    Hi Katie, I rarely comment but I just wanted to thank you so much for making these videos. After suffering for so long and having OCD affect everything from school, work and being able to take care of myself, I finally just searched RUclips for "OCD treatment" and stumbled upon your videos, not expecting much, but the way you explained everything and got through to me gave me the reassurance and hope that I can really get better. The day I watched your videos for the first time, I started with attention training and just trying to look at my thoughts differently and the treatments started working immediately; they were tiny changes but I was so surprised that they actually worked. I'm still working on my exposure therapy at home while I try to find a new therapist but these videos are so nice I feel like you're my therapist sometimes, I just hope that more people discover you and other videos like yours that make it easy for us to understand our OCD and how to treat it. I really like the way you talk about it in your videos, almost as if it's a normal thing and the way you approached the explanation of exposure therapy, it made it less scary and I finally understand its purpose the way others couldn't explain it to me before. So thank you again, and coming from California, you've reached a lot of people. I wish that more therapists specialized in OCD or at least understood how to treat it better like you. I just have a few suggestions on more video topics if you have the time to talk about them:
    OCD and its relationship with other anxiety disorders - I'm curious about how OCD coincides with my generalized anxiety, sometimes I feel like when my GA level goes up, my OCD goes down, and vice versa. I also wonder about my urge to pull on my hair, I'm not sure if it's a compulsion, whether I have trichotillomania or Tourette's because it feels like it could be any.
    OCD and reading/writing - I don't know how common this is but I have a hard time reading and writing because I keep going back a few words or sentences to check that I wrote everything correctly. This is especially frustrating when filling out forms because I keep checking that I wrote my own name and birthday correctly. This is happening right now as I'm writing this. I feel like in the back of my mind, I know I'm writing everything correctly, but the doubt is so overwhelming that I made an error or something that I keep rereading everything. If this is something you can help me and others with, a video would be so great and appreciated.
    OCD and how it affects your memory - after checking things so many times to the point of exhaustion, it's as if I've actually forgotten whether or not I locked the door or whatever it may be. If you have any insight on this, please let us know. I know that in time I'll be checking things less often and learning how to live with the uncertainty but if you have any other advice, it might be helpful to some.
    Thank you again Katie, sorry if you've gone over some of these topics already, I'm still watching the rest of your videos. But I hope you know that you're making a difference and I wish you all the best.
    To everyone reading this, I hope you're all safe and well, and keep pushing, I know it's hard but it gets better.

    When I started treatment for my OCD 15 years ago, here in Greece, I believe that ERP was unheard of. So they actually used CBT, but not for OCD. As a result, I could find some short-term comfort, but the problem was surely not solved. Now, 15 years later and having watched most of your videos, some of them 2,3, or even more times, I came to realise that ERP, although frustrating as it feels, it is the only or at least the best way to tackle OCD. I first came across the concept of ERP one year ago, when I started having serious flare-ups. At the beginning ERP sounded a good idea, but probably I could not use it properly, as I was trying it myself. Now, during this last year, I have experienced very very intense flare-ups, and every time I hit a wall, I would watch your videos and get a better understanding of how to tackle my problem. Now I can say that I am better, surely not fully recovered though, although call me crazy, I might see a near full recovery coming. Your videos helped a lot, but I am not sure (probably some OCD here intervening) if the level of understanding of your videos was the result of hitting so hard on dead-ends from all these terrible flare-ups.

      NOOOOOO!!! I'm so sorry to hear that. It's such a shame as ERP is very effective. If it helps at all, I had a client once who'd had 10 years of pscyhotherapy, being encouraged to unpick every thought and once I persuaded him to try and deal with his thoughts differently and stop asking why, he engaged in ERP and was a different man after a few months. So yes, if you have OCD you want to do ERP!!

    • @kellytheakston3578
      @kellytheakston3578 3 года назад +3

      CBT for OCD focuses on processes over thought contents. With other CBT therapies you may begin to explore thoughts and use strategies to either restructure the thought/ check the evidence for the thoughts but with OCD treatment- you may initially explore content but then begin to ask yourself " well what do they all have in common (they are doubts/ uncertainty/ etc)- then one can begin to have a new relationship with the thought- it doesn't matter what the content of the thought is whether you doubt you shut the door- or feel not right because there might be germs- the coping mechanism is usually the same (creating certainty- checking, cleaning). So one then begins to say- this thought is adoubt and I know that when I doubt I check and when I check I feed my OCD monster.

