VIDEO INTRODUCTION: On 12th of January 2022, the govt of India rolled out a revolutionary system called the unified health interface under the Ayushman Bharat Digital Mission!! and just like the UPI which brought along a payment revolution in India, the UHI is an open network designed to bring out a health care revolution in India!! and just like UPI brought in a game changing platform for fintech companies, UHI is a game changing platform for medtech, fintech, healthtech, Insurance, doctors and even hospitals!! and the irony is that inspite of this being such a legendary initiative, the mainstream media are barely covered it!!
So in this case study, lets do a deep dive an try to understand,
What exactly is the unified health interface,
What is so special about it that it can revolutionise 4 different industries and lives of 1.4 Billion people?
What are the pros and cons of this initiative and most importantly,
and as students of business what are the study materials to help you understand this game changing initiative better?

As a doctor in the top government hospital in the country, I know the problems faced by the patients when they come to the hospital without proper medical records. They get improper treatment done by unregistered/illegal practitioners and come at the end stage hoping that we can save them. Sometimes their medical records are lost in travel and they face difficulty in explaining their conditions to us. Hoping this will change the conditions in Healthcare 🙏

If they had to make healthcare accessible and affordable for all then govt wudve invested more in public hospitals, etc which is the job of any govt in any country. Digitization of healthcare is cheaper than spending on public hospital infrastructure, having said that one can't deny the benefits of efficiencies coz of digitization. While no doubt citizens will benefit from streamlined services but affordability i highly doubt with this govt more leaning towards private hospitals instead of making public hospitals. One thing is for sure, maximum benefit for insurance companies as data collected will immensely help them.

I stay in Canada, which has almost the same health system, I often used to wonder why India can't do this. When you go to the doctor, they just swipe your health card and they can see all your details. Each time they need to get authorized by your card to view your records. Only doctors can enter your health data and only during the consultation. You don't have the access to edit the data. It is a tested system and India will benefit a lot from this.

Compare doctor to patient ratio in both countries
Do you think doctors in india have the time to enter your health data, that too during the consultation visit itself( many times even the patient won't spare so much time)
Doctors in government hospital barely have time to fill the case sheet
And even in the current system, do you know how many people misuse ayushman Bharat scheme , inspite of being economically well off, just to get government benefits

Indian People here prefer going to India in private hospitals for major surgeries as Canada has 8-12 months of waiting for critical surgeries. Its a broad topic, search on google about Canada's health system. Only thing India should learn is to manage the data of patients, rest India is better despite issues with government hospitals.

I would like to point out that UHI should also have a patient-friendly database of diseases which informs the patient the details of the condition he/she is suffering from which includes risk factors, symptoms, prognosis, drug side effects and various other info.

In all this process documentation and necessary information get lost quite often. I am really happy that government is coming up with such schemes to help everyone get better health services.

Great video, have been waiting for some details on this initiative. So as a doctor who did my undergrad from AIIMS in India now training in US, my feeling is that the biggest challenge is getting this platform up and running with all its stakeholders. We will need computers in all hospital spaces, doctors and nurses trained to interface with this system (its much more complex than paper records due to sheer volume of information)- govt hospitals today are far from this. This will undoubtedly happen but will take time. Once thats done, there is a lot we can directly model from EHRs in US and UK etc, but many things will need to be redesigned/rethought in India’s context. This can often bring gamechanging innovation to the table, as with UPI. Exciting time for sure, will change the face of India’s healthcare in coming decades.

Although a little out of context, here in Nepal, health facilities are a mess. Whenever you go for any health treatment, you are almost certain to be asked for an X-Ray. Furthermore, even if you did it earlier on the same day somewhere else, you must do it again at their facilities or the specific place they recommend.
Around a year ago, I met some people working on a similar start-up idea, but none of the health institutions were willing to share the data and resources with others.
However, things will be more accessible and mainstream if the government decides to step in and implement itself. So much potential in the project that can be a game-changer in the health and related field of India! All the best!❤

Under this act, government has launched an app with excellent user Interface name Eka Care. Through the app, one can check their pulse rate on the go with their smartphone. It also has Various questionnaires for the user to answer, which us useful in ruling out Various medical condition. My suggestion is that An ecosystem must be generated in hospitals where Doctors can easily jot down important findings and diagnosis of the patient along with the list of medications and their dosage etc hazeless. It must be doctor friendly, so that doctors need not waste time entering patient data which otherwise they could have writen in seconds in patient's notebook!

The data stored in these systems is very helpful in the process of 'drug development' for disease with no cure till date or to create a drug which is better than the existing 'standard of care'. We used to leverage datasets from the US for FDA approvals and Japan, etc. among others countries for aporovals from respective regulators. The application of this data during the new drug development phase range from patient selection for clinical trial, clinical trial design, placement of drugs in insurance formularies, physician targeting, clinical trial site selection, etc. With this initiative by the GOI, India will also be able to leverage the data and create a base for these processes to be conducted in India.

