He really misses his brother and Claire and wants to love his family. But the kitchen distracts him from all that pain. There's a conversation in the show about how something can hurt so much that you don't even feel it until later. That goes for both physical and emotional pain. He hasn't really accepted Mikeys death yet. It hasn't hit him yet. But I have a feeling it will, and it will hit hard. The show wants you to think it's about the success of the restaurant. But it's actually about Carmy learning to come to terms with his emotions so he could love and be loved. The restaurant isn't "The Bear". Nor is it Carmy. "The Bear" is the pain in his heart. The weight he carries.The pain he has to face and conquer. The Bear that is caged on the bridge. The Bear is a pun. It's a play on the word Bear as in bearing responsibility or bearing pain stoically.
