    I am considered a tofi <thin on outside fat on inside> I heard lap band does also help people with low body mass and still reverses type 2 diabetes due to the lowering of ectopic fat, this is true?

      It's a great question about the potential application of bariatric surgery in folks who don't weigh so much, but who still have metabolic disease (you didn't say so, but implied with 'tofi'). Unfortunately I can't endorse Lap-Band, after having done several hundred of them in the 2001-2010 timeframe.     I am the best example of what the Doctor explained about insulin resistance type two diabetes. I am taking mounjaro with metformin my weight loss is very slow. I am looking forward for my bariatric surgery. My Dr is suggesting months ago to do a bariatric surgery but i am very hesitant. Not anymore i want to be healthy.

    I am so blessed to have him as my surgeon. I lost so many inches and went from a large to a small size in 7 weeks after my gastric sleeve surgery. I highly recommend Sage Bariatric Institute in Texas for weight loss surgery. Everything is going so well for me. My only issue is that I still feel very hungry 😢 but I can’t eat much, and I’m able to make healthy choices, which is the best part of weight loss surgery. My sugar cravings are completely gone now. I used to eat sugary foods all the time, but not anymore. I’ve lost 23 lbs in 7 weeks and want to lose 30 lbs more, then I will work to maintain it. I hope the hunger issue can be solved with the help of my family doctor and my surgeon over the long term. I feel so healthy, active, and I’m now able to sleep on my own without needing prescription sleeping pills, which I used when I was overweight. I haven’t had any issues at all. Some people experience pain after surgery, but my surgery was pain-free. I didn’t feel any pain right after surgery and was able to go home 5 to 6 hours later. My only regret is that I didn’t find this surgeon sooner and have the surgery earlier. I had been trying to lose weight for the last 13 years under the weight loss dietitian and doctors without any success. I was scared that this procedure wouldn’t help me lose weight, but I was wrong. Let’s see if I can lose the additional 30 lbs. If you have any questions, feel free to ask. This surgery is also affordable if you want to pay out of pocket like I did because I didn’t want to waste many months dealing with insurance and waiting for approval.

    Ive never had bypass surgery and still deal with this. I noticed after dealing with anxiety disorder and agoraphobia. My sugar drops after a meal on average once a week. I eat constantly snack on peanuts and sugar protein bars all day long amd this seems to help. DR Pilcher, what would you suggest? Ive tried a dietician, ive tried doctors and nobody knows anything.

    I get instant reactions (in about a minutes or even seconds) such as pain in my muscles and joints, twitching in my muscles, tingling under my feet and some kind of fuzziness whenever I eat anything carbohydrates or sweet or sugary, including banana and bread, which are the worse. I also get very weak and drowisy that sometimes knocks me off for about an hour. Recently, salt gives me same reactions as well. No Dr has been able to diagnose the problem yet. My blood sugars are high, although I am not diabetic but my sugar levels are almost prediabetic. These symptoms started at a time I was diagnosed as prediatic but my sugar have reduced now, although the symptoms worsen. I am desperately looking for help and warmly welcome any information in this regard.

      Sounds like you are not a bariatric patient, but either way I suggest connecting with your primary doctor to get a Continuous Glucose Monitor. This can be amazingly informative.

    Really good info! RNY in 2002. Sliced open! Twice because I had a hernia. But I’d do it all again. After 22 years, I’ve kept off 100lbs. I’m still 180 lbs at 5’2” but without the surgery I would be dead. It put my diabetes in remission until 2 years ago. I now take Synjardy.

    I'm now 18 months post surgery. Out of all the information available on the internet (Google, youtube, Facebook) I found your videos the most valuable for practical, sensible and reassuring explanations and information. I had the surgery done in another country ( where I currently live), and English is not spoken by medical teams generally. So pre and post surgery support was difficult/barely available, and I needed to self educate. Thank you for the time and energy you have put into producing these highly informative videos.i found them educational pre surgery, and reassuring as I went through different stages of recovery post surgery.     I hope this can help someone watching had the gastric sleeve in 2018 by the summer of 2019 I was down to 130 pounds. It was amazing. I was doing great. When Covid hit I started day drinking and I gained back all of the weight in a matter of a year and a half. Please please please do not drink alcohol. I did not eat my way back to gaining weight. I drank my way.

      @@DoctorJP thank you so much. I will check it out. I love your videos. They’re all super helpful and amazing. I have since stopped drinking alcohol completely. Next month I will be having the conversion to the Bypass 👍🙏 I strongly recommend Doctors to really educate patients on how critical of a role alcohol can play. In 2018 that education was not passed along to me. However I am grateful where I am today and I take my sobriety very seriously. After all alcohol in any volume is simply poison to the body.

    I did the sleeve in January 2019, went from to 270 to 210 I was happy, I stated developing a hernia in the incision site, 5 years later. Got an endoscopy done with same surgeon, he said he can remove the hernia, and re sleeve. He just called today and said your blue shield will not cover the re sleeve, but it will cover the hernia removal. The gastric bypass will be covered but the sleeve is no I don’t know what to do He recommended going from a sleeve to gastric bypass. Since the weight loss, and the weight gain I have developed sleep apnea and a I am back at 279 lbs with a stretched stomach. I don’t know what to do. I always thought bypass has a high risk of complications.

    Good video. It’s been 2 weeks and my goal has been walking 10,000 steps. Came into my endoscopic gastroplasty with fundus ablation (new add on option for gherelin) with a good amount of muscle under the fat from resistance training and as my energy has returned want to do that as well. If not to build muscle, then at least to preserve for now.

      That's a great question, and information is still developing. I might have mentioned it in the video, but people who suffer from the obesity disease, have an abnormal or imbalanced gut Microbiome. Both the sleeve and gastric bypass seem to help bring that towards a healthy balance.

    Thank you for this awesome video doctor. I had Acanthosis all around my neck since I was 9 years old (I am now 50. I was diagnosed with diabetes at 28 years old). The amount of teasing and hurtful looks and things ppl said to me for my whole life hurt my self confidence tremendously. I got SVG Bariatric Surgery 18 months ago and as soon as the 3rd month Post Op, I saw the beginning of the darkness start to go away. Now, IT IS ALL GONE. It’s so crazy! The amount of Products that not only my mother but also that I Used, Never Helped. Not even a lil bit. So seeing This Video has helped me to understand so much. Again, Thank you.

    I wish I had found this video before going with the sleeve, because of the acid reflux. I don't regret the decision to have surgery, just wish I had gone with the bypass instead. I'm losing a lot of weight but probably because I'm rarely hungry anymore and sometimes I'm just so afraid of the horrible chest pains that I avoid eating most of the day.

      Sorry to hear about the trouble. Please be sure you stay connected with your bariatric team because usually that reflux can be treated with meds.

      @@DoctorJP Thank you for responding. I take Pepcid twice a day. Drinking baking soda water works faster and last longer but my doctor doesn't want me to continue to do that anymore. I was desperate for the pain to go away.

    I have diabetes, sleep apnea, fatty liver, hormonal I balance, and now I have started with high BP! But I also have a hiatus hernia and I hear a lot of people have ir worse after surgery! Even if the get hernia repaired! I already have a lot of issues because of it so that is what's stopping from getting surgery!

      I understand how the presence of the hiatal hernia might make bariatric surgery seem more complicated. Nevertheless, I think it will be useful for you to meet with a bariatric surgeon in your area, and to ask that doctor about the hiatal hernia, and what impact would it have.

    On my list of food they have sugar free jello as a liquid,is it ok?

      Registered Dietitians agree that Jell-O counts as a clear liquid.