I speak from experience for the last 3 years as a startup founder, where they are just doing pilots which are unsuccessful and this problem is very difficult to solve! I feel the govt. is just making it appear as though they are doing work and solving this problem, when actually on ground nothing is happening and there is too much red-tapism for them to be able to deliver. I feel there are some important pointers that you may have missed out on.
One may wonder, the technology to implement UHI is not very complex then why has it taken so long for the government to implement this in the Indian Health ecosystem. Well, the problem is mostly with:
    1. Standardization of medical data: Every HIS (Hospital Information System) follows a different kind of standard to store EHR/EMR (Electronic Health Record/Electronic Medical Record), depending on the software vendor they might use an American standard/European standard or their own made up standard. Hence, even today when a patient goes from a top private hospital to another one (lets say Fortis to Medanta), since their HIS are different the data cannot be synced to each others systems. Hence, it becomes a manual effort again for the Doctors and staff to re-enter and sift through this siloed data. Now, who shall set this standard? Till now the politics of Healthcare in India has been passing on the baton from one agency to the other (NITI Aayog, DISHA act, NDHM etc.). Interoperability is one of the biggest challenges.
    2. Data Privacy and security: Health data is considered more sensitive than even Financial data (imagine if a person is sick and their health data leaks out, how can their boss use that as leverage to not give them a promotion!). In the video you rightly said that a user can give access to a doctor incase of some checkup/emergency. But, then why would you give access to an Insurance company, who would charge you a higher insurance premium if you are not very "healthy"? This system doesn't work well for insurance companies, hence they show they are favouring it, but they know it won't really workout too well for them. New age Insur-Tech startups want to sell Fitness devices to a section of society that is fit and give them low insurance premiums with the added revenue from their fitness devices. Also, as you may be aware, the security of the Adhaar system is a joke! Periodically Adhaar data of Lakhs of people are hacked. Imagine if that happened with the UHI! That would be utterly disastrous for the govt.

I had a similar question - would insurance companies be able to exploit the health history of patients to play around with premium pricing? Would they be able to game the system/blindly deny coverage by simply tagging some condition as preexisting?
This makes me think if this data should even be shared with insurance companies at all?!

I have been hearing since childhood that india is a major IT power. But it was only after schemes like UPI, cowin, digilocker, direct bank transfers, and now this, which actually made a difference in our lives, that i can second the proposition.

I agree to the concept and US has similar programs and registries. Another point, it can also help with the same example why area 1 has so many diabetic patients, is it genetics, food, lifestyle or combinations to run educational programs to manage. If it’s as efficient and effectively made as UPI, our model can beat it. Con side, for some minor reasons they can raise premium and gov will need to set regulations. As always amazing videos. I wish my schooling was so much fun and practical.

I work for a global healthcare system NYU Health. Their NYU MyChart Apps holds the integrity of data but does not interface with financial institution for payment. Rather, the insurance industry take care of this piece. But, even with informed users, patients in new york, still have issue using the apps. Still India will face more challenges due to the diverse uniqueness of India population, regarding universal health care.

Risk and trust are two sides of a coin where the insurance sector runs. The idea of health score sounds terrific, the insurance sector may take undue advantage of it by avoiding underwriting to people with high risk. Even if they provide some coverage as a legal binding from regulator it would really be not sufficient for the needy. Afterall Insurance companies are also business houses (mostly run by Private corporates) are meant to make profit. Think school/Ganesh it would be great if you can enlighten how UHI can overcome this challenge.

Time and again, it has been proved that adoption of new technology is always the better option. The more drastic a change is, the more are the repercussions especially on the less powerful of the society. However, these ultimately flatten out and lead to an improved lifestyle.

As a doctor in govt hospital i think this scheme seems very good on paper but will be difficult to implement as major chunk of population is still far away from health awareness and there bodily problems as u mentioned in video
But if implemented in a good organized manner then can be beneficial in upcoming decade or so

We will be getting a medical score. To get UHI useful for current population we can MARK peoples' accounts with certain TAGS which would help the health care professionals to at least identify that if a certain person has complete knowledge of his medical treatments.

What I love about Modi ji is his will - to keep trying out new ideas and solutions for our country. No doubt there can be failures and in many instances some schemes are very difficult to implement in a diverse country like ours . But the will to try new things and keep trying is what I find admirable in his administration

The efforts taken by our government are literally underrated. The way we see things happening in Ukraine, Russia, Turkey, Iran, Pakistan, China, UK and USA these days, Bharat seems to be a better place to live than these.
We aren't in a hot conflict, we aren't debt trapped, we aren't stuck in COVID, we aren't facing a recession, we aren't having a civil war. It's comparatively better and safer than above mentioned countries.

I nearly wept at the end of the video. It brings me so much joy to see that the government is ACTUALLY doing something, but as with all such schemes, I'm concerned about foolproof compliance with the UHI's standards, such as the doctor verification part, because we somehow have a knack for finding loopholes and exploiting them while not providing the services or goods required, and corruption in the agencies that maintain the systems behind the entire UHI network. I know my statement is vaguely worded, but I hope it makes sense. Corruption still is a big part of not just our politics, but our society as well, and that makes me worry a lot about proper execution of policies. One example - though unrelated - being the heavily increased fines for traffic rule violations, with traffic police stopping motorists and handing out these heavy fines on a whim, with the judicial process to appeal these fines being long and punishing in nature.
Other than that, it's *amazing and touching* to see our country lead the charge in the healthcare sector with this initiative!
