      sadly many patients take doctors for granted. it's a thankless profession. on top of that the IRS taxes them heavy. Then they gotta pay off all that debt, they never see daylight. trading hours for fiat currency as income. is it worth it? I guess it is in the eyes of someone willing to give their lives for years to help the sick.

      They should not be heroes. Healthcare systems should invest enough money to hire a proper number of doctors and avoid this struggle about working in understaffed conditions. About US: they should increase the number of residency posts too but I doubt that it will ever happens considering the healthcare system you have. I would not be touched by a sleep deprived doctor. Honestly I don't want to waste my life for just a system deficiency.

    Residency is such a rollercoaster - sounds like a super busy night in the ER for you! Kids and airways management is truly the most terrifying thing I can think of overnight! Also just noticed 300K subscribers!! Congrats Jamie, so well deserved!!

    I definitely felt the same way. I was thrown into my first trauma the other day since not enough EM residents were working the shift, and I was freaking out! Thankfully, all the surgery and EM residents were super helpful and understanding. haha I probably looked terrified

    What's important is that you help save human lives, and that is a very precious thing indeed. Being a doctor can be a very tiring and, at times, scary profession to follow. But at the end of the day, your patients' well-being and your aid to them is what matters the most. Keep up the good work, I'm sure you'll be an amazing doctor really soon. In fact, you already are. We believe in you ❤️❤️

    The one tip that I have is to always remember that tough times don't last, but tough people do, and when you're able to get through certain situations and allow your soul to be at peace, you will get through it all. I have a problem where I become detached and lose my empathy in some certain situations and that is the one thing you don't want to do RELAX and the ability to realize that once the problem is fixed is better to forget about and move on, than for it to torture your soul. Dr. Lee, we believe in you and we know you can get through this!

      I work night shifts at my hospital and I feel THE SAME WAY! I just don’t eat after night shifts because it’s so much work to eat after 😂 You got this!!!

    There is a special place in heaven for emergency medical staff. I spent many nights at the hospital with my little brother when he had asthma attacks - I was always so grateful for the attention and knowledge of the doctors and nurses in the ER!

    you’re so hardworking! I was actually wondering how you coped so well with trauma victims and if it affected you at all, because you are so mentally strong and stable! I found it very comforting to learn that you still take the time to talk to people about scary experiences at your work

    I'm with you. Had my first super psychotic patient (as a first year Psych resident) last week and it was the first time I was a bit scared for my safety (and for the patient's). It ended up being ok but talking about it with my supervisor/mentor really helped, as did talking with family. We got this.💪🏻

      Gisele Toumi I’m loving it!! Honestly it’s even more rewarding than I thought it would be. It also fits my lifestyle a bit better- I’m continually impressed by Jaime’s ability to go between nights and days so seamlessly- I don’t think I could do it! I may make a video soon. Never made one before so not sure but I think it may be helpful for people to see what a Psychiatry residency looks like!

    Thinking of you, Jamie. At the end of PGY3 year when you look back on moments like that night, I think those will be the nights you grew most as a physician. Hemoptysis is scary as it is in adults - I can't even imagine being faced with the possibility of doing a cric on a child. Glad that you were able to talk things over afterwards with your attending, your friends, and your boyfriend. Sending you good thoughts. ❤️

    As a doctor who already have finished residency I must say that this feeling of felt that you are definitely not prepared to take care of people, especially those who are very debilitated (as the little girl with hemoptise), is not something that will disappear with time. Actually, if you’re a good doctor I think that you should keep this feeling with you, because it’s make you remember that you have to up to date your knowledge all the time. Doctors are scientists, and being so we are in constant evolution... and remember the new scarring a lot, but get used to a situation should scared as well, because this means that is time to get out of your comfort zone! Amazing video Jamie! Congrats!! You really inspire me a lot! 😘

      Seriously? Doctors are Scientist?
      Some doctors are scientists-just as some politicians are scientists-but most are not. As medical students they were filled full with information on biochemistry, anatomy, physiology, and other sciences, but information does not a scientist make-otherwise, you could become a scientist by watching the Discovery channel.
      A scientist is somebody who constantly questions, generates falsifiable hypotheses, and collects data from well designed experiments-the kind of people who brush their teeth on only one side of their mouth to see whether brushing your teeth has any benefit.
      Most doctors follow familiar patterns and rules, often improvising around those rules. In their methods of working they are more like jazz musicians than scientists.

    Wondering if you could please do a video on having a relationship in med school and/or residency?? Is your person in the medical field? If not, how do you get them to understand the demand of your job/schooling. It seems like residency is more demanding than med school... how are you handle work/life balance with a relationship?

