Clogged Milk Duct while Breastfeeding
A clogged (or plugged) duct is very painful and if untreated, it can lead to an infection (Mastitis). In this video I will walk you through the steps to clear the clog.
In the video, I mentioned a massager for clogs. it is also helpful to increase a letdown. great for pumping moms too.

      @@maryamshaheen5118 How old is baby ? It can definitely look like a low supply issue at night when it’s really not. I’ll attach a couple of videos here. We do have slightly less milk in the evening but it’s slightly higher in fat. Baby will sometimes need to eat almost non stop in the evenings. The extra fat is great for growth and development and it helps baby to sleep a slightly longer stretch. Baby will often have very fussy times in the evenings. This will be difficult for a few days to a week, then better for a few days or a week. Each baby is different though. In the evening, baby can be over stimulated, overly tired or hurting from growing pains. It hurts to grow so quickly. This makes moms believe that supply is low. You can definitely give formula and baby will get really full and fall asleep. We are all sleepy after a huge meal. If you want to keep supply up, you’ll need to pump at around the same time as baby gets formula. It’s helpful to feed from both breast, then give a small amount of formula then back to the breast. If baby still seems hungry, give more formula. My baby is 2.5 months old now, she doesn’t drink from one side of my breast.. I use to hand express and take it away.. my milk doesn’t come out from the breast she doesn’t suck using a breast pump so I hand express by placing a heat pad. I give formula 2 times a day..is it enough for my baby??? If not how should I schedule my breast feeding and formula feeding pattern?

    • @marycahillclc
      @marycahillclc 4 часа назад

      @@zaynsmusics8334 You can know for sure that baby is getting enough, if you always see 6 or more wet diapers every 24 hours. It’s ok to just feed off of one breast. That one breast can totally make up for the other side. Just like a mom can breastfeed twins from 2 breasts, you can usually completely feed from one. It’s best for supply if baby is on the breast as long and as often as possible. If you can (at each feeding) offer the breast at each feeding, then give a small amount of formula then return to the breast. If baby still seems hungry, give more formula. Let baby use you like a human pacifier when you can. If you decide to give full formula feedings, you should be consistent at feeding times. So if you give formula every day at around 11am and 5pm, stay at around those times. If you’d like to keep your supply up fully, you’ll need to add lots of breast massage and expressing to keep supply up. Hand expressing is excellent to keep supply up. How much milk are you getting each time that you express?

      @@nssylla Are you still nursing in the day or is night your only nursing? I’m so sorry mommy 😥 Night weaning can be very difficult. I personally would plan for a weekend to wean because it will be a few stressful nights. Wear a button up shirt or something that will be hard for baby to get to the boob. If you nurse before bed, say that there will be no more milks until morning. When baby cries for mom, give baby cuddles but no boob. Instead of saying “no” say “in the morning” or “later”. You can offer a drink of water and say “this will make it better”. If baby sleeps in a different room, you might have someone else get up in the night instead of you going into baby’s room, that “might” be easier but no matter what you go, it will be rough. 😥❤️

    Hi Mary, my baby boy is 2 1/2 months old. He is in combined feeding and not latching at all. He is having more than 6 diapers and weight gain is normal. I am pumping 5 times per day not able to more than that. Ia m getting 10 oz per day more or less. My questions are 1) how to get 24 oz per day with 5 times oumping per day? 2) Is it ok to pump for 40 minutes or more per session? 3) Is it possible to achieve 24 oz per day by pumping 5 times per day without night pumping?

