Yes concrete was damp but the water from the bottom of the hole came up as the concrete got higher and yes you should have sent it back especially if that so out e is for an overhead traffic signal
As a mixer operator I can tell you it’s 100% the drivers fault if it makes it to your job site! If the plant did mess up it’s the driver’s responsibility to let them know so they can fix it before he leaves but as a driver is supposed to check his load for correct slump and to rinse off his truck before leaving the plant, he either didn’t check his load or put too much water in himself. Now there are some managers however that don’t want to admit they wet up a load and will send us on our way regardless if we did inform them in which case it is the plants fault. Idk if that’s what happened here or not but I guess it’s possible

If it were anything structural or had to hold weight I would send it back and dig any poured out.
