigue, tingling/numbness of the face and extremities, speech issues (dysarthria), coordination issues, dizziness, vertigo, vision issues (blurry vision, dark spots in visual field, nystagmus, pain when moving the eyes etc.), tremors, muscle spasms, bowel and bladder problems along with specific signs such as:
    Lhermitte's Sign: this is where if the patient moves their head and neck in certain positions it will elicit an electrical shock sensation down the body. Uhthoff's Sign: experiencing extreme heat will make MS signs and symptoms worst. Romberg's Sign: the patient loses coordination when closing eyes in a standing position (their body becomes unaware of its position in space).
    Multiple Sclerosis nursing intervention include: safety, educating about preventing worsening of symptoms (avoid heat, stress, infection, and overexertion), bladder and bowel education (self-cath, high fiber diet, 1-2 L of fluids, skin care), etc.
    Medication used to treat multiple sclerosis are based on the patient's signs and symptoms (there is currently no cure for this disease). Medications include: Beta interferon (Avonex, Rebif), corticosteroids (for relapse of symptoms), Oxybutynin and Bethanechol (bladder issues), Bacolfen (spasms), Propranolol (tremors), etc.

    I am a nursing student in Jamaica currently on my final of 4 years in university. I know america does NCLEX but here in Jamaica and the Caribbean we do RENR (Regional Exam for Nurse Registration) done only by nurses doing a BSn and these videos I've watched since my first year and you have certainly become my study partner.     Thank you very much for this video. I am currently waiting to have my MRI done and this concise, informative, and easy-to-follow breakdown has not only given me me more information than many of the pages I went to, it helped me see the inter-relationships of what I thought were unrelated aspects but may actually be related to each other. Now I can take better notes to share with my doctor which makes me feel far more confident in properly identifying what is going on with (even if it's not MS). Again, thank you very much.

    Thank you for your well organized "lecture". Incredible job.
    I have been caring for my son for over 10 years. He was diagnosed with MS 22 years ago. He has advance primary progressive MS - and medical staff thinks they know everything there is to know about MS. But they don't. Everyone's MS is different for everyone. PPMS is much more complicated than RRMS.
    His neurologiy team is extraordinary - But, I am so tired of nurses sending home care sheets on MS 101 that is so insulting and not relevant. I am so tired of arguing with paramedics when he is having a crisis. I am so tired of people we barely know telling us how stupid we are that we have not used the WAhHs diet. ACtually, we have. Dr Walhs lives in the same city, but, my son did not meet the criteria for her study years back because he could not longer walk.

      The one thing she didn't mention is that M.S. goes away when you are pregnant. They don't know why. I am 64 and have been fighting M.S. since I was 15. 49 years now and I will continue to fight it. Prayers and good luck to you

    I am so great full this information my mother had been diagnosed fibromyalgia and ms and I was diagnosed fibromyalgia but I knew it’s something more I confident it’s ms that a lot symptoms relates to that I thankful that bowl and bladder effectiveness of ms which answers my puzzle on that I previously called it IBS which was not seen related to fibromyalgia alone I also had worsened headaches and esteem stiffness and swallowing at time bloatingness stomach acid flux

    I'm learning so much about this. I'm not for all this symptoms idea because I have many. I have most of these..and still try to maintain. I'm not for a take over of my being..yes a shower time is harder since this happened.

    Dear MSers, i have MS and my case is moderate but it's characteristic by major anxiety that is crippling, body also feels weak and heavy fatigue/mental fog, balance issues and all of that but i finally tried the diabetes drug Metformin out of desperation, i took it on my own (750 Extended release, some days i take 2, I monitor my blood glucose levels and it's fine, no hypoglycemia, sometimes i feel lightheaded), I've been using it for 4 months and people everything is tons better, movement, energy, fatigue, lowered anxiety......everything is improving, it's still up and down but the overall path is UP. i hope this message help sufferers because it's a debilitating disease, peace and God bless
