It looks more obvious now to me but what if the second scene when Rick wakes up in the hospital with a long beard (looks like he's been there since a long time...) so what if actually it's him waking up from the coma after he blew up the bridge and went in a helicopter. Means it will end with him waking up and then he'll find Michonne and Judith back somehow like in the scene where he's enter the kitchen and Michonne + Judith's there
