I'm sure you know this, but when soft armor is pressed up against a hard object, it will fail occasionally. The extra substance behind the armor increases the amount of pressure on the armor, causing it to fail. They have to be tested on a soft target.

Wow this video has new context after the recent accident/malfunction. Had he been wearing that would the piece from the .50bmg rifle been able to slice his neck and artery? We will never know but maybe. Glad he miraculously survived. It was a chilling reminder that while people always think of protecting their eyes that the neck with all those arteries, wind pipe etc is also a real concern when it comes to protection. In the future if he chooses to use “old stock” ammo he will perhaps wear this or some other neck protection in the event of a catastrophic gun/rifle failure.
