Well plenty of pencillers have been co-plotters, or in Steve Ditko's case the main plotter with Stan adding the dialogue. Not sure how many Letterers ever wrote scripts, although plenty of cartoonists who also write do their own lettering. ;)
It would be interesting to see a comic with the letterer as the scripter/co-plotter, and the penciller and inker as co-plotters.
