      She should have got you back to the airport and concentrated on your safety and not wasted time. It was painful to listen to.

    I've been a pilot for 14 years, this is the first time I've ever heard "declaring an emergency" and "Confirm you have ATIS information" in that order.

    Door flies off, can't climb, can't see the runway through the clouds, no localizer for that runway. Then when I hear ground say the gear is not down my heart skipped a few beats. I'm thinking what the hell else can go wrong? Well done Captain.

      Loki Yes? The Air traffic controller cannot vector somebody below the MVA= Minimum Vectoring Altitude... She had to have him at 2,600 unless the Air Traffic Controller asks the pilot “Are you able to maintain Your own terrain and obstruction clearance” She can’t help unless he was at that altitude...

    As a recently retired ATC with 29 years at two of the busiest facilities in the world, my number one rule for students handling an emergency was, “Don’t make it worse!” Number two was, “Do everything in your power to help the pilot.” Things have changed I guess but I hope this was an isolated incident.

    Thanks for this post. This was a close call. The pilot did a remarkable job. With the door/stairway hanging down in front of the left wing it would have caused major turbulence causing a loss of lift on the left wing and a huge drag. The pilot probably had to trim everything the other direction to compensate. His tone is not surprising as he was probably fighting the aircraft all the way to a remarkable landing. BTW this aircraft continued to fly after this incident thanks to the skill of this pilot :).

      rkan2 Good question. Haven't heard the details yet if it was a crew error or mechanical problem with the door. Thankfully no disaster due to the skill of the pilot and the plane was back in service soon after. The door hanging out is a great scenario for pilot simulator training.

      The door hinges and the bottom and hangs downwards. They likely didn't have gear down until final because they were having a hard enough time staying level with the door hanging off.
      With the way they hang below the fuselage when open, I can totally understand how it could look like one gear down.

      did you look at the map? looks like there's mountain ranges inbetween. can't clear people to go VFR over a mountain range under recommended altitude without visibility.

      She asked him 3 separate times if he could maintain his own terrain clearance, what else where they meant to do?
      If minimum alt was 2600' that means the tallest point was 1600'
      600ft isn't much wiggle room during an emergancy with poor visual.

I think both, that’s my logic, she didn’t want them descending into unknown terrain that could be 500 feet below, pilots never said they had control issues or a problem maintaining height, it seems like she did the safest thing, I dont get all the hate.

      @@williamweaver1862 - 600' of ground clearance with vectors beats leaving a pilot on his own in IMC with an emergency on his hands because that's what the rules make you do. The rules governing this situation, and perhaps the related technology, need to be changed so that she can still give him reliable vectors at a reasonable altitude even if it doesn't meet MVA. Put another way: 600' of ground clearance is enough in an emergency.

    Very poor job done by controller. She is more worried about protecting herself than dealing with the emergency. 2600 mva is 1000 ft above obstacle, so with aircraft at 2000ft a turn can still be safely made although not "legal". Getting A/C on the ground safely is your only concern not getting a slap on the wrist for breaking MVA. She provided very little help to pilot and was not listening to him. Telling him to intercept loc after he said he needs the freq. Really bad. Asking if he has ATIS? Come on, give him the important info. You can hear in the pilots voice that he give up on this controller helping them. Listened to this with other ATC controllers ( 26 years on job for myself) and all we could do is shake our heads.

      +Josias Rivera Exactly!!! When you declare an emergency, you do what you have to do!!!!! I once had to declare an emergency into EWR during rush hour on a Friday night when I was indicating no nose gear. I took off from Linden (KLDJ), and my gear in transit light remained on. When I tried to cycle the gear back down, I was only getting a down and locked indication on the mains only. It turned out to be a bad micro switch and my gear locked in when I touched down. I apologized to ATC for screwing up the traffic, but they said "No problem, you did the right thing"! When you have an emergency, the safety of your passengers and crew comes first!!!

      Wow, yeah, definitely sounded like the controller was burdening him with unnecessary crap while he was trying to maintain control and fly the damn aircraft - and hear ATC over the noise (which IDK if the recording here could do justice in emphasizing how incredible said noise had to be).

