Hey! My name is Nathan Carlson and I’m a physical therapist, coach, and consultant based out of Kansas City.

4th metatarsal stress fracture, wearing boot 6 weeks, extra calcium, vitamin D, Zinc, magnesium. Aspirin.
just got out of boot camp not too long ago & im nervous or questioning if I should see medical before I go to MCT school in January or not going at all bc it may give me enough time to heal

Getting a bone density test
Stress fracture = Bad bones
so when at the recovery phases do one start the strength training with squats, deadlifts etc heavy? Is it once you can walk pain free after the 3-4 weeks of no load bearing of for example the foot?
I will normally have patients complete 1-2 weeks of exercise with body weight or band before advancing to squats, deadlifts, lunges, etc with external load.
@@NathanCarlsonthank you for the reply! So when can you start body weight exercises? The same time you start walking, in other words, after the initial 3-4 weeks of no weight bearing?
@@eskility with most of my patients I'll have them complete 1 week of ADLs and then begin progression to bodyweight exercises.
@@NathanCarlson thank you again. I guess it is smart to keep doing bodyweight for a while before adding heavy strength training then? How many months after no pain in ADL before it can be resumed? Squats with 5x5 100kg and unilateral calf raises for example? I would assume that puts a big load on the metarsals.
At what stage in stress fracture/reaction recovery can you start with heavy strength training again? Unilateral calf raises, deadlift, squats etc. Is it after the 4 weeks intial phase with no weight bearing? At the phase where you start walking. Or should you wait longer?
i have navicular stress fracture i have a POP cast on for 4 weeks now, so far i have zero pain infact i can move my foot completely (up and down)in my cast i can also rest my foot on the ground but sometime it swells . does this mean its healed or not? asking u cz if i ask my doctor he will charge me a bomb....

How can we differentiate between patellofemoral pain syndrome and a bone stress injury of the knee?
Good question. In my experience, PFPS tends to present more near the actual patella, often a bit medially. BSIs are rare around the knee, but are often painful with direct palpation of the medial/lateral condyles of the femur and tibia.
Nearly 5 months post lower tibia stress fracture and I still walk with 1 crutch. I hope my physio will build a programme for me to go back into running in the future and strengthen my bones..

The most accurate test I've seen on the internet for hip stress fracture/reaction
I’ve had a tibia stress fracture. Been out for over 2 months and still get discomfort. What should I do to build strength without aggravating?
I would try barefoot, more support more cushion is a never ending journey. Like never doing exercise, your feet are muscles and need to be worked out. But of course be careful as I am not sure of your situation. For me it has been the best thing I have done for my feet. I am wearing Hykes Flex which are barefoot and zero drop, my feet feel so much healthier and stronger.

I'm currently trying to become a PTA, and I'm doing some research on books that are helpful for PTAs, as reading is a big hobby of mine. If you could do a video on reviewing different workbooks and such and making a sort of top 10 list or something like this video, that'd be so helpful and awesome. Great stuff!
Jordan’s are toe crushers. Never really been a runner, Iam just trying to walk again with out pain. I just bought some altras. Might try to find a cheep shoes with a heel drop and see if that helps. Basketball shoes in general are to skinny.
I have the Hykes Escapes and have noticed a change in my feet and posture, will not be going back to regular shoes anytime soon, It becomes obvious they are better for you once you see and feel your feet change. Just read born to run. :)

Only 100? I do like a 1000 lol and I have around 12-15% body fat. 10-20 miles a week
In the case of a shin splint, can you see a photo showing swelling around the bone on X-RAY?
I had a fractured there 😞 8 year ago , my foolishness is to not use plaster , but now .....
So better to start walking again with hiking shoes, I guess? About to remove my aircast in 2 weeks, after metatarsal fracture. I don't mind discomfort, as long as I can be sure it's not damaging anything...
I suffered trauma to this bone on my right foot. My foot was wedged between a 420lbs flatbed and metal plated flooring inside a box truck. Smh thankfully no fracture. It’s been healing for 4 weeks and I still feel minor pain. Though I am active on it. First 2 weeks, we’re unbearable.
Hej Nathan, very good explained, acctualy the conclusion is that the bone stays not healthy if somebody only do long runs without extra training. I got a femor stress fracture of that, 5 month ago ( yes i tried to change my running style aswell.., and changed my shoes in carbon 😁) Then it sudden colapsed..). I am know nearly 5 month in my recovery ( healing) and started minimal very very short jog minutes, ( took that advise) only 2 times a week and try to walk distance so the push off from the ground grows stronger. As i understood i should do excersices to grow stronger the femor bone with deadlift..but thats to early, is there a lighter option excercise ? To get a little healthy stress on the femor 🤗? My bone density is quit ok. 😊