    I currently work in the ED, and I have to say my biggest coping mechanism are my coworkers. The physicians, nurses, techs, everyone. After heart breaking cases or stressful situations, we’re all able to come together and find humor amongst each other. Laughing is definitely what gets us through most days lol. I’m glad you’re finding your own ways to cope ♥️ it’s definitely super important in the EM field!

    Im from México, my boyfriend was an emergency resident but he just quit last week he worked 100 hrs x week it was too tired, here we need more regulation on the requirement for residents. Im a doctor too, just started to work two weeks ago, 12 hrs x day , medicine is a demanding career, and sometimes when you dont have the work experience you feel like you want to quick, that you are not good enough, there are some more experience doctor that judge you.

    Totally understandable. Sometimes you just need to take time to let it all out and be free of whatever the problem is and it is great that you have people near you to talk with. I think it is both a blessing and a curse for us in the medical field. I hope you get through those days well.

      Jake in mexico we do 36 hrs shifts every third day haha, nonstop as an intern or resident

    Jamie, pretty new to this channel but, just want to say thank you for your vlogs! I'm only a pre-med right now stressed about the future doctor life but I really enjoy your videos!
    Showing the difficult and rewarding sides of being a resident really inspires and assures me that this is really what I want to do! A lot of the worries that I won't have time to "live" my life is something that you show in your videos to be false, so thank you. Your ability to balance your career and home life as well as your method of self-care is amazing!

    I must confess that even for me the hemoptysis scares me (I'm a pneumologist and I have a sub specialty in right ventricle and pulmonary circulation) and although I see cases 3 to 5 times a week, they never cease to impress me. If you need more information about hemoptysis, or pneumology in general, I can send you bibliography.
    I encourage the first year of residency is very hard, but I promise it's worth it.

    Hi, this is my first time commenting but I've been watching your videos for awhile. I'm starting the first year of my undergrad degree this fall and I love watching your videos and I appreciate you so much for taking the time to create these videos because watching them just solidifies my resolve to go to medical school. While the work and the sacrifices you have to make in order to make it to this point are grueling it's so inspiring to see someone living my dream. While I'm not looking forward to 12 hours shift 5 days a week, watching you go through this process and develop in your carrer and your channel gives me the hope that i can do it too, so thank you.

    I so appreciate your honesty here. I’m about 6 months into my new career as a marriage and family therapist and I’m still battling feelings of inadequacy and struggling with imposter syndrome. It’s tough to be new in fields where the care of others is the focus. But I remind myself that every experienced therapist started out just like me. You got this!

    Hi thanks for posting this video. I saw a case with a young child with massive haemoptysis when I was an intern on paediatrics and your video took me back to that time in 2013. I did go home very shaken and I felt very insecure.
    I'd like to also endorse what your attending said, the feeling of inadequacy is something all doctors experience and it seems to pop up through the years even though your skills and knowledge increase. There will always be that case that serves as a humbling experience no matter how good a doctor we become. I've seen my seniors cry and hit low points on the job from mistakes they made. So it's especially okay for someone who is at the beginning of their journey as a doctor.
    You have a great attitude and positive outlook and that will help to get you through the tough times to come. Keep up the good work. I really enjoy your vlogs. Thanks for sharing!

    Just began working as an ED scribe. I also have the overnight shift sometimes and find myself completely exhausted afterward. I can't imagine how tired I would be if I were the doctor in the situation. You rock for this, Jamie! Keep striving!

    Working out, meditation, or yoga can help with stress and problem. I seen an ortho Surgeon Dr Webb (RUclipsr) doing yoga and he works out at the gym.

    I quit medical school at the end of my penultimate clinical year because I can't take the pressure of the system (including the responsibilities as well as the learning involved). Some may beg to differ, but I think the system was set up such that doctors are not humanely treated enough and are constantly burnt out. Good on those who have managed to have that stamina and mindset to deal with the stress, especially physical stress. They are the ones who should be the doctors (in the system that has been set up). For those who are looking at seriously getting out of medicine, it might be a good thought, especially if you don't have that stamina to soldier on for many years to come. If you can't take care of your health in the long term, it's not worth the fight, even if you need the money, or better - even if you love the profession.

      Most people can’t afford to quit when they’ve gotten so far into medical school. They’re probably neck-deep in student loans already, and the only hope they have of paying it back is to become a physician in a high-paying speciality.

      Exsanguinate yes, only those who can withstand the stress. Kudos to them. Otherwise hospitals can’t hire someone who are in such a bad shape they can’t fulfil their duties properly.

    Residency seems so scary but you are handling it like a pro. I love watching your videos!
    I can't imagine how you're switching between day and night shifts. For my first semester of M1 I have 9am lecture every day and I CANNOT get my sleep schedule in order.