      Vincent H Tan Well, when you're flying a small jet at 3000ft, the cabin door flies off, you have 5 charter pax screaming about being sucked out the aircraft, ATC is telling you to climb to MVA and you are IMC and cannot climb, she keeps asking if you can maintain terrain clearance after being told you are in the clouds, she keeps hinting you are going to hit terrain if you can't climb......if you aren't afraid at this point, there is something seriously wrong with you. Uncontrolled fear is bad, but fear coming from an understanding that you are in a life or death struggle can sharpen your mind and bolster your will to get on the ground safely. It would be helpful if we could go thru life with without fear, but I honestly doubt any pilot put in this situation could say he didn't experience fear at any time.

      One can only hope that he had some terrain display either on his GPS or his iPad and was well aware that he is not going to hit anything yet and what to stir clear of. A reminder for me to always have that information right in front of me, always.

      Its a verbal contract the controller need to establish. Due to nearby terrian or masts the controller cannot vector the aircraft below the safety altitude. Hence why she has to tell the aircraft to acknowledge they are responsible for collision avoidance and navigation below the SA. By law she shouldn't have even given a vector as a service had not been applied.

      @@Typhoon1195 I call BS. The pilot declared an emergency. At that point the FARs are scrapped and it's everyone's job to get the airplane on the ground safely any way possible. However I don't think she did a terrible job assuming we got the entire transcript here. She wasn't in the airplane and didn't know the full nature of the emergency. The pilot had his hands full and wasn't clear so it took a few calls to get her on the same page. Once it was clear he couldn't climb and had control issues she began vectoring him. Could it have been done better? Sure, but I thought they both did a fine job.
      Now who was responsible for the security of that hatch? The PIC.

    I almost choked on my coffee when she asked them if they had the current ATIS. Yeah, of course, it's not like we're flying a juggernaut here with the door hanging loose, panicking pax, probably loud wind noise, that will not climb and oh, we are IMC. So silly of us to forget to check the ATIS, ma'am.
    I know she was in a twist with the MVA, but srsly, not the whole sector is 2600' high ... you're first duty as ATC in such a case is to HELP the pilot, not the pilot to help you stick to your procedures. My goodness. And she said join the localizer, the pilots asked for the frequency (two times!) , and then she said localizer not available? How come?
    I appreciate that I do not know the full story, but what can be heard in this little clip here, is annoying, to put it lightly. But the most important part is that they made it back in piece ... minus the door ... and if they had any complaints about the controller I am pretty sure they told the proper people about it.     Some of the comments on this one are hilarious.
    After watching the video, then reading most of the comments (then watching the video again) I have a few thoughts (I am a pilot):
    1) The lady ATC departure controller did not need to bother the pilot by asking if he had ATIS.
    2) She was obviously concerned about him being too low over terrain. She had no way to clear him for 2000' when minimum clearance altitude was 2600'. What was she to do? (Even though the door fell off!)
    2) There was not initial mutual understanding as to which runway the pilot was returning to. He asked for LOC freq for RW 10 and she obviously hesitated in response. This is likely because that runway has no LOC so her immediate reference lookup showed no results so she likely was asking others to verify no LOC available for RW 10.
    3) She did not offer him any runway except 28R (and did not give him the frequency). I assume this RW choice is to get him over the bay and avoid terrain. Good choice.
    All in all she should have avoided the ATIS question AND assisted in clarifying the runway choice along with frequency. Otherwise good job. Without looking at a terrain chart it seems she did well to not authorize a return to RW 10 over this terrain.

I can only imagine the complications in this, people seem to think she’s controlling everything - meanwhile a separate ATC has planes taking off and another with them on the ground, I’d doubt she personally sets up or can immediately check those beacons, and if it’s automatic based on directional traffic it makes complete sense she could pick one only to find it suddenly disabled.. but also I don’t really know, hence I’m certainly not gonna trash her. She guided them to a safe landing which is a win in my book..

    Our CPL Law course was done by an ATC Controller. In NZ, the answer from the controller to a Mayday call would have been:"N509UP mayday roger." That was all. The controller did seem to overload the poor bugger a bit and I imagine the noise on his end plus the need to aviate would be fairly time consuming. Must have been a bit disconcerting not being able to climb. Still, good job by the crew getting it down safe.

