I have. 94 Jimmy, just replaced the heater core and have been having that issue. I just needed to replace the clip. This video is awesome, direct to the point,.

working on my 1992 right now, has the same issue,. trying to figure out where the vacuum comes thru the firewall to what? thanks for the video.
