    Physical therapy only helps so much. It's a matter of looking into other medical options and lobby our congress members to push to have a variety of procedures paid by insurance even if you are not over 65!

    Hi Sir, Below is the report stated in my MRI report. Should I go for surgery or physiotherapy can heal it. FINDINGS: JOINTS: Tibiofemoral joint shows normal alignment. Patello-femoral joints shows normal alignment. Mild joint effusion noted. BONES & CARTILAGES: Grade II osteochondral injury seen in the lateral femoral condyle measuring 10 mm showing subchondral fracture with underlying marrow edema. Contusional marrow edema is seen in the posterior aspect of lateral tibial condyle. Normal marrow and cartilage signal intensity noted in rest of the visualized bones. LIGAMENTS: ACL: High grade mid segmental disruption. PCL: Normal in course and caliber without obvious discontinuity of fibers. MCL & LCL: Normal in signal intensity. LIGAMENTUM PATELLAE: Signal changes without disruption of fibers - Grade I sprain. MENISCI: MEDIAL MENISCUS: Normal in contour and signal intensity. No evidence of tear. LATERAL MENISCUS: Normal in contour and signal intensity. No evidence of tear. MEDIAL & LATERAL PATELLAR RETINACULUM: Intact. QUADRICEPS TENDON: No significant abnormality. MUSCLE PLANE: No significant abnormality. NEUROVASCULAR BUNDLE: No significant abnormality. IMPRESSION: Grade II osteochondral injury in the lateral femoral condyle. Contusional marrow edema in the posterior aspect of lateral tibial condyle. High grade mid segmental disruption of anterior cruciate ligament. Grade I sprain of ligamentum patellae. Mild knee joint effusion.

    This is the best review I have ever seen! Thank you. I have tried several of these remedies but so far nothing works for long. My insurance will not cover the stem cell injections. I had a total knee replacement 6 years ago on my right knee and I have never know. A day without pain in it. I’ve seen 5 doctors and four of them just said they didn’t see anything wrong from the X-rays. The fifth one said he didn’t like going behind another doctors work and recommended I return yo the original surgeon. That was no help as I had done that three times. I’ve needed my other knee replaced for many years now, but I’m still too afraid to have it done after the last one. It very depressing. I may have to try again since I’m now out of options.

    My result is HIGH GRADE ACL TEAR due to basketball But after 2 days i can squat lunges any leg work out with full range of motion. No knee instability. NO KNEE PAIN. Conservative rehab treatment on going.

    Prolotherapy helps me and I have bone on bone but it's expensive - about $150 per injection. The doctor gives me a shot full of peptides then puts nitrous oxide in. It lasts a couple weeks but a couple weeks without the pain is worth it. I know I need a partial shortly

      Thank you for your comment and for sharing your experiences. We stopped doing à la carte injection only treatments during COVID when hospitals shut down and we were unable to perform knee replacements anymore. We had been performing à la carte single PRP injections since 2008, Bone Marrow Cell injections since 2017, MLS laser since 2017 and V-Fit total body metabolic fitness (the most important piece of the puzzle for long term joint pain management and relief in our experience) since 2009. Each one of those absolutely works to help patients like you we found over the years, HOWEVER, when COVID hit and we could no longer perform knee replacements, we had a group of patients who were desperate for pain relief, and increased function. they asked if there was anything we could do, and we decided to put all of those components together into one program (there are also several other components that we utilize at times, but those four are the heart and backbone of most of our programs for patients suffering from what you suffer from) and, as we suspected, “The Whole Was Greater Than The Sum Of Its Parts”! In other words, those individual “à la carte” components worked SYNERGISTICALLY , much more effectively than utilizing them as individual parts. Hence was born our trademarked “Knee Repair, NOT Knee Replacement TM” brand/systems/programs. Yes, these individual parts were not covered by insurance still, even though, during the unique situation of the COVID pandemic, we tried to get the insurance companies to make exceptions since we could not perform knee replacements on these unfortunate patients. Unfortunately, none of the insurance companies made exceptions, and these were not covered. We found that during the first year of us utilizing these Knee Repair, NOT Knee Replacement TM programs, 91% of patients reached their goals. We like to treat patient’s goals, and not necessarily their “diagnosis“. Every patient has unique goals as you know. We have, and continue to be amazed by the power of our own body, “with a little assistance from us”, to calm, soothe, and heal itself. It shouldn’t really be a surprise, however, because if our body did not heal ourselves, we would fall apart in no time. So, why not utilize the power of the own body to help restart the healing process and heal itself? This is a very exciting time and we are just on the tip of the spear so to speak with regenerative medicine. We found that even using the à la carte individual treatments described above, by themselves, they lasted a lot longer than just two weeks. So, if you add up what you were spending in time and money just to get a couple of weeks relief, it starts to become abundantly clear that yes, patients have to pay thousands of dollars sometimes, but there are much better programs out there for longer lasting relief. Many people spend thousands of dollars on their pets, but refused to invest any of their money in their own health. It is not just money either, there is no magic or magic shot, there has to be an investment of time and mental fortitude. We like to say we only choose patients who are “ALL IN”. In other words, not every person qualifies for our Knee Repair, NOT Knee Replacement TM programs. There are many other possible components to our Knee Repair, NOT Knee Replacement TM programs as they have evolved since 2020. We also now track our results in a national databank registry called Databiologics and have found that tracking our results scientifically and comparing to the rest of the country, these treatments definitely work in general, and, we feel, because we utilize programs and not an à la carte philosophy, that it is not surprising that our results have consistently been better than the national average utilizing orthobiologics. We will do a post on that soon. Thank you, again, for your comment and we wish you luck in the future! PS: on a separate note, it is our understanding that injections of peptides are not FDA approved, and since there is no FDA indication for these, technically, those cannot be performed in United States. That does not mean that we feel that peptides can and do have potential positive implications, it is just that we are not allowed to perform them as injections in the United States at this time. oral peptides are considered “supplements“, but that is also something that needs more clarification.

