When I was in biochemistry I course, one day my professor gave us a question. The pt swallowed around 250 tabs of aspirin 2 hours ago. The pt started having respiratory alkalosis with pH in ABG still trending up. Since the gastric juice is acidic, there were still a lot of undissolved aspirin tablet in the stomach. So the question guided us step by step to increase the gastric pH to above 8.5, ionizing the aspirin and use gastric lavage to remove the dissolved aspirin, which was just fantastic.

Ok, first off this video was great. Second, I have to leave a comment because I was so surprised by both an error you made in reference to medication, as well as leaving out two major points bout Salicylates in regards to poisoning. Do you care to know what it is I'm referring to above?

I just came from ChubbyEmu channel about a 16 year old kid that rubbed a whole tube of Bengay cream on his crotch and groin and got salicylate poisoning from it. All the things mentioned in this video happened to the kid, but he did live. I think the kid got to stage three. Oddly though the kid had polyuria at the start of clinical presentation. I really don’t know why yet but I’ll try to figure it out. ChubbyEmu didn’t mention how it was treated so that’s why I am here.

While I love ChubbyEmu's videos I wouldnt put much stock in the hypothetical patient's treatment and prognosis. He has stated before that sometimes what he presents as one pt one case is actually several lumped together for thematic effect as well as tangential but interesting asides. I remember one video where he mentioned an incredibly dangerous situation with a pt and the video ended with him making "a slow but full recovery." Hunted down the exact medical report of the pt and let me tell you it was nowhere near as "entertaining" as ChubbyEmu made it. Lets just say the pt didnt leave through the front door like ChubbyEmu said...
Still love his content and find it very entertaining! I see it like an opposite House MD. More medicine. Less drama -- just enough to keep you on the edge of your seat!
To hazard a guess at the polyuria:
pt's mitochondria failed produce enough ATP due to early phase salicylism. This initially causes an epinephrine spike which secretes glucagon thus mobilizing glycogen reserves. At early presentation pt likely had enough reserves to provide initial hyperglycemia. Given the metabolic derangements rhabdo is on the table. I cant find much in the way of rhabdo and polyuria but I think thats because Im not trying hard enough! I might go check out Medicosis's video on rhado.
Anyway the hyperglycemia can absolutely leads to polyuria and I would hazard that as the root cause. Mostly firing from the hip so if you or anyone have insight Im all ears!
