  • Dr. Eric Westman is an Associate Professor of Medicine at Duke University. Board Certified in Obesity Medicine and Internal Medicine, Dr. Westman founded the Duke Lifestyle Medicine Clinic in 2006 after 8 years of clinical research on low carbohydrate & ketogenic diets.
    Dr. Westman combines clinical research and clinical care to deliver lifestyle treatments for obesity, diabetes and tobacco dependence.     I’ve been reversing my ALS Bulbar Onset by eliminating all processed foods, vegetable oils, sugary fruits, all grains, only consuming nutrient-dense foods like beef liver, cod liver, grass fed grass finished beef, cheese that’s aged at least 1 year, eliminating all high oxalate foods and lectins, and doing nutritional ketogenic diet, intermittent fasting ( fasting 24 hrs, one meal / day for 5 of every 7 days ) and have done this for 1 1/2 years.
    Results: I regained my ability to walk, talk again!! I no longer choke on food / drinks, and can chew my own food as well as have the strength to cut up my food, again!!
    I’m told I’ll be the 56th person on the planet to have medical documentation of reversing ALS ( Lou Gehrig’s Disease )

      Good for you! Congratulations. Just bare in mind that some of these messengers may have a good message but aren’t credible. Please don’t put Dr Lustig (from UCSF) in the same line as Dr Berg (a chiropractic who is also a Scientologist) or Thomas Delauer (who is basically a RUclipsr with no credentials as a doctor or researcher).
      I want to add some great real researchers and real doctors: Bart Kay, Dr Volek, Steven Phinney, Dr Robert Cywes, Dr Chaffee. Same data, better credentials.

    Reversed my type II diabetes through the ketogenic diet combined with intermittent fasting. No more medications for diabetes for over eighteen months. This is a lifestyle for me that works. More energy, more mental clarity, better mental outlook, dropped about fourth-five pounds, reduced neuropathy in legs, feet and hands…the list goes on.

    This lecture supports closely what I learned about the ketogenic diet.
    Read your labels. Keto friendly may be friendly but usually not keto.

    Round of applause when Dr. Westman announces end of his introductory mask talk!
    My unsolicited advice on Low-carb: Simply cease eating the three things that we presently eat vastly more of than we did historically (pre-agricultural revolution), than we're evolved for: SUGARS, GRAINS (grain is sugar), SEED OILS. And since these happen to be the 3 prime ingredients of processed/fake foods, you'll be eating only real food.

      👍 You are absolutely right. Unfortunately, two-and-a-half generations elapse before the medical community changes its guideline recommendations to reflect the results of current studies.
      Doctors have been misinforming us for decades, and it has been very profitable for their business model.

      Seed oils are the most evil, but only refined ones. You can safely consume hemp and flax oil, freshly pressed. But you will need to do it yourself. Flax oil shelf life is about 3 days, so what people can buy in shops is pure poison. Hemp is longer, about 1 month.

    Dr Eric Westman is my savour. I was doing IF & LCHF. I reverse all my metabolic illness & all my health makers improve except cholesterol level. After listening & understood his explanation, I get to the right reading on the cholesterol of my lipid panel. I am so glad. Now, I on a mission to help others.

    One of the main things that got me through this low carb process was unlimited fat and protein. This process was about retraining my body. The key for me was to not be hungry, and the best way to not be hungry was eating fat. Over months by body got used not having carbs around and relying on fat and protein, and I naturally started eating less fat. All that said, in the back of my mind was the understanding that eventually I would want to eat less and use more body fat, that was the ultimate goal.

    I kept a detailed food diary for 7 months. I didn't/don't count anything. I'm glad that I did that because I have found that even though I am no longer doing it I am still very aware of what I am eating during the day. I think it helped me to make creating meals with my "new" foods become second nature.

    For the mask idea you're correct about it being simple but wrong about one basic principle. Spittle does not infect people, Aerosolized particles infect people and they are far too small to see with the eye; even with lasers. The average aerosolized particle is 4-12 microns. The smallest particles I could find studies on was the efficacy of masks on particles of 300 microns and at 300 microns even N-95 masks only had a 3% efficacy.
    So simply put, no masks won't and did not slow infection and probably put people in a false sense of security.

