I’m here for understanding. Best video I’ve seen. Had total thyroidectomy on 9/19/23. Got pathology results showing I have BRAF mutation. I’ll be having RAi treatment in 2 wks. Hello would you mind sharing your current progress? How are you doing? Did you have to take the radiation iodine therapy? On Oct 15, 2024, my wife was notified of thyroid cancer via biopsy. On the Oct we went to discuss what was find and the way forward. My understanding is the cells were atypical but further genetic testing indicated markers for cancer. She is currently waiting for a surgery appointment with a specialist.
I know I'm late to the party but all your information about thyroid cancer and testing really reduced my level of anxiety way down. Thank you. I'm 70, lived wide open for the last 50 years and surprised I ain't been here before now.
Excellent presentation. Love the humanity and compassion shown towards patients during diagnosing, treating and the aftercare process. Also thanks for explaining the topic it in an easy to understand manner for us not medically trained in Endocrinology and Oncology 🙂👍
What a fantastic presentation. I learned so much - the doctors are so articulate, they present a variety of fascinating material in a very intelligible way, and also are very sensitive. I think this presentation was really top notch.
First, I want to thank you so much for this excellent video with detailed information. I have cysts and nodules on mine. It first showed up on a mri of my neck after a car accident and i started off having one. I went a year later and i had 5. Now I've had several boiopes done and they do a thyroid ultrasound and a biopsy every 6 months, they're suppose to anyways. I shouldn't fall into any percentage of having cancer at all because we have alot of thyroid problems in our family . Thanks for sharing the video and the first biopsy wasn't painful at all but the second one was quite uncomfortable because i had more unfortunately show up. This was so helpful and so well presented. Since I am just beginning this journey, I particularly appreciate the comments regarding the importance of the initial surgery and imaging.
I too am beginning this journey. Months ago I asked my young GP about taking a look at my thyroid and he asked me if I really wanted to go down that rabbit hole...made me feel stupid...now one of my lymph nodes is swollen and painful and I bullied him into an ultrasound. Lots of nodules that need aspiration. This was an excellent presentation!!!
It would have been helpful to address the existence of Haistmotos and being on Levothyroxine already and the newfound nodule. Also, the fact the Levothyroxine is a tumor suppressor.
I was diagnosed with a 6 cm toxic nodule on my thyroid this past December & got it removed May 1st. I talk all about my story, symptoms, the surgery, & my surgical team that I’d recommend to ANYONE in my “meeting my thyroid tumor” video on my channel! I also have my social media linked in case anyone with similar issues wanted to talk :)

The same hold true for that same pathologist to go to seminars for oncology and endocrinology. I know this is a UCLA presentation: however, remember what I said about cross over?! The same holds true for Universities or other medical centers that are trying to help combat this cancer fight! I like a word that I heard by Dr. Julia Sosa- Endocrine Surgeon at Duke University medical center, she is also writes ATA guidelines, and is the president. She said: The patient must be part of the epi-center, this meaning the initial surgery is the most critical, and the best cure for thyroid cancer, the patient must make decisions as well within the ATA guidelines. I'll have to have a Lobedectomy and highly likely a full thyroidectomy, which for me is not only scary, but very challenging due to some lymph node involvement, and the current pandemic. The pandemic is hard enough, recently found out I am allergic to substances found in the shot, I've had already a serve shock that lasted for 3 days, was told I was lucky to be alive, what do I do??? Especially, they are encouraging cancer patience to get the shot, please help anyone??? I'm lost?
I am suffering with a nodule on my thyroid, and some trouble swallowing I made an appointment and go in on dec.20th 2022,I am so worried bc thyroid issues run in the family my younger cousin had to have surgury for thyroid cancer in her early 30s late 20s ,I lost my father in may from cancer, all his sisters passed from cancer except one.my grandmother had her breast removed for cancer was in her breast,I'm really hoping i dont have this bc i feel so terrible and throwing up cant eat hardly ,no appetite, feeling faint alot...
my nodule grew a bit and its indifferential now or suspicious cells. the DNA/genetic test said i am high risk for aggressive thyroid cancer. I am to have surgery but is it really necessary or can this wait for a second opinion or even wait another month before surgery.
