The ironic thing is that the effects of untreated ADHD are especially impactful in first world societies where almost every aspect of daily life is overly complex and information intensive. "First world problem" doesn't mean "not a problem"

What I really dislike about bipolar disorder the docs just load u up with a mix of meds which can just make it worse..

I was on six meds, one was 1800 mg TWICE a day, of lithium, at ten years old. Came out later I was literally Autistic. Damn shame.
I was diagnosed with adhd at a young age and was medicated at a young age and I agree the meds caused me to be severely underweight and zombified
We are experiencing this now, been 8 months of constant change and major rapid cycles more than 4 in less than a year.
Mixed episodes feel like using every emoji all at once. To me at least.

As someone diagnosed bipolar 2 with predominantly mixed episodes, this is accurate.
I have mixed episodes a lot and it's actual hell. It's worse than just having one or the other tbh.

I both have ADHD-I and Bipolar II and neither could be remotely manageable until I got medicated for both.
Sadly for me the stimulant causes my blood pressure to skyrocket so I couldn't stay on any (the only one that did the job was the chewable that's usually prescribed to kids; my insurance stopped covering it 2 years ago). I can still be on the non-stimulant thankfully and it works enough that most of the time I don't have executive functioning issues.
As someone with bipolar type 2, BPD, and ADHD, methylphenidate has significantly reduced manic episodes and impulsiveness. Dextroamphetamine, opioids, and alcohol; however, have induced extreme mania and impulsive behavior in the past.
Interesting. I’ve experienced the opposite between methylphenidate & d-amphetamine.
SSRIs caused me to have my first mixed episode with psychotic symptoms at 16.

As for my achievement, I don't feel I achieved anything significant yet. I'll think I achieved that once I can earn enough to maintain myself. Being from a 3rd world country, where people with disabilities are usually seen in a bad way by society, that milestone seems to be harder to reach than I thought.

Thank you for touching base of the stigma about ADHD and adults and how stimulants appear to be bonus medication for people, I got off my medication due to insurance changes and I not only failed out of college. I lost my job and I also got in a car accident accident!
I pay out-of-pocket for my stimulant medication because the thought of going without it is far too unacceptable. Hopefully you were able to either find an alternative treatment or an alternative access to your previous treatment.
@@cda6590 because of the insurance thing I have to do another intake appointment which would be 700 and then the medicine itself is 300 so it’s literally going to be 1000 for the first appointment and medication and then 500 for every refill appointment, keep in mind I also lost my job so I really just like can’t do that xD

I mean based on the outrageous prices you're listing I'd assume you're American like myself, but I'm guessing not? Typically--at least in America--something like an ADHD diagnosis shouldn't "disappear" categorically just because you dropped your insurance.
The fact that you have documented medical records indicating a positive assessment for ADHD and its subsequent pharmaceutical treatment ought to be all the proof any psychiatrist would need to continue your previous medication regime.
The waiting list will be outrageous, but have you tried free clinics which offer behavioral health services, particularly psychiatric?

I used to have hypomanic episodes (I am also diagnosed with ADHD, and pDID), but when I went too high on my Wellbutrin XL, I had my first full blown manic episode. Now I've more than halved that dose, changed to Wellbutrin SR, and am taking a low dose of Quetiapine each night. It sedates me, as long as I take my medication on time, I never have issues with sleeping and regulation... but if that balance is even mildly disrupted, I risk triggering mania. The fun part is this entire journey of finally being diagnosed and taking these psychiatric medicines for the first time ever in my life is that I'm a graduate student! It's been a yikes to handle it all.
i was on wellbutrin too! it worked so well for me until one day it just “stopped working” and i started having a bunch of anxiety. now i’m starting to think it was actually mania!
Having both Bipolar 2 and ADHD (and PMDD, GAD, panic disorder & CPTSD + I have a chronic physical illness) is tough af. Adderall has helped me so much after finally getting diagnosed with ADHD as an adult. I don’t know how I got through school without out it (it was an intense struggle for sure) and I don’t think I’d be able to work without it. Even just keeping my house clean is difficult for me.
*Edit: I did lower my dose from 30mg to 20mg after speaking to my doctor and that helped minimize some symptoms. I think my biggest challenge is the timing of the medication (taking it early enough in the morning) and remembering to eat on it.