      You are correct. Viruses are so small, they pass right through all the fabrics. And I agree, it did create a false sense of security. It is odd that Dr. Westman did not research this issue further as many other reputable doctors, with knowledge of microbiology, did. Masks actually are harmful, especially in lowering oxygen intake, causing a multitude of other problems, including facial rashes, dental problems, and worst of all, cancer cells to proliferate. Imagine how difficult it must have been for people with asthma!

      It's easy to disprove these basic cheap masks. Wear one outside when it's below freezing. Can you still see frozen vapor when you exhale? Yes. If the moisture vapor can escape so can Covid. If someone is facing me and talking to me the cheap masks will prevent projectile saliva from getting onto me which I can appreciate but it is still in the air. I've seen many people advocating masks while continually pulling / adjusting their mask and many wearing it incorrectly under their nose as well as individuals in person and on TV pulling their mask down to cough. Maybe there is some benefit in limited circumstances (projectile spit) but I also believe you're correct about the false sense of security. It reminds me of "Duck and Cover" in the 1960's when kids would get under a desk as part of nuclear bomb drills. It was meant a calming sense of security so you felt like those in charge could keep you safe. It was better than saying "There's nothing we can do."

      Exactly! Thank you. The money they spent on that study versus using common sense in ‘putting something over your mouth to stop spittle’ is moronic. Common sense tells you that, in fact any sense at all tells you that. Some people are ‘educated’ but don’t have common sense. I’ve come to believe that the less common sense you have the more you follow the narrative and the crowd.

    I enjoyed your talk. Ive been on keto since January 2021. I’ve struggled with weight since childhood. I no longer take blood pressure medication and also arthritis knee pain is minimal. My cholesterols went from 220 to 168. I’ve lost 35 pounds and feel better than ever! This is the only way of eating that has allowed me to keep it off. I have encouraged many to eat the SIMPLE KETO DIET.

    Eric is one of my favourites on Keto but his mask story is emblematic of the thought an expert in one domain is not necessarily an expert anywhere else. There is no doubt a mask stops spit etc but if you’ve ever blown smoke through or on the side of a mask you’ll know that stopping something as small as a virus is a folly. Prof John Lee said it simply “It’s like trying to stop a flea with a tennis racket”. Kudos to Eric on Keto, he’s a star.

      We always wore our masks pretty strictly at work. As soon as one of us got cvd, we all got it within the next 3 weeks. Masks don't work.

    Yes, the diet does some unbelievably healthful things that one might think that you're selling a snake oil diet. I've been in ketosis for over four years and I no longer am obese or prediabetic among other things, but I hardly ever announce that I no longer have motion sickness, which I suffered with all of my 62 years.

      For 20 years I suffered from vertigo, high BP, GERD, joint pains and asthma. After over 2 years of Keto - all those conditions are gone and I am 68.

    Thanks for this talk - it’s very reassuring to hear how simple this way of eating can be. When I explain about my diet they say it must be dangerous because it’s so restrictive. My answer is that humans thrived for many thousands of years before farming, processing, packaging and marketing was invented

    I've been mainly ketovore for the last 2 1/2 years, had the odd slip up and the last was when my mum had been admitted to a psychiatric hospital in February(just a few weeks after her second depop shot she went rapidly downhill mentally...), my diet went to hell bread, biscuits, chocolate etc, and was drinking alcohol WAY too much as a (crap) stress coping mechanism. I quit all that in mid March and checked my bloodwork to see what damage I'd maybe done - TC-314, HDL- 88, TGL-70, LDL-212, blood pressure perfect as was fasted glucose.....weird.
    So, after 13 weeks of strict ketovore and no drink, cycling 3-4 times a week (up to 50 miles a time), just done my bloodwork (at home, Mission 3 in 1 equipment) which is now showing TC-442, HDL-66, TGL- 70. LDL-362..... I'm a bit perplexed as to why the last lot of bloods was showing arguably better.... I'm not concerned about the LDL though, I tend to view with high suspicion anything the government and "health" authorities tell me is good and know the role LDL plays in fighting off disease (and that studies have shown that those with higher LDL generally live longer lol)....I'm wondering if during this "bad episode" my lack of eating eggs has something to do with the TC/LDL readings, despite assurances that dietary cholesterol does not affect blood levels. I'm toying with the idea of strict carnivore again but maybe just lard/meat/fish and maybe ditch the eggs and redo my bloods each week to see what happens...this way I can easily monitor my daily food intake and make a strict 25/75 protein:fat and just do away with all carbs....?