@@saulbecerra5830 Thanks for asking. I had the thyroidectomy in december 2018. My TG did not go undetected. My lymph nodes were never tested. So I did finally have central left neck dissection this past July 2021. Right now, 4 months later, my TG is 0.06. Yay! I’m not sure Ill do Radioactive Iodine again. Only if my TG begins to climb. It’s been a journey.
I got FNAC done 2 days back and report given is Follicular Neoplasm. The biopsy was done by locating the nodule with hand. Sonography was done separately a day before FNAC. Is it necessary to draw sample while detecting with sonography probe in parallel? If so, should I try for FNAC again at a different Lab to get a conclusive result?
Follicular neoplasm, from what I have read, is one portion of Category III or "Indeterminate" of the 6-tier Bethesda system of classification. According to journal articles I have found online, it carries a 15-30% risk of malignancy. If your cytology report says Follicular Neoplasm, you may want to have surgery to remove the lobe, or you may want to have a repeat biopsy, since about 80% of the repeats will give you a more definite answer, whether benign or malignant. If the nodule is too small to be sure they are placing the needle in the correct location, they will use ultrasound guidance. So no, it is not always necessary to use sonography for the biopsy.
This was a very informative presentation on thyroid cancers. My mother has recently found a lump on her neck close to 2cm and currently being scheduled for ultrasound biopsy over at the LAC USC medical. I want to find a good institution for her so that she can be treated correctly from the start and promptly before the condition gets any worse.
I told m doctor something was wrong for months she ignored me until I switched doctor's and he had them do an ultrasound sound. In have a very suspicious lesion tumor. In just want this out.
Had a 3mm thyroid nodule that disappeared after a year of daily supplementation with lugols iodine, a couple of drops in filtered water each morning. I would certainly try a holistic approach with good nutrition and supplements before putting my fate in the hands of medical doctors, no matter how well meaning they are. Unfortunately, holistic medicine is frowned upon by the AMA because it hurts profits.
J HS I’m really not sure if nodule was solid. But I have read of other people either shrinking or eliminating nodules with iodine supplementation. Most people in U. S. do not get adequate iodine in their diet.
My Biobsy came up negative for Cancer, yet, I am being advised to get the oiter removed along with the left side of my thyroid gland...Is there any other way of healing and reducin this goiter. My T3 and T4 s are all good --harmone levels are normal???? Is there something more I should know????
I had over 4inch tumor, i did Gerson Theraphy, and down to 1.5 in 3 month time! This theraphy require 2 yrs continuous to eliminate all cancerous cells heidden. It is very tough, cannot eat anything you used to eat, a complete different eating for 2 years! Juicing, enemas, all eating per the book. When i cheated, mean ete disallowed food, it immediately stopped reduction...because it is a tumir i can touch, i know... so basically tough, but worth it!!!!
Good presentation, I was one of the outliers, male, very physically fit had a nodule that was biopsied 2x and came back normal. It constanttly tickled my so I asked to have the nodule removed. They thought it was benign and surgery would only be a partial thyroidectomy and should take 2 hours or so. Surgery ended up being 6.5 hours long and was in 14 lymph nodes. I was 38 yrs old but history of fluoroscope use in my podiatric surgical work....
My biopsy was reported as hurthle cell lesion rich on my left lobe 1.9cm and malignancy is indeterminate. It is recommended to remove my left thyroid for exclusion of the hurthle cell. Is it worth doing the surgery?
I need a reoperation as my surgion has forgot to take out some lymbnodes... on level 6. Also 2 other 1cm lymbnodes on left and right side of the neck he advice not to operate now. Does this effect the prognoses?