    finaly i got something which is making sense . do you kindly tell what grade it is Mild joint effusion seen. Anterior cruciate ligament is swollen showing T2w hyperintense signals suggesting its intrasubstance tear at tibial attachment, however, attachments are intact. Posterior cruciate ligament appears normal. Medial and lateral menisci are intact. Both collateral ligaments are intact. Patellar slant and bilateral patellar retinacula appear unremarkable. No bone bruise noted.

      @@VictorySportsMed ok thanks a lot . intial i have loose body issue in my knee which is moveable and it is been easily seen in x ray but now last week i just try to straighten my leg then i feel like something stuck in my joint which is obviously loose body then i have a svere pain at that time ,and from that time my knee is been lock and i have a slight 10 to 30 degree movement only . or then i did my MRI and now there is no loose body is been shown and i am now unable to straighten or bend my leg . i only have 90 degree bending . so, what is happening right now

      It depends on how partially torn. Grade 3, no. Grade 2, probably not, but every case is different and it depends on. Grade 1 = best possibility of getting back to 100% or close, but again, in medicine every case is unique, so almost nothing is absolute. Thx!

    MRI result revealed that i got cronic ACL sprain.. i dont know how to grade by the statement of cronic ACL, but its been 2 years since, and still not getting better. Im really hopeless 😢

    Thanks for a great video. In my MRI report it is stated that, "Anterior cruciate ligament shows increased intrasubstance signals, suggesting low grade tear. Femoral and tibial attachments of ACL are intact. Grade 2 tear is seen in the posterior horn of the medial meniscus not reaching up to articular surface. Anterior horn of the medial meniscus is normal. Minimal effusion is noted in the knee joint." Everything else is stated as normal. What grade this would be?

      You are welcome and thank you for your question. It depends on your physical exam and the amount of laxity (if any) on Lachman and pivot shift testing. From what it sounds like, grade 1 or 2, possibly “just” grade 1 which is rare, but the best prognosis. What did your ortho say?       Thanks for the reply, doc. After seeing MRI and testing clinically, my ortho did say that surgery is not required for recovery. Just rest(no squatting and not taking stairs etc) for some weeks followed by exercise for range of motion and strength will do the job.

      @@williamdgmartin the periosteum is the covering on the bone and is very highly innervated with pain nerve fibers. It can definitely be anesthetized. Less so deep in the bone, but there are some tricks for helping with that too.

    Also I’m in my late 50s three months ago on a skiing accident. I tore bilateral ACL’s and MCL 😢 would that Laser help me in healing my MCL. I’m choosing not to do any reconstructive ACL surgery. I’m very active and devastated that I can’t exercise the way I used to.

    Best ACL video I’ve ever seen. What grade is this? “There is partial avulsion of the ACL fibres from its tibial attachment. Rest of the ligament appears sprained. There are contusions to the lateral femoral condyle and posterior aspect of the tibial condyle. Small contusion to the posterior aspect of the medial tibial condyle are also seen. There is a small contusion of the posterior horn of medial meniscus. No meniscus tear. Lateral meniscus is intact. Collateral ligaments and structures of the posterolateral corner appear normal. Extensor mechanism and patellofemoral ligaments appear intact. No significant patellar al tracking or chondromalacia. There is small effusion. No popliteal cyst. No significant bursitis.”