    I can't have many sweeteners, as they kick me out of keto. The only one I found that doesn't effect me at all is erythritol... and its been wonderful discovering it! Easiest weight loss I've had since accidently discovering fasting!

    It's one thing to want to do it every day and believe in the keto diet but if you have to do it; if your life depends on it, you should do it.
    The really hard part is getting off sugar and if you can do that, you can anything.

    Dear Dr. W first of all Thanks and I truly love you to make my 25 years T2D go away. I know my number will go up if start eating again. I accepted the fact. But I already passed the age that my dad died with imaginative awful way. I am 57 now and i always did what I told and it didn’t work. Now low carb work for me. I have CGM and log all my data. It might be late for me although I still have functional body except my left eye with complications. Thanks again I believe it will works for many people and at least it do not do any harm.

    From 300 mg/dl 11.7 hga1c) to 98mg/dl in a few years on a keto/ carnivore diet. Off all meds, and doctor is baffled I won't consider a statin drugs for raised LDL, with a 1:1 HDL: Triglycerides ratio.
    Diabetes in remission, weight loss ( 300 to 195#) , hip pain gone, frozen shoulder gone, volentary activity level increasing regularly, hieghtened energy , and no colds since started,.....all I say is Kudos to Keto!

    I can say that the act of tracking things can be useful in the gamification way. The most useful thing I have tracked is my blood sugars. If I am testing 2x times a day, I know if I cheat, or overeat, my glucose numbers will go up. I am much less mindful of how I am eating if I am not testing.

    Also reversed my T2 diabetes (A1C of 7 to 5.4), arthritis in both knees gone after only 3 weeks, tons of energy, improved mental clarity and ADD symptoms dramatically reduced, snoring and sleep apnea gone, and so much more. I thought I would miss and crave sweets and sugar, bread but I am loving eating delicious meat, eggs and seafood with some veg, cheese, nuts and berries. Keto has literally saved my life.

    This was kind of useful but a little irritating In how this talk was a constant ‘waffle’. Just not stop talk all over the place.
    Pausing in silence when you speak is a powerful tool to use when speaking.
    Also structuring your talk is useful.

    This talk really excited me. You began with the mask issue. Conclusion was that masks work. Masks catch stuff coming out of your mouth.
    But this is so simple. It's like saying that the filter on a cigarette works, so this keeps the bad things from your lungs.
    I remember the cigarettes without filters. Filters worked! Look at the tar and smoke color in the used cigarette. The filters actually DO work. Sadly we as a public were advertised and driven to believe that the filter took the bad stuff away.
    So, what is missing from your mask talk, is data or even the idea about how well a mask limits the transfer , one way or the other, of a virus. What is good to minimize dust may not be so effective at protecting one from a virus.
    I started my Keto way of eating before I ever heard the word. Years of figuring out certain foods made me ill, brought me to the practice of zero added sugar or sugary vegetables. I already figured out the fruit juices were just wrong.
    As I lost weight and gained energy and ability, I heard about Keto and Carnivore eating styles, and my education began.
    Thanks for your presence on the internet and you tube.

      Masks may work to catch spittle, but aerosol does not get caught. Viruses are way smaller than the mask catches.

      The virus is smaller than the weaving of cloth masks (which most people use). So using those is completely useless. I've read studies as an Registered Nurse. I can send you links about that if you care to read them.
      By way, my dad smoked cigarettes with filters from 20 to about 45, than he died of lung cancer. So filters in cigarettes don't stop bad things from getting into your body.

    I get the masks stop droplets getting in and out to some degree but where does the air go when you breathe in and out ?

      Surely the droplet's are far larger. Air molecules pass through a mask very easy and the large droplet's are caught.