Hi Dr. Tracey Marks, I know you know this, but I wanted to add context as I noticed something that some may have mistaken/misunderstood in the first minute due to the audio and visual aspect.
Klonopin (as branded in the US and more commonly branded as Rivotril, both by Roche) is Clonazepam which is a benzodiazepine that acts by increasing gamma-aminobutyric acid (GABA) and is not an Anti-Depressant which some may have mistaken.
However, it is probably the only benzodiazepine that is occasionally used to assist with bipolar mania as well as its well know anti-anxiety effects.

This video came at the perfect time. My husband was recently diagnosed with Bipolar II and has been diagnosed with ADHD since grade school. His current psychiatrist seems to be very anti stimulant use in ADHD and this decision is literally keeping him from going to college and forcing him to continue his miserable job that doesn’t require a long attention span. Obviously I want him to fulfill his dreams and the idea that not being properly medicated will keep him from doing so is extremely bleak.
Thank you for a very informative discussion Dr Marks. It's so important for all of us dealing with Depression/Bipolar disorder that we arm ourselves with as much valid information as we can. I am on a cocktail of drugs (the only way I can describe it) for the last 6 years and for the first time in my life I am feeling well (I turned 66 this year). I am a person who needs to know what drugs I am on and why. I find your videos very helpful. They add to the information I have got from my psychiatrist.
At a similar state to Chad. I have ADHD, bipolar disorder, OCD, Binge Eating, and anxiety but was only treated as an adult. Was told to be medicated for bipolar first and then add in a stimulant. As mentioned as a possibility, an anti depressant and two mood stabilizers are used for me. I then use Vyvanse for ADHD, and Trazadone to help me sleep. I get annoyed when people think I need less medication without understanding the logistics of why i'm on as many as I am.

im sorry that people who have no medical experience are commenting on how many medications you take. clearly they’re all very necessary for your functioning. some of us deal with a hell of a lot more than just depression and anxiety.
I also have BP2, ADHD, OCD, and Binge Eating. I take Vyvanse that’s helped with binge eating but I still struggle with it. Have you been able to control your binge eating? I take Lamictal for BP2

I was misdiagnosed with BPD, and ADHD. I was on the wrong medications, and exhibiting a less than satisfactory response to treatment. My ASD diagnosis changed my life and opened a whole new door to feeling regulated. I never related to my bpd diagnosis but I did admittedly have poor emotional control. It's important to be open with your doctor, instead of listening to others speak about your behavior and make a judgment. Things may be related that you arent thinking bout, and a doctor can evaluate all that. When I began treating Asd, OCD, and PTSD the behaviors causing suspicion of bipolar were dismissed. Life is so much easier
Armodafinil behaves kind of like a stimulant, but is not actually a stimulant since it does not stimulate the release of neurotransmitters. It has been studied in bipolar disorder and was found to be well tolerated. It is a dopamine reuptake inhibitor and "wakefulness promoting" agent.
Additonally, it acts as an AMAZING antidpressant. I have no idea why this isn't better known and more used, especially since this is such a difficult area for bipolar people.
I have trouble with stimulants, and even just 200-300mg of caffeine can make me go hypomanic. But armodafinial does not and has been a game changer for me. Per the clinical studies, it just needs to be titrated to prevent cycling when you first start it.
I think Modafinil is still too poorly studied yet, no?
Dr. Marks, I am diagnosed with bipolar and borderline personality disorder. When I was prescribed Vyvanse for binge eating, I abused it, unfortunately. I know this is wrong now. The double dose along with my bipolar made me experience delusional parasitosis. I thought bugs were crawling all over me and I was in the shower trying to scrub “them” off! I ended up having to be hospitalized. It was not fun. Now I’ve learned my lesson.
Just had a phone call with my provider the other day about how my insurance rejected my request for Strattera because they wanted me to use stimulants first (my cardiologist asked that I try coming off stimulants for my ADHD). I’d also since been diagnosed bipolar, so I was explaining this to my provider for her to put in the appeal letter. I mentioned I was concerned about rapid cycling and she was like, nahh, that’s fine; we have a lot of bipolar patients who take stimulants. That she didn’t hesitate to say that, without caveats, was mildly concerning lol
I have Bipolar 1 with Psychosis and always had really bad ADHD throughout my life and had been taking off stimulates after a hospital stay, my doctor and I made a deal with I got sober I could be given them back! I was off them for 4 years without getting anywhere with my life, I'm now back on them and I feel like I have my old brain back. I'm now sober and medicated. (And no bipolar symptoms of ANY kind for one year)