    I understand the ERP component of OCD therapy. Can you give examples of the CBT component for OCD therapy? If that means identifying cognitive distortions and identifying healthier alternative thoughts, doesn't that run the risk of engaging with the OCD in the exact wrong way, e.g. seeking to reassure oneself that what you're obsessing about is not really a problem?

    • @23katied
      @23katied  8 месяцев назад +1

      Sorry I missed this. Yes! In short in CBT for we do not ever engage in identifying alternatives or rationalise the thoughts in any way, instead the cognitive part looks mainly at the beliefs people have about their thoughts and the irrationality of these rather than of the thoughts themselves.

      That’s an interesting and important question. I wouldn’t say side effects but there is certainly a possibility that other things you’ve been avoiding dealing with because your OCD has been all consuming can become bigger issues as the OCD starts to get better. This shouldn’t put you off though - dealing with other things that arise is part of the therapy so you can either explore these with your therapist or if for any reason they think you may be better off seeing someone else they can refer you to someone.

      I hope my OCD story helps others in some ways.
      When I was a teenager both my parents died through two separate incidents and at the age of 19 I started to have OCD at the timeI looked confident to the world but I was feeling insecure. This was the first time in my life I had had OCD which took me about a decade to get over reading self-help books and I did completely until last year when it came back bad.
      Last year I did have a little bit of emotional stress and of course, there was the lockdown combined with for the first time ever I had little bouts of tinnitus which I still got but it's been getting less frequent
      Over this period I'm discussing here the last year to the outside well that kept total normality and kept by a business working ok as much as posable in lockdowns but I have a problem in private life. Recently I've been managing to curb my OCD tendencies so I feel I'm finding slowly a way out and get moments of feeling totally normal in all respects, but I can slide back in a moment. Anyway, this is my problem area and I am trying ERP. I read Managing OCD with CBT for Dummies which co-written by Katie and Rob Willson and found it clear and very helpful. I open to do anything to stop it as soon as possible.
      Last year I was getting worse, I started to have a compulsion that if I do not see and understand something, I may never get the chance again. This could be in the outside world silly little things like a year last February I remember driving past a telephone box in a remote spot and I wondered did it still work and I turned around to check driving back a couple of miles as I would not be coming back again it was on my mind, ridiculous I totally understand. This then manifested itself into television programs so when I watched I got to the point where must get all the details and even the point of lookup the meaning of words I do not understand. It means I could not watch things without tension and interruption. I would wake up the next day and it's like my mind looks and finds reasons to want to look back at programs, which I did feel silly but once I found what I needed I could forget but it happens again and again. At one point I never watched live only recordings and if I did not follow my compulsion at the time when I have refused to rewind things my ongoing compulsion is to go back and watch again. I have been refusing to do that for a little while, but now have a small list of recordings I feel a little too anxious to remove. Would you offer me a little advice have you ever heard of this type before? Or anybody else had this type of problem? I will do anything to stop totaly as soon as possible.
      Well done Katie with the new RUclips films they certainly bring the subject to good clear conclusions from your previous ones which are all very useful.
      Regards to all Alan

      @@AlanDavidson1958 hi Alan. Thanks for posting here. Sorry you’ve had such loss in your life. So sounds like your underlying concern is that if you don’t check or find out for sure then it’ll just keep being on your mind. So when you do check it solves that problem but it just crops up somewhere else so you can see that the checking in effect scratches the itch but doesn’t cure the rash. So, you’ve done this before you can definitely do it again. Write a list of typical triggers and do some deliberate exposure - e.g fast forward a little bit of something you’re watching or go and put the kettle on without pausing etc - I.e deliberately make yourself miss something and learn to tolerate the feelings that go with it. Also write a list of all your safety behaviours like keeping recordings, concentrating super hard etc and then do the opposite of these things when you are doing your exposures. Remember the more you check the more you’ll feel the need to check. The more you learn to tolerate discomfort through deliberate exposure the less discomfort you will feel in the long run. Hope this gives you some direction to get started.

      Yes absolutely. Meta cognitions (your thoughts about your thoughts) are what we target in CBT for OCD. It should hopefully settle down quite quickly. The other thing to note is that medication doesn't tend to make the thoughts go away - it can however make them less loud and your reaction to them less extreme so it can help you to engage better with therapy.

