You probably know that players didn't always wear helmets. But did you know that goalies didn't wear MASKS until the 1960s? There's old black-and-white footage online of this. It seems inconceivable, despite the slower pace of play back then.
I believe that they kept their helmets loose so they could ditch them quickly in a fight. There are rules around losing your helmet during play, or removing them for a fight nowadays so you don't see them pop off as easily.

Scott Stevens the career ender...would love to see you react to one of the most infamous moments in hockey history if you haven't already: Scott Stevens flattens Paul Kariya... buddy looked like he died, he was out cold. Then Kariya somehow keeps playing and scores an insane goal and the commentator loses his mind. If you've heard of Scott Stevens you've heard of this hit and the legendary sequence that followed. All of this was in the Stanley cup final too. just crazy

In the time that Stevens played, most of his hits were perfectly legal. They've only tightened up on head contact within the past 15 years or so.

Targeting the head was never legal, especially by sticking your elbow out