@@32421AF_CG if they do only take stimulants, I’m assuming they’re one of the people in whom stims have a calming/sedating effect. Or, maybe the stims just help them manage their ADHD sooo well that they’re able to be strict with bipolar management methods like diet, sleep, routine, etc :)
Totally just guessing though! But considering not everyone with BP needs mood stabilizers (or at least, not forever), it makes sense to me that some people with both ADHD and bipolar COULD manage one with meds and the other without. Hope OP replies to us, though!
i feel like i have something like bi polar, but instead of mania, it's just neutral, or a bit chipper/or positive. Which can really just be fluctuations of depression that can last for weeks or months, and it just changes suddenly. Like a gear shifts, and my perspective on the world and how i view things, changes. Stimulants don't change it, but it does help with not being tired.
Maybe you have cyclothymia, or even just a “cyclothymic temperament”. Those are on the bipolar spectrum, but not nearly as overpowering as bipolar I or II.

What sort of threshold of anxiety would a patient ideally have in order to take the risk of putting them on anti-anxiety medication while also on a stimulant?
In cases where this combination is warranted, what sort of time limits would you put on its administration?
In my experience (I know a lot of ADHD individuals), our moods can be fine on stimulants and anxiety only begins to rear its ugly head when the stimulant wears off later in the day. In these cases, am I right to assume that benzo's are a bad--maybe potentially dangerous--answer?
I was diagnosed with bipolar disorder long before I also got my ADHD diagnosis. My doctor prescribed me Ritalin. The couple times I stuck to taking it regularly, I experienced sudden mixed episode bipolar attacks that were very sudden and very severe. Right afterwards I would experience prolonged dissociative episodes. I'm scared of taking ADHD meds again because of this.
The problem is the doctor believes in the guidelines more than the patient. The patient obviously manages well on the regiment, but because the guidelines don't recommend it, the Dr becomes more concerned about lawsuits. The Dr loses the "healer" and becomes a "medical beurocrat."
Yes! I got desperate and started taking my friends meds when covid made it hard to see a doctor. I told my doc how well Vyvanse it was working for me and she kept telling me it's not good for me because of a lot of reasons this doc is against it. I kept explaining how much better it was but she won't here it.

Funny as someone with BPD and Bipolarité type 2 I was asking to myself If I could drink stimulant drinks because to hype myself for a futur music festival. Be aware that, unless these drinks are laced, the 'stimulant' effect they have operates fundamentally differently from the stimulants used to treat ADHD.
@@cda6590 while true, they should still be used with care. When I was in a psych hospital briefly the doctor there was adamant I not even drink coffee or pop.
Anti depressants sent my ex partner into a severe manic episode for more than a year and it completely destroyed our relationship, I can only imagine what stimulants would do (I take them, I have ADHD.)
Antidepressants are way more dangerous and harmful than stimulants in my experience. They have insanely long half lives and horrible withdrawal symptoms when you taper off them… stimulants at least are in and out of your system in less than 24 hours… never caused me more than the normal side effect problems like nausea and dry mouth. But antidepressants ruined my life, while doctors were preaching they were the holy grail for all my problems.

