I have been reading Berserk for the past 10 years, and Miura's artwork and story has always affected my profoundly. A story of humanity and it's flaws while showing such beautiful artworks that left me staring at the pages as if I was pulled in and trapped in them. Miura will be missed, and I can only hope that he had passed on without any regrets.
Yeah, and it seems to be quite a theme for loads of mangaka out there. I mean Eiichirō Oda has been working on one piece for over 20 years now and it has been the bestselling manga for several of those years, but there is still only a handful of images of the guy. Same goes for Yūsuke Murata, Yoshihiro Togashi and several other big names. One reason may be that most of these people work unfathomable hours on their manga. I mean Miura has been quoted saying he starts working at 8:30-9 AM and doesn't stop until 11:30 PM - 3 AM with brakes for eating. No matter how you interpret that that is at least 14.5 hours and at the most an ungodly 18 hours, excluding brakes that seem to be quite short. It is crazy. And this seems to apply to loads of these mangaka as well with Oda being known to work for 20 hours a day on several occasion and literally being forced to take brakes by his editors so as to not work himself to death. With a schedule like that it is frankly not weird there are no pictures of them, they simply don't have the time to go outside hahah. The people creating these masterpieces frankly don't get enough credit for the work they have put in over the years these series have been going.
There's actually one more picture of him. It's of him at a party, holding a glass of beer, alongside a guy that worked on translating some of Berserk into Spanish. It's from all the way back in 2003, so who knows what the man looks like nowadays.

I don’t think Miura intentionally “cartoonified” the characters for Elfhelm, however, I think he knew that he’d have to at least initially simplify it. So he made it coincide with the beginning of Elfhelm to give it a distinct aesthetic when compared to the rest of the series, and by the time the crew leaves Elfhelm, Miura will have finished adjusting to digital and the art style will have gone back to the Fantasia style.
You just have to look at the griffith chapters post elfhelm, the style is basically the same as it were before. Same with the few panels where Caska has her attacks and even when they go see the witch in Elfhelm last chapter. I very much think the change was on purpose awell as being a way to adapt to the new medium.