I usually repeat them mentally, though I have used my teeth and jaw to keep track as well. Basically, the sentences must start on my right side and end on my left side. If not, I try to "fix" the sentence in my mind and if I can't, I get very frustrated. I do this near constantly, which makes it difficult to give my full attention to anything/anyone. I am unsure what the obsession is that causes this compulsion, to be honest, but it is very exhausting. If there is any advice you could give that would be helpful, I would very much appreciate it. Thank you.

      do you have a feared consequence if you don’t do this or is it that it will just not feel “right”? In short, you’ll need to expose yourself to things that bring on this compulsion and start doing it deliberately “wrong” - go the other way, get stuff jumbled etc. Once you learn to experience the discomfort of it not feeling “right” you will end up not feeling the urge to do the compulsions. Might take quite a bit of time and patience if you’ve been doing it a while but it will work if you stick at it. Good luck!

      @@23katied It is mostly a feeling of not being "right", though I do worry quite a bit about not understanding things I am reading/what is being said to me. I will do my best to deliberately do it "wrong" and sit with the discomfort. I would say it is the compulsion I do the most often. Thank you so very much for responding to my message. I really appreciate it!

    Hello Doctor, thank you for all this help.
    How do I know that I am doing my erp properly? For example , sometimes I just find myself anxious and thinking my compulsions kind of automatically (I’ve got the pure o) and then anxiety increases once I get to see that I am not doing “the right thing” because I did the compulsion PLUS I am still anxious about the obsession from the beginning. Those times I just dont know what to think! Just do nothing? Thinking that I may be doing erp the wrong way makes me anxious..

      Hey Pedro stay firm on the erp and also say to yourself " hey ocd I see if you I'm busy goodbye" keep disregarding and go do something else. Everytime the thought comes back be certain and disregard. Takes so much practice. I relapsed on my ocd after a year. It's like I forgot all my information on retraining my brain from ocd. I feel so much better. Also check out Ali Greymond On RUclips she is amazing like Katie as well

    I’m currently seeing a therapist, but I’m not sure. She’s asked ask me what I’m afraid of if I don’t check, and notice if it’s an overarching theme. Although, I thought thoughts don’t mean anything? I was also concerned when she said it may be GAD not OCD, that I also have. However, compulsions are only present with OCD from my knowledge. Please can you add some light, Katie?

    • @23katied
      @23katied  2 года назад

      Yes it's true the thoughts don't actually mean anything but of course in OCD you do attach meaning to them and that's what the therapist is asking about - e.g If i don't check something bad will happen or I won't be able to forget about it and relax etc. There is a lot of cross over with GAD and OCD and people can often have both. All the of the work learning to deal with thoughts differently will help with either. With GAD it's important to look at your beliefs about worrying too.

      @@23katied Thanks, for the reply, that’s helpful. Would you think I should stick with it? I’m actively working on it by myself but struggling to engage in therapy with her. I’m a very black and white thinker, as well. When she said GAD do have compulsions, and I saw on the Internet that they do not it made me doubt her ability.

      @@amitrajput6904 it doesn’t have to be a thought that is repeating. You are clearly full of doubt, engaging in those doubts and then maintaining the cycle. The trick is to treat it as ocd and follow the advice here (or another useful source) and see what happens. The content of your thoughts is irrelevant. It’s not uncommon for people to become obsessed by their ocd or their thinking processes - it’s all the same stuff and can be treated with the same techniques.

      I have known people do this. If you can stick to it then in theory it can work BUT you have to be clear that you are learning to deal with any thoughts and feelings differently and tolerating uncertainty rather than unwittingly getting really good at blocking as this won’t work long term.

    • @AlanDavidson1958

      @@23katied I understand totally agree. I found over the last month the TOTAL cold turkey ERP method didn't totally work for me, I tried but I was not strong enough but I didn't totally fail. It's basically like winning the battles but then realising you need to have a rest bite to win the war hopefully and I allow a bit of blocking avoidance etc. But I can see for me it was unrealistic to flick the switch and stop no matter and how much pain psychologically. So this gradual path towards normality is working...... The one thing I've noticed is that at the height of anxiety when I don't think I'll be seeing a unique situation ever again my OCD wants to see and understand etc and if I don't I feel it big anguish but it ALWAYS seems so less important and it's so much better later after more of these moments. Then my normal mentality starts to shine, so hopefully, I'm on the path away from my OCD. Thank you, Katie
