USMLE Step 1: How to solve Shock Questions; Up and Down arrows

  • Hey guys, in this video I’ll show you an easy algorithm to approach shock questions of the USMLE Step 1 that involve those up and down arrows, including how to differentiate septic shock, cardiogenic shock, obstructive shock and hypovolemic shock. Hopefully after this video, you wouldn’t get those questions wrong again!

    Thank you so much for this incredibly simple explanation!! I have watched/studied this topic 4 times in the last week and it has not stuck at all!! This Finally makes it so much clearer!!     Could you please explain what happens in cardiac tamponade? Im confused whether PCWP is increased or decreased as its obstructive shock?

      PCWP is decreased in tamponade because the surrounding blood compresses the heart preventing it from filling properly. So blood pools backwards into the venous system ?

      @@andremarques6872 PCWP is a measure of left atrial pressure. Since the heart is smothered by surrounding fluid in tamponade, the left ventricle can’t properly contract and empty the blood. the blood in the left atrium backs up and can’t be moved to the left ventricle increasing left atrial pressure . That’s my understanding, please recheck

      The pressure will decrease bcz all chambers will be be compressed and all chambers will show decrease pressure including the left atrium , thats what says boards and beyond

      Sorry for mistake that was typed
      Pcwp will increase bcz all chambers will compressed from pericardial fluids so this will rise pressure in all chambers

    Hey madam .. can I reach you? I have to discuss my case with you.. I am preparing for step 1.. I am done with anatomy, histology, embryo, physio, biochem and a bit of pharm. Left with micro and patho.. in systems I have a good command over Neuro , renal and Endo .. kindly help me how can I reduce burden in less Time.. I have to give step 1 in 6 months
