About: Salvatore "Sammy the Bull" Gravano is a former underboss of the Gambino crime family. He is known as the man who helped bring down John Gotti, the family's boss, by agreeing to testify against him and other mobsters in a deal in which he confessed to involvement in 19 murders.

If you are a part of any kind of ciminal organization, they don't really care who pulled the trigger ; They're always after the one who gave the order. Of course the one who pulled the trigger will get punished but not as much as the person who gave the order. As far as i know from reading and watching about his life in the mob, he never reached or promoted to the boss postion or some one who's giving orders to have others killed and i guess thats how he got away with the 19 murders
