Go for it! The "physician-scientist" career can be particularly challenging, but full of promises.
First, I disagree with Dr. Griffin's argument at 46:26 that case control is inadequate to study the benefits of a PCI intervention. The main issue is that you cannot use a 1:1 matching because you will never match on principle factors. Statisticians and epidemiologists have argued for at least a 1:4 ratio of cases to controls, which allows for maximum deployment of potentially confounding information across 4 rather than 1 control. The real issue is the number of patients that are needed for such as design. Second, I wonder why some of the newer technologies for crossing the CTO were not mentioned by the fellows? These devices, such as the one offered by Boston Scientific, allows for the bridging coronary CTOs by either the lumen or subintimal paths. Overall, really great presentations by all and thanks to Yale for posting for those of us in practice.

@ 27:00 my galaxy watch once in a while shows a drop on heart rate while sleeping and this is the risk of SDS while you sleep. Still doctors keep scratching their heads, but now people have smartwatches that can track your heart and oxygen while you sleep, literally keeping an eye on your heart. They should aggregate this type of data, as now these devices are more common.

I'm 65 with 100 blockage in Rt Carotid and 70 to 90% in left. I was Diagnosed with asyntomatic carotid stenosis Oct. 2020. This was caused by radiation I received for throat cancer 10 years ago. I eliminated most sugar and went on a Keto-ish diet, I am now in the gym 2 hours a day, 1hr Am 1 hr in the afternoon and I walk 5 miles on average daily. I walk a mile in 16 minutes. Still asymtomatic.
