When I partially did the bloody finger questline by accident and even stumbled upon the final quest progress due to my drive for exploration before leaving Liurnia I faced what is imo a postgame boss, with 20'ish vigor. *8 hours later resorting to every trick and tactic I could think off* - Welp, that was ... a experience.
Aye. I myself had done the entire Ranni questline before even stepping foot onto the Altus plateau. Basically every move of Radahn's was a one shot for me, lol. Also I hadn't bothered to fight Adula at the Three Sisters so he had full health when I finally did at the end there.

Not entirely. He still does less damage and has smaller hitboxes than he did on release. The latest patch just corrected the damage calculations because apparently they "over" nerfed him.
