at 10:09, I think you weren't healing as much as expected on Veigar because he already had ignite on him! the grievous wounds from it would have reduced your healing a bit.

I'm willing to bet that the Graves went for the Krugs in a weird way or was seen while kiting them around.
Edit: Graves is seen by the incoming minion wave at 1:24 and goes for the camp anyway.
