In the book, she confronts don vito about it and she even said " do you think you married a fool?". Also she washes the blood of don fannuci from don Vito's clothes. She knew and eventually accepted it. There were times when he used don Vito's influence such as when she help Signora Colombo (the one being evicted in godfather 2 where vito helped). She even had a rule not to discuss it on the table and she was there when don vito and sonny was shot.
I'm pretty sure she knew from the moment she accepted the rug after Vito helped Constanza hide his gun. The mafia existed in Italy and had basically overrun the government at that point so she would have known about it growing up. We see examples of how different the mafia is in terms of power when Micheal is exiled to Italy. What was happening in America at the time was almost childsplay but was also up for grabs. This is why Vito was able to snatch it for himself, he had been through worse as a child and therefore learned from the best.