I only ever struggled with ADHD meds as being either too low of a dose or ramping up my blood pressure into hypertension. Anti-depressants caused me a ton of side-effects and one med (Wellbutrin) was the first time I felt so close to suicidal in all my life). I was a mess until they switched me to mood stabilizers.
@@MsVilecat Honestly antidepressants are soooo dangerous and have more downsides than good. But the industry as a whole just ignores that…
Even now I have multiple doctors recommending antidepressants for my ME/CFS which is a physical illness. They ignore when I tell them I have a bad history with them and do not want to try it.
The pharma companies have a bad product, so they’re trying to find new audiences for it. Pain, ocd, anxiety, sleep.
And docs like prescribing them cuz they’re “safe” in the sense of OD’ing. It’s really hard to explain the damage of them without experiencing it yourself since it’s not measurable with any tests.
I guess that I am lucky. I do well on an SSRI, lithium and an antipsychotic. I have gone off antidepressants without any major problems.
Thanks for this! I was diagnosed with bipolar 2 about 15 years ago and diagnosed with ADHD last year. The ARNP I work with to manage my meds has been great; we were (and are) both on the lookout for symptoms of hypomania. At one point as we started adjusting the ADHD meds dose upward, I started feeling things I recognize as symptoms of oncoming hypomania. She reduced the dosage of one of my antidepressants, which brought me back to stability.
I appreciate the reading help via the cover art. It's kind of like when the psychiatrist knows that you're in and out of that dark cave self-mending but takes the time to set out a small dish of Delaware River Caviar.
Interesting topic. I'd add that even coffee, tea, and soda are variable opinions. And obviously illicit stimulants (c*ke, m*th, cr*ck) should be completely avoided. Apologies about the censoring but YT hss been deleting my posts lately

I’m here because I realized that I can constantly change my mind on important matters more than once a day in many former situations. I don’t think I’m bipolar though.
I am bipolar, I take Ritalin and Atomoxetine in the morning with my anxiety meds. Then I take lithium and quetiapine at night. It took me over 3 years to find a good combination.
For more context, stimulants may not give you the desired outcomes or results when trying to Manage the symptoms of Bipolar Disorder and ADHD as opposed to taking antidepressants which would have an adverse effects. So it is essential to check in with your provider and by all means be very explicit when discussing your challenges as it would help your provider to know the next steps to take on whether to use either stimulants or antidepressants to manage the symptoms of Bipolar Disorder or ADHD. But since but challenges are alike, you are your best observer of what is going on in your body.

@@rachna0367 and what "cure" is there for genetic and trauma triggered bipolar disorder? I have Bipolar 1 with acute mania; no Psychosis since 2013 and ADHD. So, cure? A cure would mean a preventative for it to never develop.

@@rachna0367 over the years I have been on a big range of different medications with horrid side effects. I think that may be one reason patients tend to stop taking their meds. Once on 5 different Psych meds at once was not helping me in fact was doing the opposite. I made a decision to wean myself off these meds; never stop suddenly. I ended up finding another doctor that removed the antidepressant completely as it increases my mania. I am now on 3 meds. That I was quite nervous about but anything was better than Lithium to me. Everyone is different of course and respond differently. I was afraid to try this combo because as a former nurse I knew the contradictions in mixing them. However, for me they work well. Seroquel is for my Bipolar in general. Yes I know ppl have stories of a zombie like feeling from it or being too groggy that is cause the dose is high. I started low and found the strength perfect for me that does not cause that. I also take Xanax for my panic attacks and horrible anxiety, my dads cancer diagnosis helps with that. And lastly, I do take Adderall for my ADHD. These 3 have helped me way more than any combo i have been on since 2013 and I have been guineapig'd a lot. I do not recommend these for anyone as anyone's conditions and way of life it different.

It depends on the genotype. People with the COMT met/met genotype or MAOA or GAD1 or ODZ4 mutations with or without bipolar disorder must avaoid anything stimulant, including coffee
I take lithium, Latuda, and low dose atomoxetine. I don’t do well on stimulants.