Great video. Thanks. I have navicular bone pain. I am not a runner. Just a walker. My doctor said it could be tendonitis. Is this possible ? Does the tendon run over the inward pointy bit of the navicular bone ? Thanks
Very helpful! I've been in a CAM boot breaking my fibula 6 weeks ago. I'm now at Full WB, but haven't started weaning off the boot yet. I see my doctor in a couple of days and this video will be a great thing to keep in mind!
How are you doing now? Are you back to running now?
@@jomymatthewsI got rid of the boot at 8 weeks. I'm now at 11 weeks and I have graduated to jogging on the treadmill for 10 minutes, 3 times to-date. Feels OK, but I'm not ready to do it outside on pavement yet. I have no issue walking now.
@@ainslieann That's a great update. How about the stretching ? Do you feel OK with the stretching of your leg and all before running? What's hindering you from trying out the pavement running until now?
@@jomymatthews it's probably mostly mental, but I do feel a little something in my ankle even when jogging on the treadmill. No issues stretching. I do practice yoga, too.
@@ainslieann I hear you. What do you feel in your ankle on the treadmill ? Do you think you need some more healing ?
Pain is more local but doesn't hurt after warm up and also if I use me calves less when running it doesn't hurt

I don't think this is an accurate interpretation of the study referenced. Muscle density is not a predictor for bone density. HOWEVER, muscle density IS highly coorelated to bone density as individuals with good nutrition and exercise habits will (ideally) have both characteristics. People with little muscle density likely do not have good nutrition or exercise, meaning likely low bone density. This is like saying ice cream sales going up means shark attacks will go up. This is not true, but the two are positively correlated as a resultant effect of summer activities.

Found the video very helpful. I had a stress fraction last year at the left side of my shin. The doctor said it would take 6 weeks, then I could start running slowly again. Then it healed, I felt no pain. Now recently I've been feeling slight pain in the same area. Gonna take it super super slow this time. Injury is a bih😢

Could you give an example of an external que for a runner?
Sure! I’ll use the cue “land soft” a lot with runners or will cue them to smash through the ground when performing weight bearing exercises like squats, deadlifts, etc.
You forgot spine
Had a distal fib fracture with a plate and screws. 6 weeks nwb. Doc said 1-2 weeks in the boot fwb. I ditched the crutches straight away and the boot after 1.5 weeks because it was just keeping my ankle stiff and swollen. I think once fwb the sooner you can exit the boot the better imo

running shoes aren't even ok for squats and deadlifts with the amount of cushion even in stability shoes. buy a minimalist shoe or a pair of chucks

All great models as long as the athlete finds them comfortable.

You’re bang on. The only self test that showed my Femur stress before scans was the offload of pressure on my quad during a lean forward.
I have medial malleolus fracture and doctors fixed my fracture with orif surgery I m very confused regarding ankle movement and exercise when I can walk with full weight bearing and I have four week can I start exercises

Do NOT, I repeat…DO NOT LISTEN TO THIS GUY!!! I went and bought a pair of Jordan’s for my Achilles tendinitis. As a floor nurse, I was desperate for a nice pair of shoes for my Achilles tendinitis. I’ve worn various Hokas (which were nice), Brooks (durable, not as comfortable), and Sauconys (literally fell apart). I ended up wearing my hunting boots-Irish Setters, and they were the best. I bought $175 Jordans and I damn nearly had to crawl to my car after my 12 hour shift. My bone spurs are so swollen, my feet aching, and I was in SO much pain. Another nurse said, “Jordan’s are more for style, not comfort”-I’m like, really? Because Jordan’s are the ugliest damn shoes I’ve ever seen. Made in China. They are torture devices
