Can that address be my personal mailing address number or should that be the address from the physical location of the private practice?

Can I start off the contract with out a facility and then add a facility later when we rent an office?

Absolutely. I have many therapists who start by only treating patients in the patient's home then progress to opening a facility. It is a very easy transition.
