Hi Hannah. I know this is the new updated sub-cut skill in the NMC. However, the marking criteria of the university that I am going to do my OSCE, haven't changed yet. Should I follow the marking criteria of where I held my OSCE? I am doing a self-review only. I don't want to come in unprepared. Thank you very much.
HI The test centre have said the following " The mark scheme will not be changing as there will be some situations where the candidate has to administer a sub cut injection using a syringe. It is only when administering insulin that they will use a pen, this has been implemented to reflect best clinical practice." Therefore you need to follow best practice guidelines if you have insulin pen and this is not as per the marking cirtiera. I have a tutoring student who had their exam last week and had the insulin pens she passed

Keep practising and being familiar with the medication charts remove unnecessary communication keep it systematic and structured
@HannahAmes24 Good morning!! Can I prepare the medication, educate the patient about the indications and side effects for due medication, put them in cups as I read the chart. For example, I go to once only medication ,read out the due medication, explain indications and side effects, find out if they are happy to take the medicine, get out the required tablets into a cup and put them aside before moving to other sections of the chart? Then after reading all sections and preparing the due medications,I give the patient to swallow.
@@ivanlumu7842 if this helps you with time management yes. I would just have a quick look through the chart and see what medications are due and if there are any that are contraindicated

Hi hannah, i want to ask if in the station theres no need to don apron and gloves but you do it, is it consider a fail? Thank you in advance
@@bashful030 you must don PPE in this station as it is an aseptic procedure and the risk of introducing infection via the IV route is high, if you do not wear PPE it would be a critical fail

Hi Hannah, this is unrelated but it's about Blood glucose monitoring. My exam is in a few days. Please, if it wasn't stated to do either premeal or postmeal, and the patient verbalizes that he has had a meal. Please, how many hours after a meal is considered premeal or postmeal? Also, what are the ranges for normal blood glucose in diabetic and non-diabetic patients? I know it's 4-7 mmol/dl for a non-diabetic or so.
@@iXmorph post meal is up to 2 hrs post the meal NICE guidelines for diabetes are 4-7 pre meal and 5-9 post meal

Hi marking cirtiera is not updated yet for pens but you should definitely wait 10 seconds as per NHS England and Diabetes UK guidance
Thank you Hannah. I have seen some people cleaning the pen with chlorhexidine wipes before attaching the needle and also recapping before removing the needle. May I neglect that?? Thank you
Hi there is nothing in the NHS England or Diabetes UK that indicates that you should wipe the pen, if it is the patients own pen which it normally is then there is minimal risk of cross contamination. You should not recap a used needle this would be a fail

Hi you only need to verbalize the care plan there will not be anywhere to document the only documentation will be the MUST score
Thank You Hannah for clarifying the "X" on MAR charts. If I may ask, a pre-op patient that is scheduled for surgery, during assessment I'd verbalize the importance of maintaining NPO prior to surgery. My question is, how long should the patient be on NPO for? Is it 12 or 8 hours for food and clear fluids? Secondly, when a SDH pt is on Warfarin, do I confirm for active bleeding in the last 24hrs or 48hrs?
Hi for NBM this is very dependent on the local trust policy the current evidence-based advises against prolonged periods of being NMB and most places follow 2 hrs as patient outcomes are worst with long periods of NBM and the risk of aspiration does not appear to change with a short NBM time some places allow clear fluids up to surgery. I would advise against verbalising a set time. In relation to SDH yes you would withhold if the the SDH had happened in the last 48hrs

Thank you Hannah. I still have 2 questions. * What is really considered low pulse. Is it 50 or 60 . I have found many sites indicating that we code if pulse is less than 60. 2nd question is that Is it common to prescribe low or higher doses?
@@ivanlumu7842 the NEWS2 parameters for normal pulse are 51-90 so 50 and under scores so that's why I teach 50 it is also important to use clinical judgement and ask if that is normal for the patient. From experience it is not common for the doses to be prescribed incorrectly

Hi, thank you for your amazing videos. Just a quick question, what does do you mean by 'disturbing the curtain as little as possible' . Thank you very much
I think for 2 reasons one you do not want to vigorously move the curtains as this could cause cross-contamination onto your trolley and second to maintain privacy if the patient is exposed for the dressing
what does this mean Hanna I failed my OSCE I got the Patient BP was 107/78 the rest the score was on O, So I put my total score 1 escalation Y and monitoring frequency I wrote 4-6 HRLY to my surprise I failed and this was the response -The candidate failed to document the score using the early warning score (NEWS) tool. The candidate left the section of the chart blank for NEWS Total. Accurate and complete documentation using the NEWS tool required so that the patient receives safe and appropriate subsequent care.Failing to document the total NEWS score may lead to Patient receiving incorect treatment plan and may affect future monitoring and care of patient. This poses a risk to patient risk. I remember I put my total score as 1 Monitoring frequency 4-6 HRLY and Y I dont know what I missed please help me before my resit. Thank you. I passed all the other sections especially implementation by watching your videos. I really appreceate you because I was self -studying not sponsored.
Hi I would email the test centre and ask them to review your paper and say you did write the score, so can they clarify the feedback. You can appeal it if you think you did it correctly which it sounds like you did. Well done on passing your other stations a great result.
@@bwonthuga78 personally if you are certain you did it correctly and that is your only error in that station I would appeal it the exam is so expensive to retake and 2 weeks is not a long time to wait for a saving of £400, they are only human and can make mistakes

@@iXmorph personalised signature that are unique to you. In time write the time you were asked to give and instructions you write "omitted" to indicate you have not given the medication

@iXmorph there will not be any controlled drugs due at your time to administer so don't worry about this, codeine phosphate is not a controlled drug it is also not normally a drug prescribed for your time to give either

@@Huda20248 yes you I would advise you looking at the NEWS2 escalation policy which you can get from here and you have a copy in your exam www.rcp.ac.uk/improving-care/resources/national-early-warning-score-news-2/. 0 = 12hr no escalation 1-4 =4-6hr you do need to escalate but you can also escalate to the senior nurse if you were concerned 3 =in a single parameter= 1hr monitoring and escalation 5 or more =1hr and urgent escalation and assessment 7 or more = continuous monitoring and emergency escalation

Hello hannah is it still okey even if i do my assessment not following the ABC i mean if i start my obs instead of respiratory rate first as it is in B . To do my bp on one hand and capillary then oxygen saturation on the other before i will do the RR and PR? Or it must be ABCDE in order
@@Huda20248 hi no the observations are part of your ABC and not separate you must follow ABC and complete your observations as part of this for example airway first thing then B do the resp rate and verbalise all of B resp noises signs of reps distress ect, do oxygen saturations and then move on to C and do the pulse. This is important because an issue in B is more of a priority than an issue c. The respiratory rate should be taken first before any other observation it is the most important vital sign and why it is too of the NEWS chart, an issue with respiratory rate is going to kill a patient before an issues in C and D. This is a key OSCE area that is often wrongly taught just go to the Resus council UK A-E assesment for the evidence base. It will also massively save you time in the OSCE.

Thank you so much for this wonderful information. I am also requesting to know the difference between QD and QDS in terms of Frequency. I have seen a prescription at NMC website with Flu cloxacillin 500mg QD, for 7 days to be administered at 6:00,12:00:18:00 and 00:00. I am concerned because some websites clearly state that QD means Once a day and QDS four times. Now which is correct. The due time is 12:00. Can we administer the medicine???
@@ivanlumu7842 I think this is a typo error. OD is once a day QDS is 4 times a day. QD is not an abbreviation and I think it should say QDS