    The medical field is truly a draining career. I'm an RN (a new RN) on a very busy floor working the night shift 7pm-7:30am. So I relate to you 100%. Because it can get so busy and the acuity of the patients are so high, you do start to question your intelligence or competency. I'm really lucky I work with a great team so I'm not feeling alone on the floor.
    it's also so important having a great support system for your venting days! This is my second degree, so I do have days when I question why I entered this field. That's when we go out into the city with my friends grab some food, have a chill day and I feel better. =) you got this!

    I don't know if you'll read this but I'm a PhD student from Spain (totally non-medicine related) and ever since I started following you a couple of years ago your channel has been such an interesting place where I (oddly enough given cultural and vocational differences) often find comfort in the familiar. Maybe because at the end of the day, both a medical career and a PhD can be very stressful and lonely yet incredibly rewarding paths. Maybe because I too have a quirky talkative cat 😋. Whichever the case, I'm sending you lots of good energy and a wishing you good luck in everything you do. Keep up the great work!

    Thanks for sharing the difficult side of Medicine. As a doctor myself, I feel it's important to talk about both good and bad side of it and not just sugar coat things. It can be so draining. And people outside of the profession usually dont get to see the hard reality

    I did not cook at all in my intern year. I studied, prepared for morning rounds and worked and ate at the hospital. Some things had to give but second year was better. This is different season in your life and learn to love it and give up somethings that you love because you can get to those things later. good luck. enjoy your content

    Thank you Jamie for sharing this. I’m a 5^ year medical student in Italy ( med school here is 6 years long) and i’m starting to think about residency situation and all the responsability and things you have to manage. I think that the worse thing in medicine is that you have to deal with a LOT of pressure , stressfull things over and over and the tought of being inadequate to the all situation. I personally feel unprepared about managing a patient right now , but i also know that everyone or almost everyone felt like this at this point so..it kinda conforted me.. I wanna ask you if when you were in med school you had somekind of very down moments or felt overwelmed about the amount of things to do

    Emergency medicine, like many fields in medicine, is one field that is experienced based. Reading/studying material in books helps but actually applying it to every case which can be so different from every other case is where the hard “unprepared” feeling comes from. You will do amazing because you have drive/passion, but it will take time and experience. :)

    When I was a tech in the ER we would do 7p-7a one day and then 7a-7p one day after and then 3p-3a the next and my sleep was always a wreck! I didn’t know about the difference in hour capping for EM. I can’t believe how far you’ve come! I’ve been watching since a couple months before you were Step 1 prepping and I wasn’t even in med school. Now you’re a resident and I’m a second year! I can’t imagine the transition between med school and residency.

    Damn, I feel like I'm a psychopath now! I'm a first year resident too but in Egypt and I get worse cases than hemoptysis and totally on my own, here the attending isn't in sight, I'm left alone with the nurse to deal with the cases that come in! I get death sometimes and I have to confirm death, write the official paper and finish everything without the attending ever coming or seeing the patient! But I don't feel shook tho, that's the strange thing, I'm always kinda emotionally numb!

    I'm 14 and I want to be a dermatologist and I think when I will be in my residency its not gonna be so hard, because I will put my heart in it and I will be just in a great relationship in my work.     Could you do a video if you haven't already, on how to do that transition between night and day shifts?
    Or how to get used to it/best way to do it to maintain your health, getting enough sleep, break, etc.

    Im so glad u had support it really helps alot. Unfortunately in some countries we dnt hv it. We usually as an intern or wat u call a resident, we learned on our own as there were to few senior doctors, u can call ur consultant bt they would jus talk over the phone which to be honest is just more stressful, bt atleast we had amazing nurses and sisters who would hold ur hand through the resuses and intubations. I only started getting great support and teaching in the Family Medicine dept of the tertiary hospital. So shout out to Fam med physicians who run the emergencies units and basically everyting else no one else wants to do. Wen ppl hid frm covid Fam med was in front line EC unit and Internal med so big up to them. Goodluk wit ur residency and always remember ur a physician in training its ok not to knw and its ok to make mistakes be blessed. PS after 14 yrs i still ask for help🏵

    Wow 60 hours/week is a lot of time at work! And you have overnight shifts! Working graveyard/overnight shifts is also very difficult on your physiology (Night shift workers have higher rates of cancer). I never really understood why they have shifts that are 12 hours instead of 8 hours. It's probably has something to do with handoff being a time when patients' safety can fall through the cracks. It would be better, though, for providers, if they were able to work shorter shifts and not have to work all night long - e.g. swing shift and then early morning. Also having a regular schedule (instead of rotating shift work that changes from week to) is less disruptive to circadian rhythms, melatonin suppression, and seems to reduce the incidence of hormonal cancers for women. Thank you to all of the hospital shift workers that provide high quality care at any time of day or night!
