Shigaraki got his quirk when he was 5 and accidentally killed his family(minus his father he killed him on purpose)
But there's a theory (who got really got point) where it say that tomura was in fact born quirkless (because of the shoes) then AfO gave him a quirk (the decay one) so tenko would accidentally kill his family so he can take him.

If this is true then that means Inko and Kotaro are only half siblings and with the "abandonment" of Shigaraki leading him to AFO and since it doesn't really say whether or not if Hisashi adopted Shigaraki then saying he didn't, Deku and Shiggy are like 1/4 of related and on a general family tree, they would be separated creating 2 different blood lines therefore saying they are barely related so I wouldn't quite call them cousins.
Also it's not like Hisashi can legally adopt anyone anyways 💅🙄