I take 25mg of adderall on top of 2mg of Abilify and I have never been calmer, focused, and attentive.
Without the Abilify tho it would make me go hypomanic and I didn’t realize it until after the antipsychotic. 😂

I have a pretty similar mental workup as Chad, and my psychiatrist is also concerned my stimulant will trigger my mania, but so far it's not been a major issue. My solution is taking my meds (like my anxiety meds) that have a mild sedative effect alongside my stimulant to even me out.
My body went supernova and just exploded out into Infinity. It was like I was an ice cube thrown into the ocean and it just melted. This was by far the most incredible and most positive and most joyful experience that you could possibly imagine. That was the moment of death and I had the Epiphany that death is the greatest thing that could happen to a living creature because it feels like in that process of dying for a split second it's terrifying because you're dying and you have no idea what's coming next. But then as you fully let go and the death occurs the next Split Second you're so relieved. It's like your entire life was this giant thousand pound weight wrapped around your shoulders just tons and tons of weight and then all of it was just lifted in a split second and you realize that you're untouchable and that you're Immortal and that nothing could ever disturb you again. It's complete peace just utter peace and joy and profundity. The beauty of it is Indescribable.

Hi Tracey, I’ve just seen the sample of your E book bipolar basics you mention here. That you have a chapter on “metabolic syndrome” which I was surprised to see in there. What does that have to do with bipolar?
I’ve recently found out I have PCOS, which most likely means I have metabolic syndrome too. I have bipolar 2. I’m just wondering what correlations there may be that I should know of?

You have to be careful. In my case, avoid Madderall. But do what works according to your doctor.

I didn't know I had Bipolar disorder until I started on a stimulant for my ADHD. I tried to unalive myself within just a week of taking that medication. I was rescued at the last moment. Then at the psych ward they evaluated that I had bipolar. Seems like the medication triggered a manic episode.

This was an interesting video, Dr. Marks. I have a question. What a about oral medications used for weight loss if you have bipolar 1 and get psychosis in severe depression or mania? I am pretty sure they are also stimulants. I had a psychiatrist prescribe it once, but I was afraid to fill the prescription because of my bipolar. I'm prone to depression more, though I get mixed mania with psychosis frequently. I don't have ADHD. I'm on two antipsychotics (Vraylar & Zyprexa, the daily Zyprexa is only short-term as it's usually PRN only), two mood stabilizers (Lamictal & Tegretol XR), an antidepressant (Wellbutrin), Anafranil for OCD, and Atarax for severe anxiety. I'm currently in a severe depression and have been for six months, and that was before my cat (she's my ESA) got diagnosed with terminal cancer. Please forgive me not using any generic names, I call all meds by the brand name because I can't pronounce or spell the generic names for so many of them.

I've just always been told to stop my stimulants if I notice I'm going hypomanic. I'm BP2 and usually notice I'm hypomanic so it works.
But I do tend to have mixed episodes.
I was on stimulants (Concerta) for years before I was diagnosed as bipolar.
Do you think taking Concerta contributed to developing BP2?

@@May-or-May-not Interesting. Stimulants can trigger onset of things like BP, or schizophrenia. I think my use of stimulant meds (always used exactly as prescribed) contributed to my mood disorder. I, too, was on them for years before being diagnosed.
Dr Tracey..can you please differentiate between fibromyalgia and somatization of depression?
I couldn't handle the mood stabilizers. They gave me nightmares.
My doc. Put me on Strattera- which is considered an antidepressant in Europe - but used for A.D.D.
I am more anxious though.
I am checking out my hormones soon.
I think I have adrenal dysfunction.

I take Modafinil when my moods are low, works great, is not a high risk of causing mania.

