18mg to 36mg - CONCERTA XL VLOG - I effed up and ran out of medication!

    I'm Emily and I had a late diagnosis of Inattentive ADD (a type of ADHD) just two months after my 30th birthday. Finding out I was neurodivergent was scary and exciting, but now I'm ready to start treatment!
    In this video, I start to vlog my fourth week on Concerta 18mg, talking about my side effects, benefits and improvements, and then I start taking 36mg on my doctor’s advice. Unfortunately, I ran out of tablets and had a bit of a meltdown! Hopefully next week my prescription will arrive and I can start 36mg permanently.

    Concerta XL is a prolonged/slow/long release stimulant medication (its scientific name is methylphenidate!) used to treat attention deficit hyperactivity disorder (ADHD). This medication is sometimes called Ritalin, Daytrana or Focalin - particularly in the US. Other ADHD medications that you might be familiar with are Vyvanse, Dexadrine and Adderall, which are amphetamines, rather than methylphenidates, however, they are all stimulants. This video shows my 2nd week on the drug and highlights some of its effects on my mind, mood and body.
    I will be continuing a vlog showing my experience with ConcertaXL as I test out my current dose and through the titration period. (Started on 18mg, now taking 36mg).

    You are not disappointing us, Emily. This is all part of your journey and we are glad that you are sharing every bit of it with us. That includes the good, the bad and the ugly. You are being “real” and transparent. I think others will agree, we would rather you be exactly as you are than to think you have to put on a show for our sake. I hope that you are feeling better and I can’t wait to see you when your 36 mg kicks in! You will have “down days”. That’s normal with ADHD. 54 year old ADHD female here. On my down days, I give myself a pass to be lazy, if needed. I will get a manicure or pedicure or just watch Netflix all day. I use it a self care day. I’ve spent so many years of my life beating myself for being how I am that I just decided to stop doing that for things outside my control. It took a long time to figure this out but I started watching my moods, routines and cycles. Keep your chin up. I find you very real and interestingly comical. 😊

      Are you recently diagnosed? You should like you are quite at-peace with your diagnosis and know what it is you need and when. I hope I can get to that level some time soon :)

    What was scary for me is that I wasn't sure what diagnosis I was seeking. I told my nurse practitioner that I'm seeing now about not ever being able to change, doing things without thinking, forgetting words especially when I need to appear thoughtful, probably a lot of other things too. She said "have you ever thought you had ADHD?" I said "No, I don't think so but I'll try the treatment. I always thought having ADHD would be awful." So yeah, I had like zero idea, I was an idiot 😅 I'm 40, lost so many jobs because of these symptoms. It really sucks though. It took a lot to come to terms with it

      Thanks for sharing - I totally empathise. Coming to terms with it is a challenge in itself, let alone treating and dealing with it post-diagnosis!

    I always tell other people happy Saturday and Sunday on the weekend I'm in a group home

    I'm not sure but I think a lot caffeine is bad to take with certain adhd meds. Sometimes it can make them not work.

      I think because they are both stimulants it can leave you kind of anxious and jittery. I'm trying to keep it to one cup a day!

    Just watched through your vlogs, thanks for sharing. It seems like maybe I started on my meds a week or two after you did, so I'm still on 18mg. My naps were also reduced a lot, maybe i've only had one or two in these past three weeks, as opposed to like every other day, haha. Overall it doesn't seem to help too much, though. Maybe I'll get put on the 36mg after the next visit. I did kind of work out that coffee, well caffeine, is like amplified for me, but it doesn't happen until after the concerta wears off or something. Before I would drink 2 cups a day. I dropped it down to 1/day when I started concerta, and would sometimes feel a little weird at times, but occasionally I would have 2 cups, and right around when the concerta would wear off I would get that over-wired "I drank too much coffee!" feeling... Also happened with chocolate, ate like a bunch of my valentine's chocolate in one sitting and got that too. So now I'm no caffeine, or chocolate (which has caffeine), but feeling normal now...