    • @marycahillclc
      @marycahillclc 4 дня назад

      @@meenumeena8944 I don’t know if you could get up to 24 ounces with your five pumping but you should be able to get your supply up more than it is. At each pumping session, you should stop pumping after it’s been about 10 minutes since you’ve seen drops of milk come out. But you can take a break for about 10 minutes and pump again. During that 10 minute break you should be massaging your breasts. Adding the second pumping will increase a second letdown. It would be good if you can do this at every pumping if you have the time. It’s important that you pump at around the same times every day and have those pumping times spread out as much as possible. It’s generally not necessary to pump through the night. Getting in one pumping in the morning that is as early as possible, is helpful. You should be seeing the largest amount of milk pumped in the morning. By late afternoon, it’s normal to be getting much less, but do be sure to pump anyway. It’s important to add lots and lots of breast massage before and during pumping sessions. If you can take 5 to 10 minutes to massage your breast before pumping, this adds stimulation to your breast and tells your body to make more milk. Massage is a big important key when pumping. Applying heat before pumping can help to speed up and increase your letdown. It will take 3 to 5 days to start seeing an increase in supply after you’ve been very consistent with times and adding lots and lots of breast massage. The increase will probably just be a few mls in the beginning, but then will very slowly increase. After about eight weeks postpartum, our milk supply regulates down to what baby needs. This is if baby is feeding from the breast. When you are pumping, it’s up to you to tell your body how much milk you need. Be sure that your pump parts are replaced if needed. You might try a manual pump and hand expressing. Many moms can get more milk out by hand expressing and hand expressing ads more of the needed breast stimulation. Even when baby receives partial breastmilk, the benefits are great for baby. Those benefits will stay with baby for the rest of their lives. ❤️

      @@meenumeena8944 Latching is best for increasing supply if you’d like to but you can add 1 more pumping session and really focus on breast massage. In a couple of weeks, you be amazing at the difference. Massaging should be comfortably firm and the whole breast. You can even massage through (on top of) your shirt. You can use your wrists to massage if you want to be a little discreet about massaging. ❤️

Milk use to have pretty good fat content but I noticed my milk have become very watery and not much fat on the top anymore. • @marycahillclc
      @marycahillclc 5 дней назад

      @@baelau8917 That would be a good video idea. I should also do one on fat content. Even though your milk looks thin or watery, your milk still has all of the fat that baby needs. You really can’t tell the fat content by looking at the milk. Even if your milk was lower in fat, it still has more fat than cows, milk, and its nutrients are better absorbed than formula. (the formula is a good alternative to breastmilk when a mom can’t or doesn’t wish to breast-feed). The only moms that need to be concerned about fat content in their milk are moms that have a forceful letdown/over supply. Do you believe that could be you? Is your baby growing well and staying on the same growth curve? And formula at any age, should be mixed with the proper amount of water if it’s in a powdered, concentrated form. So if you want to mix it with breastmilk, be sure to add the water to the formula first. Though breastmilk will be the most nutritious for baby, even if your milk was lower in fat.

If he wakes up and fusses from burning and changing, she can bring him back and nurse from the other side. Breastfeeding helps your body to heal faster but it very understandable that you are extremely tired and sore. You just grew a baby then had your body cut open 🥲 You’ll need to pump as often as baby gets formula. It’s common for baby to not like one side. Just keep trying. He might always fuss about that side. There’s many reasons why a baby might not like one side. It could be the shape of your nipple, how fast or slow the milk comes out, the taste (it can be different from side to side) or that he’s not comfortable laying on one side. Try to change the position while nursing on that side. If you’ve been nursing him in a Crossbody/cradle position, you might try a football hold. Baby doesn’t like one breast, the other side will make more milk to provide what they needs. You can always know that babies getting enough you see seven or more diapers every 24 hours from the time that baby is seven days old and older. Letting baby be on your best as long as possible. If you’re afraid that you’ll fall asleep with him, ask someone to stay in the room with you. 😴 How old is baby now?