      No kidding about the noise.. I'd forgetten that that model Citation has the door/stairs right behind the cockpit rather than back near the left engine.. The noise must have been horrible, probably overriding the sound in the pilots headphones.. Think about the wind noise in a car with windows down at 100mph.. Nearly double that noise RIGHT BEHIND your head when you're trying to fly/talk to atc... VERY good work on the pilots part.. Less good by ATC..

    female atc was asking him if he had atis, how about giving it to him, some of the controllers are horrible but she was marginal. In an emergency atc should be as accommodating and helpful as possible.

      +Brad Grafelman As soon as the citation declared an emergency the red sea parts. You get whatever you need. AVIATE, navigate, communicate. Always. He clearly stated he had his hands full. And even with a high timer that mental "muscle memory" can be a distraction to continue to safely maintain, an already struggling aircraft, flying. His priority is to continue flying, not fiddle around with atis. Even if its just a little push of a button away. I can't imagine the struggle this pilot had to obtain a trim level flight attitude to compensate for the aerodynamic disturbance.

    This controller didn’t have a clue, once he declares an emergency she she have given him immediate vectors back to the airport. Asking him to maintain his own terrain clearance just added to his workload. She should have brought up her EOVM (Emergency Obstacle Vectoring Map) and kept him away from obstacles of course but continuing to treat him like a normal flight was horrible. Complete lack of experience is evident. Asking him if he had the ATIS? Are you kidding me?

      Exactly... When I had an Emergency and had to land at Newark (KEWR) possibly without my nose gear (I only showed 2 green - my mains) ATC GAVE me the information not asked if I had it. Man, with all that drag from the door he HAD to be struggling just to keep straight and level let alone trying to climb... What part of UNABLE did ATC not understand???????????

    If he can't climb, then you need to immediately give him an alternative. She asks him to climb several times, despite his protests, before she final gives him an alternative. In that time, he could have easily had CFIT in the clouds.

      She wasn't asking if he can maintain the altitude, she was asking if he can maintain terrain and obstacle clearance (essentially, can they see and avoid things around them). When he said no due to IMC, she needed to get him to 2600ft because that's the minimum obstacle clearance altitude for IMC (the lowest altitude she can be sure he won't hit anything in the area)

    How many times did she ask if he had ground contact? He was in the clouds. Give him the best heading for terrain separation. He told her he couldn't climb. Atis....he just took off 1 minute before. What could change? It's only updated once per hour, or more with crazy weather, but I don't think this was the case.
    This lady needs some serious retraining in handling emergency procedures. Maybe normal procedures as well.

      JT: No, you are incorrect. MVA is for terrain clearance, not radar contact. In an emergency she could and did vector him. She didn't handle this very well. She was not helping the overloaded pilot.

      He was flying towards a mountain range, (semi)controlled flight into ground would not help him much. Options are A. climb high enough that radar/tower has time to notice/react to him going to hit. B. Be able to SEE the mountain, so he can avoid it himself, or C. turn so not aimed at it.
      Tower went with A as the standard, and wantred him to go option B if he could. when couldn't, she switched to C.
      Turning, when you can't climb, probabbly means you're going to sink, brining you even closer to said mountain, and skyscrapers, and population zones...

      Yes but, how many times did he have to say he was in the clouds, if he is 400' clear of terrain, ATC should stop wasting his time by repeatedly asking for his visual, also ATIS? give him a break, his doors come off, his passengers are alarmed, huge wind rushing etc, and she wants him to check ATIS? come on. ATC clearly only worried about herself not getting in trouble, again with the LOC, he says he needs a freq twice, but she just tells him to establish on it, only later tells him the LOC isn't available, she may not be panicking, but she is not helping in the emergency, you can hear it in his voice towards the end, in an emergency rules go out the window, she could only keep pestering him with them.

    I am sure your hearts will be warmed to know this same ATC controller was promoted to San Diego and was the controller of EVA015 flight of 16Dec16 who almost sent EVA015 into the mountains and into the path of other jets east of LAX.

    Jet stream super 31 had 10 micro switches in Door that had to make contact in series ,showed door open when closed. Eventually Pilots ignored the light indicating open. Eventually took off and lost a door.

