I don't know if you remember the theory that Megalovania is the theme of Frisk/Chara rather than the theme of Sans, because of the implications of the word "megalomania" (hunger and desire for power) Well... In this case, "Theomania" refers to the belief that one is a god, or that one is possessed by a god/is the herald of some god. In this case... I feel like it would look good with Frisk in underfell, being a rotten world because of the "kill or be killed" law. Frisk, possessing the power to reset, and basically being the master of the timeline, sees themself as a kind of God during the genocidal route, with every right to judge monsters with the death penalty by their sins. Frisk would be the theomaniac, and Sans, who judges them, is in fact being judged at the same time by the eyes of that child who, drunk with power, feels like a deity.