      Exactly, air molecules are very tinny and separate from each other. Water molecules are attracted to each other anf form droplets. Droplets are much larger particles that get caught in the material of masks. The better the mask the smaller the water droplets it will catch. Masks are not too dense to prevent gasses like air and CO2 to pass through otherwise surgeons and nurses in operating theatres would pass out!

      A typical example of an experiment that doesn’t translate to reality. The only way to test a mask is have an infected person exhale through a mask and recover virus. Virus laden droplets evaporate fairly quickly in a dry environment and the virus travels through the air, and right through even an N95 mask. People! Only biohazard level protection is 100%. Funny little experiments by bored people in the basement are fine, but they aren’t virologist & lastly statistics don’t lie - states with no mask mandates were worse off than ones with draconian mandates.

    You can jump around the issue OR you can try consuming only fatty meats for a month and nothing else .When great improvement in probably all conditions is experienced , no-one will need to convince the patient of natural diet efficacy at creating natural well being .

    I still feel hungry while on keto. Only when I truly cut out carbs by eating only meat and cutting out seed oils did I stop feeing hungry while in ketosis

    Indeed Keto is supremely uncomplicated. Zero carb is even less complicated, as there is no such thing as a "limited food". It either has carbohydrates (total, not net) or not.
    As for the expense, I happen to track all of my expenses every month and had a few years' worth of data before starting keto. In my case, the increase in expense is negligible. Nearly identical (except for recent inflation effects), within 10 euros monthly margin of error. Then again, I didn't eat many processed foods even before keto. Those who do may actually see their expenses decrease if they stick to a proper whole foods keto diet.
    These are the two least complicated and most beneficial diets I have ever followed.

      Situation may be different in different locations. I track by spending, too. While prices have gone up a lot recently due to inflation, at least where I am, the overall food budget remained at a similar level for me. I suspect it has to do with the lack of fancy drinks (I was on sugar free stuff and drink water now), snacks and carbohydrate including foods (rice, bread, pasta). In addition, the subsequent weight loss also reduces the total amount of calories required by the body, so it may turn our to be a net zero from a financial perspective despite the percieved increase per item or meal.

      I ate quite a lot of junk food during Covid (well, by my standards) and after cleaning up my diet again, I'm spending way less. It just comes down to food volume being so reduced, in my case. Some ground beef and a few eggs, with the occasional vegetable, doesn't really cost much at all compared to ice cream, chips/crisps, or bread and everything you put on it.

    I'm surprised at the need to be careful of calorie consumption under keto. For me all I have to do is to keep my carbohydrate intake to less than 20g a day and eat as much of anything else as I like until satiated. Works for me.

      That works for some people. Additionally, many who are patient accept overeating the fats at first to ease the transition; the satiety signals usually improve with time. And, many are not patient, often have a difficult time, and even abandon their journey in some cases.

    Great talk. I got sick 4 months ago and decided to heal myself on my own. Through a elimination diet I automatically moved towards ketosis. Once I learned I was suffering from inflammation I dove into RUclips. Today Im at 180 lbs. I lost 20 - 30lbs. I now find myself eating 1.5 to 2 times a day and eating as an example baked chicken with broccoli, califlower, carrots, tomatoes, beets and my own homemade chocolate made with real butter, cream cheese and 100% dark chocolate bars sweetened with Splenda. I feel awesome and can make a long list of improvements in my life. Do I just keep doing what I'm doing? Will I lose too much weight? Do I just listen to my body?

    Honestly ppl just need to get out of their heads trying to be fancy with Keto. Just eat whole food, meat, veg and dairy (IF it agrees with you). Period. That's it. We usually get ourselves into trouble when we start eating from a box cause now we have labels. If you can't grow it, hunt it, or make it on a farm without industrial help, don't eat it.

    The patch for quitting smoke say in the adds that they are 50% more effective than not using it so that is really not very effective. What percentage of people are really able to kick it?

    It just occurred to me to look up if my psychiatric meds affect blood sugar / insulin and it looks like Vyvanse (ADHD) might increase blood sugar. maybe why i get headaches after lunch on some days (try to be low carb but struggle with consistency)

    Can someone explain by the concept of net carbs is counterproductive? Is it because we should actually be targeting a lower amount of digestible carbs? Or is it because microbiome actually gives us energy from fiber? Or what?