This is interesting . My son was diagnosed with ADHD and later bipolar disorder in childhood. He was on a stimulant and mood stabilizer and did well until he got to 9th grade. The transition to high school threw everything off and he had a major psychotic episode and had to be hospitalized twice and missed a month of school. His doctor took him off the stimulant and he never went on it again. He struggled with school after that and has struggled with learning and developing a career since then. I wonder if he should have gone back on the stimulant but his doctor was convinced it was causing mania. Now he is an adult and doesn’t want to take any meds which is another issue but I wonder if it would have helped him to try the stimulant again?
Personally I have bipolar 1 & I am a mental health prof. My experience is as long as you take the stimulant with a mood stabiliser you should be relatively stable. It's taking the mood stabiliser that is key. If you're afraid he won't go for regular monitoring then I'd recommend depakote over lithium. Stimulants aren't contraindicated in bipolar disorder they're only cautioned.
Does he not take a mood stabilizer as an adult? Maybe the doctor wasn’t putting him on the right balance of medication or dosage (perhaps a lower dose, shorting acting stimulant + mood stabilizer might work for him?). I’ve been on various medications for 8 years now (in addition to going to therapy) (I have bipolar 2, ADHD, CPTSD, GAD, Panic Disorder - all diagnosed as an adult but all things I definitely have struggled with since childhood). I’ve had to try multiple “cocktails” of medications throughout the years to get the right mix. Finding the right medication can take a lot of experimenting and is honestly a lot of guesswork & finding the right psychiatrist is the same thing. And while I’ve been on some medications that made me feel like I was going crazy and/or had pretty bad side effects, continuing to work with my psychiatrist to find the right meds for me has been a total game changer. For me, it’s just not worth the struggle of every day of life being such an immense struggle. I hope he can find the right doctor and medication for him and remember that trial and error is very common, but worth it in the end!
EDIT: I’m not sure what mood stabilizer he was on. I take lamictal which has been great and had minimal side effects. I know that lithium can be a lot tougher for a lot of people and have more adverse side effects. I would encourage him (and you) to research lamictal (aka lamitrogene) and inquire about it with a professional. It’s the “gentlest” mood stabilizer in my opinion. Doesn’t work for everyone (in rare cases people get a bad rash from it and should stop immediately if they get that, for example) but I think it’s a good first thing to try before taking lithium.
@@yesvember11 right now he is on Prozac and risperidone and trazedone to help him sleep. He was on both depokote and lithium at different points but the doctor said there were issues with his blood work so she took him off both. I’m not familiar with Lamectal but I can look into that. But he is very reluctant to take any meds now because he doesn’t like the way they make him feel, even though he knows he needs them. I don’t know if he will be open to adding anything else but we can ask. Thank you for your response.
I got POTS so I had to get off of Lamotrogine. It made me much more dizzy. I had to go back on Lithium. Lithium works well for me but it has some unpleasant side effects.@@yesvember11
A family member, who is in the bipolar spectrum, takes his ability 4 days a week and don’t take it on the weekends bc he “enjoys” drinking alcohol. I think he has an addiction but he refuses to get help for that. His mood swings are very rapid and cycling fast. He could get very aggressive, impulsive and have rages episodes. I don’t know what to do to help him. Do you see patients in NYC? I wish he could have a visit with you. He needs your help!
How fast is the cycling? In the US bipolar is often over diagnosed for pharmaceutical reasons, are you sure he doesn't have BPD given the rapid cycling in conjunction with an addiction issue as the treatment pathways would be very different. Obviously you can get rapid cycling bipolar but we're talking about weeks between episodes here, not days & alcohol can increase the emotional lability we see in BPD/EUPD.
Pretty much anytime that he gets triggered. How do you treat that?