There is no need to worry about this because breastmilk has lots of fat, even if it appears watery. Our morning milk will usually look thinner than our evening milk. This is very normal. @@tsionendale6326 That changes things a bit. babies who are exclusively fed from the breast take in an average of 24 ounces every 24 hours. 40 ounces is a huge amount. Making this much milk will actually cause your milk to be slightly lower in fat. It can also cause baby to have tummy issues. Baby may be diagnosed with reflux or even misdiagnosed as having dairy sensitivity through mom‘s milk. Many doctors do not understand the condition of forceful letdown/oversupply. It will be best to get your supply down to under 30 ounces every 24 hours. You need to do this very slowly so that you don’t get a clogged duct. I have a three part video series on your condition of forceful letdown/oversupply. The first video is just to help you to determine if you have the condition. We give her formula at night, but I don’t like doing that due to my low breast milk supply. During the day, I breastfeed, but I don’t use a breast pump. @@fathimasabiha6606 Hi mommy! Is this your first baby? If not, how was breastfeeding with your last baby? How many wet diapers has baby had in the last 24 hours? What color is baby’s poop now? Do you see and hear baby swallowing while nursing? Are your breasts feeling larger (slightly engorged)? Mature breastmilk is usually in between day 5 to 7. First time moms can be longer (day 7 to 10). If there was a traumatic birth (like a C section), breastmilk can be a few days slower coming in. You will have Colostrum for baby. Colostrum is available after 20 weeks of pregnancy. Colostrum is “usually” all that a newborn needs until mature milk is in. You are correct that giving formula will reduce supply but occasionally it’s necessary. To keep supply up, you need to be expressing milk at around the same time that baby gets something in a bottle. You can pump or hand express but it’s important to express if bottle feeding. Say to yourself “if anything goes into baby, something should come out of me” When you do give a bottle, it is best for supply to feed like this: Offer one breast for baby to nurse on as long as possible. When baby seems done or is falling asleep, burp baby to wake her up a little, then latch her on the second side. Sometimes baby won’t want the second side, sometimes she’ll nurse for just a few seconds or sometimes she may keep nursing For a few minutes. Either way is fine. If baby doesn’t seem satisfied after nursing on both sides, you can offer a small amount of formula then latch baby on both breasts again. If baby still seems hungry you can get more formula. Having baby on the breast as often and as long as possible is best for supply.

      @@deahnabarcelona9915 hi mommy! Do you have a forceful letdown/over supply? How old is baby? Your breasts will start to feel much softer and less full but still make lots of milk. When your breast feels full, that’s inflammation of the breast milk isn’t stored in your breast. It’s made while you nurse or pump. If you don’t have a forceful letdown/over supply, it’s best to offer both breasts each feeding in the first few months. To do this, you would nurse on the 1st breast for as long as possible. When baby seems to be done nursing, offer the 2nd side. Baby may not want it, or just nurse a few seconds or nurse for several minutes. We want to nurse this way because (you’re right🥰) we want baby to get the fattier hind milk but when we offer the other side in that feeding, it helps to keep supply up if you’re a mom with an average milk supply. ❤️

      @@asifullah4567 Is baby having 6 or more wet diapers every day? If baby is having 6 or more wet diapers every 24 hours and is gaining weight well, then you know that baby is getting enough. Breastfed babies eat more often than formula fed babies. A newborn should be nursing 8 to 12 times a day. If baby is a slow eater, they might nurse every hour. The afternoon and evening times, baby may want to nurse almost non stop. Baby will start to nurse less often but will have times that they nurse almost nonstop again for a few days. If you’re concerned that baby isn’t getting enough, you should try to see a lactation counselor or your doctor as soon as possible. Formula is sometimes necessary if baby isn’t having lots of wet diapers or isn’t gaining weight. If baby needs formula or you would just like to give it, keep in mind that your supply will drop unless you pump at around the same time that baby receives formula. On average, babies take in about 24 ounces every 24 hours when they are exclusively breastfed and feeding from the breast. Sometimes babies will take in more than they need from a bottle. We definitely want the baby to have all that they want and need, but it helps to have a slower, flow nipple and take a couple of short breaks during the bottle feeding. (this is called paced feeding). Keep in mind that our milk supply goes down as the day goes on. This is completely normal. When we have less milk in the afternoon, it is higher in fat.

    Hi Mary, Thanks for your video series on OS/FL. They are really informative! I have read your responses to comments in your videos too, and they are truly helpful as I can relate to some of the questions that viewers had posted. I hope you could enlighten me on some matters as follow: 1. Does baby suffering from reflux due to OS/FL tend to struggle clearing out secretions (e.g.; saliva) especially upon waking up? My baby tends to grunt, move his head left and right, and cough out or gag when waking up-not always but often since i suspect i am overproducing. 2. Me and baby exhibit the signs of OS/FL that you have mentioned in your videos. Hence, i started 3-hour block feeding. It makes the other breast really engorged and painful before its turn to be fed on, but i am managing it with cold compress and i am refraining from pumping it out despite the pain and pressure. A. In a 3-hour block feeding, supposed that the designate breast to be fed on has not been emptied during the 3-hour period, should i left it be and proceed to the other breast during the nect block period? B. I wonder how long does it usually take for block feeding to work in controlling my milk supply? C. Would the absence of engorgement be the indication that OS/FL has been corrected? D. What is the risk of developing mastitis with block feeding especially that the other breast is left unlatched on for approx 6 hours in a 3-hour block feeding session (with 3 hours being the block feeding session and another 2-3 hours before baby usually demands for another feed)? How to prevent it from developing? 3. From your videos, i gathered that lactose in foremilk makes baby gassy and irritates his GI tract. Is there a way to avoid this while mommy addresses the OS/FL issue or does baby just have to deal with it while the issue is being addressed? How bad could continuous ingestion of high amount of foremilk affect baby? Looking forward to hearing back from you. Would really appreciate if you could spare your time to clarify my concerns. Thank you!

