battle academia ezreal has a different animation when casting his ultimate each time he levels it, getting more confidence with each level (which fits so well with the lore of the skin)
empyrean pyke also transforms while executing enemy champion with ult, the transformation lasts until his free ult cast runs off and it changes his model and VFX
Ashen Knight Pyke also has special sfx for takedowns with his ult, playing a slightly longer music with each takedown and a really long one on pentakill
I'm not sure if you'd count it as a "second form", but High Noon Leona grows a progressively more complete crown of hell's butterflies the more abilities she lands and takedowns she gets on enemy champs.
I’m pretty sure you missed part of Battle Academia Caitlyn. Her ult actually triggers a comic book style special attack animation in the corner of the screen. I noticed when a friend was streaming gameplay with the skin.
For Project Ashe it's not about which side on the map she is on. It's about the proximity to allied structures. She puts her hood on in the jungle as well

Precisions on the True Damage Ekko skin : the music changes after the use of an ability and not only his ult When you use Q, the music slows and is distorded because Timewinder smows time When you cast W the music slows and then stops if you enter the W area When pressing E and dashing the music goes distorted and speeds up greatly After each ability is used fully, the music goes back to normal And yes when you press ult you get what was shown in the video
Also as a Katarina main, i think its worth mentioning that every Chroma of Battle Queen Katarina Skin has matching Daggers :D so imagine how many different daggers katarina has :D
They really miss Katarina's Battle Queen skin as a legendary skin, If it changes clothes with the dagger, I think that would be an Ultimate skin perhaps.

Kda Seraphine has a lot of features too. Each of her forma have unique music that changes beats depending on the people around you. She also has different back stuff, completely different recall and VFX designs and sounds and VO.
winterblessed diana becomes purple and a gets a crown alongside new vfx when she ults 2 or more people, and the map becomes wintery for a bit if she gets a pentakill
