The kid does have cancer, based on the medication listed on the clipboard in his room. Also, the kid lying in the bed behind the curtain who disappeared was the main character, only with hair (you can tell because of the suspenders), which confirms that he's undergoing chemotherapy. I don't think it's fake as in he's actually on his deathbed, but I do think that he and his mom are playing pretend together. And the whole thing about going to "Rakuen", well...all I'm gonna say is I've watched Wolf's Rain and I think I know where this is going.
