In fact, SurveyCTO has a user-friendly graphical interface for form design that you can see here: www.surveycto.com/videos/form-designer-intro-to-form-design/ Alternatively, forms can be authored using the XLSForm standard, allowing you to design forms in Excel or Google Sheets. You can download the SurveyCTO XLSForm template from the Design tab of the SurveyCTO server console as part of the form creation process, using the "Advanced download options".
