    where's the proof SGLT2 is doing anything you don't get from lasix? They make a big deal out of all the diabetics lack of heart failure but those people weren't on lasix at the time either now were they?

    I have HFpEF with normal blood pressure. My cardiologist is treating me with spironolactone and torsemide. My feet have been swelling (not really bad but still miserable) for the past two months but my cardiologist can't increase the torsemide without causing kidney function loss. Is there any new treatment that can help me? I feel awful almost every day. I am only about 5 pounds overweight and do not have diabetes.

    My dad has stage 4 small cell lung cancer and just started tecentriq.. He started developing heart failure three days later. Had vt at 150 which didn't respond to beta blockers. His legs swelled like balloons. He is at home now but high risk for a heart attack or stroke. We're not sure whether to discontinue the immunotherapy and stick to the carboplatin/etoposide combination

    I have recently had very good, comprehensive testing...but now, that it finally has been determined that I do have the HEFpEff form of Heart failure...Why , when I was first diagnosed with AFIB, was I not given these tests earlier on? I then could have pursued an Ablation operation, that might well have helped me avoid my eventual falling into heart failure...I have been dealing with the AFIB situation for nearly five years....at no time did any of my cardiologists suggest having an ablation...now I am being strung along with drug therapy, and only now am I being steered towards having an ablation--maybe too late to do me much good.

    Great presentation. In the "Mr Wynded" case study, the best course of action is to add Entresto, Ivabradine, and an SGLT2 inhibitor (presuming no fistulas, fissures, or other perineum-related abnormalities) and remove the the ACEi, as you noted. However, it would also probably require reduction of the beta blocker (bisoprolol) to 5 mg (from 10 mg) because the BP was low at 103/72. It's likely that adrenergic blockade is covered at 5 mg anyway. The ACEi dosage is relatively low at 2 mg, so it's unlikely contributing to the low BP. Introduction of Entresto at 100mg or 200mg will lower BP below 100/60 based on current profile. So, modification in dosage of BB and MRA may be warranted. Also, if LVEF does not rebound from 25 percent to above 35 percent within three months, I would strongly recommend an ICD (I would recommend a loop recorder in the meantime). This patient is vulnerable to SCD, even though he presents in sinus rhythm (for now). Finally, if he is experiencing any signs of gynecomastia (thin arms, thin legs, heavy, drooping chest suggest it), you could switch him from spironolactone to eplerenone. Thanks again.     Who is lying to us? There was a study that proved that injecting cardiomycits growth factors into scar fibroblasts can reverse and regenerate scar tissue in adults. Other study of terminally acute heart failure 11 patients (all were told will not survive 24 month) after injecting stem cells all 11 were doing well and alive after 3 years. Soo the question is why we dont have this therapy available in 2021? Is a money rip off industry who cares about funding and earning millions. that what it is! Someone is lying to the public.

    I'm so surprised this talk hasn't received more views. This is of especial interest to the low carb / ketogenic and fasting communities who promote a healthy diet often with intermittent and extended fasting. I do have a question though! At about 14 minutes in, you mention that a high-fat diet can induce diabetes. Would you kindly elaborate with more detials on that diet? I would suppose the high fat diet you're referring to is also in the presence of high carbohydrates.? From what I'm seeing, there is abundant evidence that a low carb high fat diet, such as used by virtahealth.com , can acutally, and has reversed, or greatly reduced diabetes in some patients. What fasting windows and/or frequencies have you found to be safe?

      @@southerncalifornia3155 yes. They said only if I take certain medications does it “act up”. They did a 30 day EKG and a stress test. I asked to do a blood test but they said I didn’t need it, that I definitely had the syndrome type 1.

      @@scrotemcboogerballs590 I mean I’m alright. They’ve done more EKG’s briefly and my heart hasn’t skipped. I have to avoid certain medications but exercise is still freaky to me. I no longer have numbness and tingling and pain in my chest. It’s an uphill battle mentally. The doctor told me the only symptom is sudden death so I’m struggling with that. And odds are it’ll happen in my sleep. So my sleep issues are not better than they were.

They found some weird line on my ekg and they said they weren't really certain if it could be brugada or if it's just something else. I know about the sleep thing and that paralyzes me with fear.