    The most important thing after this situations for pilots and controllers is a debriefing to check, what can be learned. Bashing somebdoy for doing something wrong does not help. That only makes somebody afraid of this situations. So, if they did a professionell review on that situation there are some things, that can me learned: 1. Do not bother the pilot with the ATIS Letter. The pilot is in trouble and is working hardly to fly the plane. He has no time to check the ATIS. So look at the weatherscreen and keep it simple: Ceiling, visibility, wind. As he just departed, he should still have the correcht altimeter. So that is not that important. 2. prepare frequencys for ILS / LOC for all relevant runways / airports. That can be a job for the Co / Q Controller. That gives the radarcontroller time to focus on the frequency and radar. She did a very good job und keeping calm on the voice. That is very important. Good job there!
    I'm not familiar with the MVA situation over there. But when a pilot is in IMC and not able to climb, you really have a problem as an ATCo. The only thing you can do, bring the pilot back to a save area as soon as possible. That is, what she did with the vectors. So, another good job there. She didn't freeze, she tried to solve the problem. Well done!
    As an german ATC I can say, there are always situation, that are new for you and after everything is over, you are going to think a lot about what just happened. "Did I do everything correct?" Sometimes, you forget simple things. Do we know, if these maybe was a young controller working here first real emergency? No, we don't know that. So, stop bashing!

    This pilot did exactly what he needed to do he refused to take ATC advice and said "No a door has come off my aircraft we cannot climb we are descending now give me a vector to the runway." God your the boss up there not them safety above all else not schedule let the airliners be late they got billions in dollars they can afford it.     you guys bashing the ATC, take a fucking look at a chart of the area surrounding SFO. if you are in IMC and unable to climb you had better hope like hell that the ATC vectors the hell out of you to get away from those hills.
    there is dangerous terrain around 2000 feet all over the place at SFO. there are mountains right at the west and south ends of the airport. she's trying to make sure he doesn't smash into them.

    This kind of thing reminds me of the aircraft brief we got for the CASA-212 for airborne drops. On the CASA configured for airborne operations, the side door is removed and a small net is placed in the door, so that the jumpmaster can observe and determine when to release the paratroops. The thing that always brought a chill to my spine was the brief on this door and net.
    "If your reserve chute deploys in the aircraft it will start snaking towards the door, everyone will make an effort to contain the parachute, if the parachute reaches the door, you are going with it. at this time you must make a decision, you must follow the path the parachute take, if it went over the net, you go over the net, if it went under the net, you will go under the net, if you mess this up, you will attempt to take the aircraft with you using your body. Your body will fail this task, and you will die." I hated that door so freaking much...

    Can't remember if it was a mayday or a pan, but they were requesting an immediate return.

    Hate it when ATC asks a emergency aircraft obviously with their hands full if they got the ATIS. GIVE THEM THE ATIS.

    The same thing happened in 2006 to a TAM AIrlines (now LATAM Brasil) *Fokker 100* jetliner with *79 passengers* plus crew on board while climbing over the city of São Paulo (population 11 million, metro 20 million). The door hit the roof of a large hypermarket, causing a terrible loud noise, but fortunately little damage and no injuries. The plane returned safely to the airport.

    One thing for sure I had an emergency once and spoke once to ATC. I declared the emergency gave all the information and then flew my airplane until I was conformable that the situation was under control. Aviate- Navigate then Communicate.

    VASAviation - Always get's a BIG "LIKE" from me!!! You've had a lot of great postings but I'll NEVER forget the 17 yo student and her first solo. She lose's her right main gear. Kept in the air for a spell then lands the plane perfectly, considering her predicament. That was a nail biter!!!

      unless you're a certified radar controller i'd say its not your call to judge, coming from a controller I think she did a pretty good job, as well as the pilot.

      +Jack S lol i doubt anyone reaponding is a controller and can't talk. everything in the FAA is CYA and its either adhere to the rules or risk losing your job. an emergency does change things up a bit but not when it comes to terrian avoidance. nice job being sexist at the same time

      ATC was trying to get the plane to climb higher than known obstructions in the way unless he could see to avoid.
      Radar is not really good enough to ensure clearance vs the skyscrapers and "hills" (some hills > 1000 ft with tall buildings on top) right in the lines of flight depicted in the graphics.
      Her charts said get him above 2600 ft if possible because if not... he can hit something. There have been planes that flew into terrain in the fog in that area.
      She said over the radio that he really needed to be above 2600 for radar vectoring if he could not maintain visual terrain avoidance.
      Trying to thread him blind through the narrow pass between hills wasn't the most desirable solution.
      Really, its surprising this one didn't hit a building. There IS stuff to hit at 2000 ft right in there.