      Yes. Much fiber is metabolically active. I couldn’t get into ketosis till I stopped my fiber supplements. Dr W has a video on this.

      You can't know how much is digestible it varies from leaf to leaf. But I would not go insane worrying over small amounts of carbs.

      Introduce her to some of the keto/carnivore channels on RUclips. People like Ken Berry are perfect for beginners and people who are curious about this. Short videos at the beginning help to not be overwhelmed with all the new information.

    I love Keto and i know the power of Keto. But what i dont understand is, that "to much protein" sounds not really like a point for Dr. Westman.
    But if i eat to much protein (also its from whole foods) kicks me out of ketosis. An i know from others, happans the same.
    But Dr. Westman dont say "you can overconsume protein". Why?

    I know that too much sugar (glucose) causes problems. Isn’t excess linoleic acid (Omega-6 polyunsaturated fats from seed oils) suspected to be 10 times worse because of the detrimental effect on the mitochondria’s ability to convert glucose to ATP (energy)?

      Not everyone’s numbers respond the same way. Maybe it’s some dietary influence that hasn’t been identified. Or some people may have more toxins in their body.

    15:58 the calculation is not quite correct. glucose is mostly measured in plasma, not whole blood. Glucose concentration in whole blood (the five litres) is probably 10% lower, so more like 4.5g. Some say even less. It's all on average anyway, so doesn't really matter.

    Probably one of the best talks on Keto and low-carb diets i've heard in years, including clearing up the net carb issue. I've personally read about Low -Carb diets for years, but currently, i've seen a ton of misinformation posted on Facebook etc. Especially from the so-called Carnivore followers, who sadly think eating no carbs whatsoever gives them license to swig butter, and somehow fat is king over protein. The problem is so few people actually read books or look up their information, they just want someone to meet their confirmation bias

    I am a keto advocate, have been on keto for 10 years, I have during that time eaten no carb, no sugar, no alcohol and yet my Calcium Artery Scan 8 years ago gave me an Agatson of 24 and now in 2022 it is 96. This is quite a surprise and not what I expected, a four fold increase. I would be very grateful if anyone could advice me why this could be so ?

      What exactly have you been eating? I'm surprised (and worried) as this is the opposite to what many have experienced. I started the ketovore diet recently and I'm feeling fantastic. I have so many health problems plus sarcopenia and this is the only dietary approach capable of lowering my inflammation.

      You probably don't do seed oils so increase your K2 Mk4 intake from food to remove the calcium from where its not wanted.

    Dr. Westman is a great guy and is one of the current original supporters of low carb but he needs to get over himself, you can certainly improve what we know we don't need to hold on to the past. You won't be forgotten Dr. Westman but just because you want to be the man doesn't mean we shouldn't improve and continue to support a low carb approach to good health. Dr. Westman should embrace the good changes and enjoy the fact he was there early on.

    I got bombarded by many comments on “eat lots of fat to burn fat”. “If you wanna burn a lot of fat, you have to first eat a lot of fat”. Isn’t very low carb sufficient to induce fat burning?

      Maybe it's a relative matter? There's no need to gulp down a kg of butter daily but I can think of two reasons, many have heard to decades about how bad fat is so they may have to be reminded, especially if they have lean meats, beans or such. And then there is those who perhaps have a history of eating disorders, a percieved need to snack (which for the most seem to fade away pretty quick) or perhaps have more difficult to adjust to fatburning?

    You Doctors really don't want to let go of the calorie thing. Until we really put effort into understanding how your body uses the energy in our food it is mostly guessing. One thing for sure, when I eat a lot of fat is goes out in my poop. And after listening to a LOT of doctors lectures about constipation it is fairly clear, to cure it you need high fat in your excrement not damaging, inflaming fiber. I know this all by EXPERIENCE. O.k. get ready here comes the common scene part, if there is fat, sometime high fat in your feces's then all of it MUST NOT BE TURNED TO ENERGY! NO CALORIES! Instead of trying to hold on to the calories in calories out nonsense, why don't you try to find out when fat is used and when it discarded by the body. I lost 100 lbs in a year and I ate a LOT OF FAT! You are doing your patience a disservice if you take away the one cool thing about the Keto diet. You CAN eat a lot of fat.