      @@joyanntan1289 Let me start by asking, how old is baby? Is this your first baby? If not, how was BF with other babies? I can’t say for sure that baby might wake and have trouble with secretions but it would make sense to me. If most people consumed a large amount of high lactose before bed, they might wake up phlegmy. When you start to block feed, you will probably need to do a little pumping on the side not nursed on. It’s important to go down very slowly so that you don’t get a clogged duct. There will be things that can put you back to the beginning and you’ll have to wean back down again SLOWLY. If you get a clogged duct, you’ll need to keep moving milk so it doesn’t turn to mastitis. It’s a bummer to have to start again but it’s necessary 🥲 You must pump if you’re uncomfortable. If you are engorged, pump for just a minute or two, then wait 5 to 10 minutes and see how you feel. That breast will probably soften up in the 5 to 10 minutes after pumping. If not, pump a little more. I know that engorgement seems like the breast hasn’t been “emptied” but it’s not really milk in the breast, it’s actually inflammation/swelling of the tissue around the milk ducts. Your body is wanting to make milk at that moment but if you start letting it know that you don’t need it, production will start to go down. Theres no way to know how long it will take to get supply under control. Every mom is very different. Some moms get it under control in days or weeks and some moms may never get it totally controlled. The further out you are postpartum helps. Supply regulates over time for most moms. Yes, less engorgement shows that it’s getting under control. Some moms try to pump a little bit before nursing to remove some fore milk but this is a BAD idea. Removing extra milk will just increase supply and make it worse but if you have some previous pumped milk (or milk that you needed to pump when you’re engorged), you can give a little bit of that at the beginning of a feeding. This works because it reduces supply a little faster. It reduces supply because of less milk removal and because there’s less breast stimulation. Some moms will add in formula or donor milk to reduce baby’s time at the breast to help control supply. I hope that helps. Let me know if I didn’t answer all of your questions or if you have more questions. Moms wish they had an over supply but they don’t understand that it can be harder than low supply. 🥲🥰

Regarding your advice on pumping out engorged breast, when is the best time to pump-is it as soon as i feel it being full or do i have to wait for its turn to be nursed on and pump just before or after the nursing session? How much is enough pumping-is it til engorgement is gone or just until it becomes less uncomfortable (but still engorged)? For me, engorgement seems to be only completely gone once baby nursed on the breast for a good 12-18 minutes. Does clogged milk duct feel painful or not necessarily? My right breast seems to react to block feeding quite well-feeling less engorged than before even when not nursed on. Usually survives the 6 hour period of not being nursed on without being extremely hard and painful. However, the left breast still gets engorged pretty quickly-within 2-3 hours from being nursed on. Also, i was unable to bear the engorgement yesterday so i pumped out for 5 mins and felt just a bit of a relief but it is still engorged. I seem to notice some slightly hard lumps on that breast too, but they aren’t really painful. Could those be clogged ducts?

    My baby is 2 months and 10 days old. I made a mistake by introducing the bottle too soon. I didn’t keep her on the breast often enough when she was a newborn. She struggled to latch correctly, which affected my milk supply. I didn't realize I needed to do more skin-to-skin contact to help increase my supply. I’ve been using a pump, but I can only collect a little milk during each session-about 20-30 ml total. In the last two days, I've only been getting around 10 ml or even nothing when pumping. I've been putting her to the breast more often recently and working on breastfeeding. However, she often falls asleep very quickly and doesn't nurse firmly enough. But when I feel my breasts are full and engorged, the baby nurses strongly and steadily. However, as the evening approaches and the milk supply decreases, the baby tends to nurse weakly and doesn’t want to feed. I'm wondering if I can increase my milk supply now that my baby is over one month old. I would be very happy if you responded to my comment.

