    “Fomepizole is a drug that inhibits alcohol dehydrogenase. So, it allows methanol itself to be excreted unchanged in the urine so you don't build up toxic levels of formaldehyde. Ethanol is a competitive inhibitor of methanol intoxication and metabolism, so sometimes we will actually treat methanol toxicity with ethanol. Myself have treated a number of patients who had drink wood alcohol or methanol with intravenous ethanol to the point where we actually had to intubate them and put them on ventilators to compensate for the severe intoxication from the ethanol until they can clear the methanol and then they just were allowed to wear off the ethanol on their own…”

    I wonder what is the possible damage of ingesting a very small amount of ethylene glycol, lets say 0.5 ml. What is the threshold for damage?

    I don't understand why ingestion of ethanol would not cause metabolic acidosis, since the ratio NADH/NAD+ would be increased due to the conversion of ethanol to acetaldehyde, and subsequently to acetic acid.
    The increased NADH/NAD+ ratio should, in turn, increase the depletion of pyruvate into lactate, which is basically acidic ? So why ?

    It should say that formic acid / formate is responsible for damage, not formaldehyde directly. Hmm..

    How do you tackle the fact that doctors are not testing for poisoning cases on people with long history of chronic illness?
    What type of doctors know about the after care of something like this?
    All videos I find are about prevention and immediate treatment but not the long term treatment of recovery or even the survival rate.

      The alcohol and delirium psychiatry lecture from this same channel covers some of the topics listed in your comment. I am a nursing student but I am assuming doctors were also taught about the use of referals to AA or other support groups as they have the highest success rate to treat alchol abuse disorder. Most nursing/medical lectures focus on the inpatient setting (hospital) as a majority of the professionals work there. Referalls to outpatient therapy with a psychiatrist, Psych NP, and other psych professionals also help via using psychotherapy, CBT (reforming negative thoughts into positive ones to, for example, deter them from drinking) and the prescription of aversion therapy and narcan to make drinking alcohol have side effects and not be enjoyable. I hope this answers some of your questions.

    Ethylene glycol is not a drug of abuse. It has none of the effects of ethanol and methanol on the CNS. It can still be clinically important since ingestion of anti-freeze can occur, not so much for want of a "high", but because it tastes sweet.

      Ethylene Glycol (ethanediol) does cause intoxication, but it is just under half as potent as Ethanol at causing intoxication.
      If you drank 5 shots of 40% Ethanol on an empty stomach, most people would feel a significant buzz, whereas with Ethylene Glycol, you'd probably have to drink 12 shots of a 60% solution to get an equivalent buzz

      @@GreenGameLol I get the point, and so this (CNS effects) must be corrected. However, I still contest that methanol and ethylene glycol are drugs of abuse [of any importance]. Ingestion is usually accidental, not deliberate (in contrast to ethanol), since the toxic effects are so dangerous (and unpleasant).

      @hinkelstein69 Yeah, which the reason why Methanol and Ethylene Glycol aren't manufactured as drinking alcohol like Ethanol is due to (like you said) the metabolites of MeOH and EG cause their horribly toxic effects.