I have bipolar disorder and ADHD. My psychiatrist recommended that I take small doses of Rubifen (20mg or 30mg) because of the risk of falling into mania.
Oh hey, this applies to me 😁 I have bipolar 1, and was put on Vyvanse for Binge eating Disorder and found out I also have ADHD. I haven't had a single manic episode on them. Tho I am also being carefully monitored and am on 1000mg of depakote
What about non-stimulant ADHD meds like strattera?
Adhd meds seen as "not that critical anyway" apart from massively increased risk of divorce , car accidents, suicide... Just because they don't assess based on distress doesn't mean ADHD isn't as distressing as the other things in that book. Emotional dysregulated and ADHD go hand in hand so your ADHD sabotages your life and relationships and then makes you extra emotionally sensitive when it all happens to you

So... my father was diagnosed bipolar, and my grandma had some interesting stuff going on. Now whenever I drink coffee or do rigorous exercise the day prior, I get super motivated and I'm afraid it might be mania or hypomania. I haven't really proposed anything too crazy, but I've htought of like "YO, maybe I could start a non-profit which advocates for something"
What about your sleep pattern after such days? Hypomania and Mania consist of a bunch of symptoms and if it's only "feeling motivated" that would be completely normal

Hello, Im diagnosed with ptsd and bpd. I have trouble focusing, decision-making, and problem solving and frequently get brain fogs. Is it not recommended for those with bpd to also take stimulants as it increases irritability and mood swings? When I brought this up with my psychiatrist, I was told my struggle with concentration with brain fog is due to bpd and I'd need a IOP. What are your thoughts?
Since when is Klonopin (a benzodiazopine) considered an antidepressant? Is it used for treatment resistant depression? I'm confused.
I think that they made a mistake.
Or people like me get Adderall in a dose too high and get real anxious and SI. My pill pusher says oh that's bipolar and medicates me wrongly for that for all my teen years till at 18 I say enough and get off them.
Fun fact it was trauma related with ADHD. Vyvanse at just 10mg is the perfect dose prescribed by my general GP.
Only time I was seriously SI was on anti crazy pills and mood stabilizers. Gained weight and got bullied all from a wrong diagnoses😑.
Get second options on young teens before jumping on the "try a pill" rodeo.

No it's not licensed for ASPD or PTSD. They're more likely to be abused in the ASPD population as well given their potential to become habit forming when taken at higher doses. I'm a clinician & haven't noticed any discernible benefits in either clientele group unless they had co-morbid ADHD.

So interesting... I think not all the drugs were created equal, but there's a bias against depressants and in favour of stimulants. We need to discriminate between pharmaceutical drugs using only science... pharmaceutical drugs feelings don't matter
Stimulants have made a dramatic difference in my emotional wellness and my functioning. I thought I had this ongoing severe depression because of my lack of motivation and this difficult to describe fatigue that was ever-present, even when I "felt" good. I also had this anxiety that even Benzos did little to help.
My psychologist pointed out all of the ADHD traits I had, and while I didn't initially believe it because I had a poor understanding of the disorder, I eventually did go to a psychiatrist for an assessment. Sure enough, I had ADHD-Combined type paired with the Bipolar disorder I was already diagnosed with. When I started the ADHD treatment, the difference was dramatic. I no longer had to take the anxiety medication that I had taken since puberty. I had motivation to complete tasks, and I felt alert instead of like I was living life in a tub of molasses.
I do react to them in a way that may not be typical though. They never cause my problems with insomnia, even though they make me feel more alert. I describe it as, "Off ADHD medications, I could have napped all day and would never feel awake. On ADHD medications, I don't feel the need to nap during the day, but if I want to nap just for the sake of getting an extra rest in or to enjoy the afternoon sunshine, I can just close my eyes and drift off into a peaceful sleep." Even when I was experiencing mixed mania in the hospital, they allowed me to stay on my stimulants because the only time I would sleep would be about 45 minutes after my afternoon dose. I think it is because the medication calms my brain down to a point that I can then use normal strategies for falling asleep or whatever other task I need to engage in.
On the other hand, I do have a strong suspicion that I have narcolepsy, so maybe that is why I react this way? I have never had a comprehensive sleep evaluation done, and they are fairly difficult to access where I live, so I'll probably never know for sure.
I'd say no. Bi-Polar and stimulants is a baaaad idea. Increased delusions/mania incoming.