      @@nguyetanh1245 The best way to increase supply is by having baby at the breast as often and as long as possible. Put baby to the breast before and after any bottle feeding. It’s very normal to have way less milk in the evening. Our evening milk is higher in fat. Baby’s usually nurse much more often in the evening. They can be fussier in the evening too. Hi. I’ve had to start giving my daughter top ups of formula as she wasn’t gaining enough weight. I had to do this in her first 2 weeks as she wasn’t latching properly but I exclusively breast fed for 5weeks and then she got weighed and had only put on 5oz 🥲 she was born 6 pounds 5 oz and was 6 pounds 10oz at 7 weeks, plenty of wet and dirty nappies, really happy within herself but as she wasn’t gaining enough weight they said I need to top her up after each feed and I’m not able to pump enough to just top up with breast milk so I have to give her formula now as well. Im pumping after each feed and only get 2ml- maximum of 15ml. I’m topping her up every other feed or so and She gained 7oz in 6 days! I haven’t had her weighed this week yet but she clearly wasn’t getting enough from just breastfeeding. I really don’t like having to give her formula and I’m trying to eat better and I’ve seen a slight increase since trying fenugreek (literally only 2 teaspoons over the course of 2 days) but I’m really stuck as I wish I could exclusively breast feed again or at least be able to pump enough to top her up with breast milk 😩 she’s now 9weeks old any advice? Xx

      @@BelleAmorePizza Sometimes supplementing is necessary. Don’t be hard on yourself about it. You’re doing great! ❤️ If you haven’t already, I would see a local lactation counselor to do a weighted feed. You can do this at home if you have a scale. An average feeding from the breast is around 2 ounces. It might be higher in the morning (3 or even more ounces). In the evening it will be lower (could be around 1 ounce a feeding. Afternoon and evening milk gets less in volume but it’s higher in fat). Feeding from the breast for an exclusively breastfed baby is an average 24 ounces every 24 hours. This is for babies who are 1 months to 6 months old. Formula fed babies need more in ounces though because it’s not absorbed as well. The foods that you eat and even supplements, don’t increase your supply. Fenugreek can even reduce supply for some moms. If baby is having 6 or more wet diapers every 24 hours, you know they are getting enough. Though if baby is not gaining well, something is obviously wrong. Supplemental feedings are sometimes necessary. Especially for newborns. When baby is having lots of wet diapers but slow weight gain, it is usually a sign that mom has a forceful letdown/ oversupply. I know that may sound crazy because baby isn’t gaining weight but low or high weight gain is one sign. Low weight gain is the most common result of oversupply. Most moms with an oversupply can pump a large amount but there are moms who don’t respond to a pump. Is she choking or at least gulping very quickly during a letdown? Is she having tummy issues? Reflux? Green or goopy poops?

      @@farhanamuhammed3994 Breastfeeding with a forceful letdown can be very painful because baby will try to bite down and slip out to the nipple to try to control the flow. Once supply gets more under control and baby can get in a deeper latched position, it usually feels better. As far as time to feed, every baby is different. If baby seems satisfied after 5 minute feedings and is always having 6 or more wet diapers every 24 hours, then you know baby is getting enough. My baby was born premature so my milk had not come in enough for her at the hospital so they had her on formula. We have continued to bottle and breast feed her but the breastfeeding can be really inconsistent. I’ll get her to latch and she can suckle for 5-10min and fall asleep for a little bit then want to feed and we can go for another 5-10min but then she gets very fussy. I’ll try to get her to latch and she either cry’s and pulls away or try’s to suck a moment then pulls back. I’ll message my breast for a minute and I’ll let down milk, so there is still some there. I’m not sure if my let down slows or what the issue is. It also happens when I hand express my milk lets down in strong bursts, kind of like a water pistol. I’m not sure what the issue is, I really want to move my baby to full time breastfeeding but this has been the main issue.

      @@imnowan1363 you can ask all the questions you want! ❤️ Baby’s tummy is sometimes bigger after formula because baby takes in more than breastfeeding. Bottles let the milk or formula flow fast so baby’s often don’t realize they are full and over eat. It won’t harm baby to overeat but some get a tummy ache later. Our bellies get big when we overeat too. (It can be uncomfortable for us, and it makes us sleepy too) When we breastfeed, we know baby has had enough when they come off the breast. You should offer the other breast (older babies don’t always take both breasts at one feeding).