      Thank you very much! With the caveat that not all MRIs are 100% accurate, but are usually fairly accurate, if what is stated in the report is true arthroscopically, then that sounds like a grade 2 sprain of the ACL.

    Misha shock absorber is what I’m looking into in 2 days with Dr Frush in Novi Michigan. I do PRP/Hyrolonic Acid hybrid injections every 6 months and it’s saved my life. But I’m looking into the MISHA system as well

      I have done Ozone therapy with stem cell and PRP on one of my hips and the Ozone works more as a pain relief for a couple days to a week or so than tends to wear off. But it’s pretty cool.

    Apologies I could not figure out how to edit my comment. I meant to ask, can walking every day grow cartilage if one is bone on bone? Thank you. I am bone on bone and I really don't want to get a tkr and am not in a lot of pain but I do get flare ups every now and then. My MRI says I'm a grade 3 & 4 in different compartments. I'm just afraid I may be trading one thing for another and I've read that 20% of people are not happy with their outcomes. On the other hand I don't want to wait too long either if I really need it. Stressed...

    I went to the orthopedic because my knees started locking and had a mri done. The mri showed osteoarthritis, loss of cartilage. I am trying physical therapy but feels like when i do physical therapy my knee locking happens more often vs when i don't do anything. I'm scared of doing physical therapy, I don't want to get worse. I have no insurance and i need to work or I'll be homeless :( please, anything you suggest?

      Walking in and of itself probably does not grow cartilage. Don’t know of any studies that show that. Having said that, MOVEMENT is very good for the health of the cartilage we do have. Joints were made for moving, so, in general, movement is good for joint and cartilage health and maintenance. We have the motto: “Get moving…Keep moving” at Victory In Motion for that reason.       It depends on why and how bad, etc. the patient is anemic (low red blood cells/low hemoglobin/low hematocrit), but in general, low or dysfunctional platelets would be worse. So if not “too” anemic (and if not anemic due to blood cancer, etc…), with normal numbers of functional platelets PRP could still be done. What a lot of doctors are calling “PRP” actually is not a high enough concentration of platelets which, depending on the condition, needs to be anywhere between 3.5 billion all the way up to 10 billion. Are they using single spin, double spin? Are they using leukocyte rich vs leukocyte poor? How much blood are they drawing? Are they using ultrasound to inject precisely? What is the rehab like, are they doing total body fitness like we do? Are they adding other adjuncts such as MLS laser? Having said that, we have found that adding cells, i.e., bone marrow aspirate cells, does seem to improve our results. Bottom line: not all “PRP” is actually “PRP”.     What about collagen, bpc 157 or other supplements, can it help?

      Good question. We do not Rx peptides at this time, but BPC 157 is reported to aid in the recovery after exercise amongst other things. Interestingly, it is actually a banned substance for competitive/professional/Olympic/world class athletes, so that does lend to the fact that it sounds like it is truly a performance enhancer since it is banned. Again, we cannot comment on personal use because we do not prescribe it currently. It is also not FDA approved, so there are regulatory issues that also need to be considered. As far as collagen and other supplements, and some people would consider peptides supplements, that is a personal choice. Always read the package insert and/or consult with your pharmacist/physician/medical provider regarding any supplements you take. Since collagen is not a banned substance it is doubtful that it enhances performance, but some people do feel that taking collagen, even going way back to Knox Nutrijoint helps their joint pain and their flexibility. Nothing Earth shattering, nothing as profound as using cellular based therapy, but a personal choice. We hope that helps. Thank you!

    Subchondral fracture with surrounding Marrow Edema/Bone Contusion noted in the Anterolateral aspect of Lateral femoral condyle. Anterior Cruciate Ligament: Partial tear noted at the tibial attachment of its Posterolateral bundle. Medial Meniscus: Grade I signal in its Posterior horn. Medial Retinaculum: Grade I signal in its Posterior aspect - MPFL. Mild knee joint effusion with evident synovial thickening. Minimal fluid in the suprapatellar bursa. Mild myofascial edema is noted in the Posterior aspect of the lower thigh and Posteromedial aspect of the knee this text are highlited in my mri report, what i do in this case surgery is needed or not

    Hello Doctor Grade 2 strain T2A intensity increase and thickening were observed in the ACL. An increase in the amount of articular fluid extending into the suprapatellar bursa was observed. Instability in my knee I don't feel it. I never had the feeling of falling down while walking. The doctor said there was no need for surgery. He gave me an exercise program. I follow it. I play football as an amateur and the season will start in 4 months. What do you think I should do?

      Little by little we are getting there. There have been some papers that show some cartilage can be grown back. We will eventually get to the point where we can grow cartilage back, but even more importantly PREVENT cartilage loss in the first place. Thank you for your comment!
