Why You're Waking Up in the Middle of the Night
This brief video discusses the phenomenon of middle-of-the-night awakening, a form of insomnia that can be both a symptom (e.g., of a major mood disturbance) and a side effect. Although there are medications one can take on an as-needed basis in the middle of the night to get back to sleep (tune in for details), the key to managing this all-too-common side effect is to avoid getting out of bed for any reason (you don't really have to pee!), and practice mindfulness and _acceptance_ until, lying there in the dark with your eyes closed and becoming increasingly bored with it...you drift back off to sleep. Rinse and repeat as often as necessary, until morning.

The Anxiety Disorder That Never Skips A Generation
Просмотров 7016 часов назад
In this video, I discuss how major anxiety syndromes that are genetically-based are passed from one generation to the next, and how the astute clinician should approach diagnosis, once conditions like Generalized Anxiety Disorder and Panic Disorder enter the differential, by honing in on two elements of the history: age-of-onset, and immediate family history. These histories can be tricky to pr...

Tunnel Vision: Indulging an Alternate Reality

This is a brief follow-up to my "Esoteric OCD Symptoms, No. 3" video, which discusses the phenomenon of involuntarily emotionally reliving traumatic experiences in recalling or recounting them to others, a phenomenon I call "Re-living." In its most pronounced form (when experienced in isolation), there is a momentary sense of derealization, followed by a mental re-enactment, which can include r...
Is Your Psychiatrist Treating YOU, or Your Diagnosis?
Просмотров 7421 день назад
In this video presentation, intended for medical students, psychiatry residents, nurse practitioners and other trainees, but also appropriate for, and surely of interest to, the laity, I discuss the all-too-common mistake mental health practitioners make when approaching patients: treating a diagnosis-a concept of illness-rather than the person before them, and in so doing often selecting the w...
How to Measure Your Blood Pressure Like a PRO
Просмотров 5121 день назад
This re-release of one of the very first videos I recorded for the benefit of my private patients demonstrates the correct technique for measuring your blood pressure, what kind of readings should provoke doubt and remeasurement, why it is absolutely vital (no pun) to measure your blood pressure on a regular basis, and which home sphygmomanometers are the easiest and most accurate machines. Dr....
Denial: Hidden Dangers in Plain Sight
Просмотров 8428 дней назад
Most of Freudian psychological theory has been discredited, but the ego defense mechanisms as a class are a particularly useful exception: largely unconscious or preconscious sleights-of-mind the psyche indulges, in order to avoid anxiety, guilt, shame, the perception of inadequacy, cognitive dissonance, and other subjective psychological threats. Denial is one of the most common, and well-reco...

The Astonishing Behavior of Twin Molecules
Просмотров 69Месяц назад
These clips from my Celexa/Lexapro and Provigil/Nuvigil presentations recap the concept of chirality in chemistry: that some molecules, enantiomers, exist in 2 forms, right-handed and left-handed versions, mirror images that seem identical, except that they are non-superimposable in three-dimensional space, and therefore they interact with nature in distinct and astonishing ways. In the case of...
The Greatest Generation?

A comprehensive review of the major tricyclic antidepressant agents: amitriptyline, nortriptyline, protriptyline, imipramine, desipramine, doxepin, amoxapine and clomipramine. This presentation explains these medications as a class (serotonin-norepinephrine reuptake inhibitors, anticholinergic agents and antihistamines), their actions and major side effects, and briefly reviews each member's un...
Yawning Fits and What To Do About Them
Просмотров 62Месяц назад
Few patients realize that the massive yawns they experience in response to some antidepressants has nothing to do with being tired or sleepy (which is another, common, but unrelated side effect). Understanding the nature of these unnecessary, unsatisfying yawns is the key to suppressing them.
2025: Defining Addiction
Просмотров 52Месяц назад
This is part II of "Defining Addiction." Whereas part I focused on historical definitions of "abuse," "dependency," tolerance and withdrawal, in this video I give an overview of the current nosology, where defining what clinicians still _colloquially,_ interchangeably and somewhat confusingly (see part I) call "substance abuse" and/or "chemical dependency," falls under a unified diagnostic para...
The Surprising Connection Between Norepinephrine and PAIN
Просмотров 110Месяц назад
Most people know that a surge of adrenaline (epinephrine/norepinephrine) is accompanied by a temporary increase in the speed and strength with which skeletal and cardiac muscles can respond to emergency situations, but few laypeople know that norepinephrine, at the level of the spinal cord and in the brain, also modulates the perception of pain. Can the neurotransmitter and hormone that gives u...
Is Seroquel For Sleep Worth It?
Просмотров 120Месяц назад
Is Seroquel (quetiapine) Really the BEST Sleeping Pill for You? This presentation, a coda to my recent "Insomnia Armamentarium" video, answers the question, "Is Seroquel an acceptable sleeping pill?" Quetiapine as a hypnotic agent is in widespread use, but is it really safe and well-tolerated? This is an 8-minute excerpt; watch the whole video for a more complete presentation of this second-gen...
Defining Addiction, Part One: Abuse, Dependency, Tolerance & Withdrawal
Просмотров 44Месяц назад
In this first of a two-part series, I explore the terms _addiction, abuse, dependency, tolerance_ and _withdrawal,_ taking an especially close look at the former* "Substance Abuse" and "Substance Dependency" diagnostic categories, in order to best understand the historical meaning of _abuse_ and _dependency,_ as psychiatrists understood them, clinically. *According to the DSM-IV-TR, the Diagnos...
