The result from the Neural Filter was fine. When you really laugh, you laugh with your eyes too and that's exactly what the filter created. Why swap the eyes back to create such a "cold" smile...
How does the program know the shape of the teeth? Like, in this instance, that the teeth aren't straight & even? Also, I guess it assigns a best guess to the color of the teeth, then you adjust it?
It’s most probably generated with a machine learning model, which is trained on existing photos from the internet. And yeah it’s a known problem that such approach might perpetuate existing stereotypes and it’s bad...

So it is indeed random. I haven't used these filters yet, but in theory you could get different results if you try it again.

No control unfortunately. It looks like a decent starting place to bring in teeth from another image that you know looks similar, if capturing their smile in-camera isn't possible
Downloaded and tried it and as expected it aint really that good doesnt even look like the same person anymore (facial structure totally changes). With that said it would be useful when the goal is not to make someone "you know" smile rater where the goal is not for memorial purposes.

It doesnt know the shape, it's not magic.. In any case it's much easier to tweak the result than doing it all from scratch.

This thing is very good to get a quick giggle out how awful it is. After you're done getting a laugh out of it, I'd ignore this feature.
After you get the photo subject to giggle at it, quickly snap a shot of them smiling and then re-photoshop the real smile
This filter didn't work on my child. It made her look angry instead of happy lol