    If you all would look at why God forbid foods such as pork you would see why we have such issues. Eat low carb foods with good fatty meats such as beef and see. Cut out pork and other forbidden foods and your bodies will thank you
    The pork is your underlying concern now that we figured out the low carb way to cut out grains and sugars the fat building cement being consumed to death.

    Eric, yes you experienced that masks block droplets. So what? That is mechanistic. Now the discussion should turn what happens next and what is the connection to all cause mortality. How about particle physics and weak interaction of different molecules and organisms. I see not a significant causation in the mask discussion.

      Yes, the droplets you let out when speaking drop quickly, the ones that stay in air are small enough to pass through a mask anyway. And, if you don't speak at all in public, how would masks help at all?
      edit: not to mention how much more evidence (though still not great) there was for high D3 intake being effective, but no official mentioned that

      Exactly, air molecules are very tinny and separate from each other. Water molecules are attracted to each other anf form droplets. Droplets are much larger particles that get caught in the material of masks. The better the mask the smaller the water droplets it will catch. Masks are not too dense to prevent gasses like air and CO2 to pass through otherwise surgeons and nurses in operating theatres would pass out!

      No. I do not. Our body needs a small amount of glucose and is capable of making all it needs. Eating "food" to obtain glucose potentially introduces lots of chemicals our body neither needs or wants.

      Your body only requires around 4 grams of glucose in your blood at
      Any given time. That amount is easy to get converted through that process and cleaner than dietary without causing insulin spikes. Ps you could say the same about keytones and essential fatty acids

    I would argue that (green) olives could be unlimited.
    Low-cal; most of which comes from (anti-inflammatory) olive oil.
    Low-carb: Two pounds of them ~30g of net carbs (1 pound ~30g total carbs).
    Satisfying snack: Especially garlic-and-jalapeño-stuffed olives.

      Bill. Allow me to wreck that idea. LOL Olives are still too high in Omega 6 PUFA. Better than seed-oils, but not by much. Some olive varieties - especially ripe, California black olives - may contain high amounts of acrylamide as a result of processing. All plants are made from sugar primarily. Olives are no different. Both carbs and fiber are made of sugar. The snack sounds like something I would have gobbled a couple of years ago.

      Agreed. Somehow it seems inconsistent with low carb eating to be in favor of masks. Not that masks are food but usually low carb folks question the establishment etc. So when the establishment says wear masks I question the reasons and "science" behind it.

      There are new studies that people who have metabolic issues are more susceptible to having greater health problems due to COVID (to include death). The reality is that the majority of Americans and American doctors don't think the human bodies hormonal response to processed foods are responsible for any serious health issues. So if he is dealing with those who are at greater risk... He is being responsible by wearing the mask. Otherwise he could very well be Dr. Killpatient...

      Yes, low-carb tends to attract anti-establishment folks. Among anti-establishment folks there is, however, unfortonately a larger than normal concentration of loonies of all sorts. Including idiots who don't believe that a layer of fabric stops flying droplets of spit. No, it's not healthy to wear masks all day, but FFS it's an effective way of making sure you don't infect someone with your shit.

    Why protein not counting and vegetables counting? You cant eat to many vegetables, for ex cabbage, but you can eat too much protein.....also stimulate insulin.... i dont understand, thats confused.... and when i want 1 Tbsp Almond Flour to my pancakes, thats definitely not too much....

    thats all right, but this is atkins diet, also good , not keto! protein not counting - there are a lot of people, they eat sooo much protein, they have health problems. I think you must not over eating protein. Too less fat can cause constipation and other problems.

    I don't think his study numbers seem as rosy as he makes them out to be. Year 1 went to 6.3 A1c which is great but year 2 back to 6.7% so creeping right back into diabetes and the body weight change -14% also of the good but then year 2 falling to -10% that says to me they had early success and are losing ground the more time goes by. based on that I wouldn't put my money on year 5 looking impressive at all.

    Keto labeling is just a marketing tool. This couldn’t be more simple. Don’t eat anything not on the list. (Page 4.) Don’t eat anything from a box. Don’t eat things wrapped in plastic. Just eat real food.