      How is your Concerta going now? Have you increased your dose yet?
      I've cut back to one cup of coffee maximum a day. In fairness, I don't drink coffee at home, just as a "treat" when I'm out and about, but I definitely felt that amplification effect you mention. Interesting that you mention chocolate; I've not noticed that, but then again, I don't generally eat lots of chocolate in one sitting. Tea is my kryptonite! Minimum 3 cups a day...! Stereotypical Brit.

      This is interesting. Recently been diagnosed and been on the 18mg of Concerta for a few days. I find it totally different with or without coffee, particularly proper bean to cup coffee. Without = feel very chilled and like I need a coffee to give me a bit of zing. Then when I have a coffee it goes the other way for a while. Generally feel quite a big interraction

      @@adhdemily I was brought up to 36 last week, before that I was pushed up to 27mg. I'm seeing some positive side effects from the meds, but I'm not sure I'm seeing the main feature yet. It's been a little difficult to judge, because I've also been sick and taking prescription cold meds, so it's been hard to really give it a fair chance. The cold subsided and I finished up the cold meds the other day, but now there's another cold going around our family, and it seems I'm catching that one too, argh! ... But as far as the positive side, generally feeling less frustrated and more calm with things and just feel a little bit more positive over all, I don't feel like napping during the day, and food cravings are less and easier to ignore, so I'm not snacking a bunch or eating huge meals... and I was experiencing that on the 27mg too, maybe it's a little more pronounced now... wondering if my dosage should be increased or not... I've seen some on YT say it's been a night/day difference when they find the right amount... so I don't know...I know everyone will have their own experience, so maybe I shouldn't expect that... Anyway thanks for your videos!

      @@lauren_jo yeah it really seemed to amplify the caffeine for me... once I stopped caffeine and got bumped up to 27 and now 38mg I'm no longer sluggish, but I'm feeling a lot more calm, less frustrated, and more positive in general... still doesn't seem like i'm feeling the main feature of the medicine though, maybe a little? hard to say

      ​@@fretnoize Now the Concerta has really settled down for me.. I'm still on 18mg and the main benefits for me are also all what you described. I was expecting just an increase in concentration. But the most amazing ones have been a calm mind, ability to relax, and not feeling constantly angry or agitated. I can now drink 2 or 3 coffees with it no problem .

    36mg here aswell.. does calm me down alot but I also get that small "had a beer or two" dizzynes/sleepyness.. been on this dose for a month now. Dunno... This helps me to not get so overwhelmed all the time, but don't like the dizzyness. Might ask if I can try dexamphetamine next. Certainly don't want to raise my methylphenidate dosage

      Hiya! Yes, it's 100% like a very low-level, drunk feeling, without the unsteadiness.
      How are you getting on with the 36mg now? Did you change to the dexamethamphetamine? I hope you've found something that doesn't make you dizzy but still helps with your symptoms.

      @@adhdemily I'm having my 36mg as 2x 18mg. I'm now taking them like 1-5h apart so the "comeup" is softer and the effect lasts longer. Still getting dizzy especially if I take the whole 36mg same time. For me, even if I take them 5h apart the 18mg is enough for me to manage my emotions a bit better, and you kinda get used to the dizzyness after a while. Haven't tried the dex yet.
      Edit: mylan has a methylphenidate that (if 27mg+) you can split into two if needed. That's kinda the cheapest option if you have that in your country

    Hi I’m 19 and I’m Currently on my first ever day of concerta xl 18mg. I feel somewhat euphoric but I also feel quite tired. Is this something you ever found whilst you’ve been on concerta?

    Im noticing you're mixing concerta with caffeine, that might not be a great idea.

    That is so weird. You took 2x 18mg and you felt weird, but when you took the 1x 36mg you felt amazing. Do you know why that is?

      I have no idea! Maybe something to do with the way the drug is released? A pharmacist would probably be able to explain it!

      My experience is I feel really good on 1x18 or 2x18 but 1x36 is really shit on me