What are the guidelines as of 2019? My nodule has grown from 2cm to 2.1cm. I have multiple nodules front and back of right and left side of the thyroid. And yet to have more tests other than 2 ultrasounds. Thank you
I know someone in a similar situation to you. He has multiple nodules on both sides of the thyroid. He has so far had four ultrasound tests done over a period of several years. The three biopsies did not indicate cancer. He has a goiter which could be seen as a small protuberance on his neck. Similar to you, his nodule has grown and as such finds it difficult to swallow food. He used to feel as if his throat was constricted as if the slight lump inside was impeding the passage of food. His solution was to get the Triple Iodine complex 12.5mg capsule from Swanson Vitamins, break open the capsule, and take about one-third of the powder inside in the morning and with breakfast and the other third of the capsule after lunch. He says he could feel almost instant relief in his throat as if the lump has shrunk. He could now eat his food not fearing that it might choke him. He also feels that the aches and pains he had along his spine have disappeared. Iodine is required by almost every cell in the body for it to work properly. He feels that he has found a way to make up for this shortfall. Years ago they used to add iodine to bread. Now they add bromine instead which does not serve the body in any way. Another supplementation method is to use Lugo's 2 % solution on the skin. He prefers the Swanson Triple iodine complex over all others. The body needs Iodide and molecular iodine for it to satisfy bodily needs. Please do a google search to see how Iodine helps. Apparently, the vast majority of the US population is iodine deficient. please search Sherry Tenpenny, DO, Outlines many disorders that come with Iodine deficiency. Finally, I hope you get better.
Excellent and informative. I had a total thyroidectomy and RAI back in 2009 (pappillary carcinoma). Given the advances in molecular testing and the newer guidelines, they might have gone with a lobe-ectomy and no RAI if i'd had it today instead of then, but better safe than sorry, so no regrets. QUESTION: Is there any data regarding the incidence of thyroid cancer that may have been caused by radioactivity affecting milk in certain regions ? I had none of the classic risk factors (i'm not in a low iodine region, nor was I exposures to known radiation sources), but I drank LOTS of milk as a kid back in the 60's & 70's ... cant help but wonder if there was any linkage between high altitude fallout plumes from secret nuclear testing at the time (re: cold war era).
@Wesley Hinson AFAIK, a fairly signifcant percentage of nodules turn out to be benign, and can come and come and go on their own. They can turn out to be just benign cysts, temporarily inflamed lymph nodes, et cetera ... but if a biopsy reveals a carcinoma, prompt intervention (surgery plus chemo and/or RAI) is usually indicated. I've had all three at one time or another. In my experience, homeopathy and/or medicinal herbs can be of some help for the first two, but tends to be a lot less effective with the third.
my wife was recently with thyroid cancer 3cm in diameter other thyroid looks healthy and it looks completely contained in the thyroid. would a thyroidapotny or lobeapotny be the best and what about radioactive iodine?
hi sir /mam , good work your explanation is very good , please help me , on 4-12-16 i gone through ultrasound scanning give the result there is a nodule in right lobe of size 1.9 cm x 2.2 cm and while left lobe has few small centimeter size hypo echoic nodule and i gone through FNA and the result is "adequate cellular smears showing hermorrage in background . follicular epithelial cells are arranged in mono layered sheets , vague follicular/acinar pattern and small clusters . cells are medium size , moderate cytoplasam ,eccentric or central nuclei, mild anisonucleosis , chromatin appears fine in most , colloid is scant"ANY DEFINITE PAPILLARY STRUCTURE , NUCLEAR INCLUSION OR GROOVES ARE NOT APPRECIATED " my doctor suggested me to go for surgery , then my doubt is can i go for surgery immediately or can i take time for it and is it cancer or not please suggest me
Does your pathology report say anywhere the word "Indeterminate"? Or "atypia of undetermined significance"? I am not a medical professional, just an informed patient, but thyroid malignancy, if present, are typically slow-growing; so I would say you do not have to make a decision immediately, and take your time to study your situation. Get another opinion or two. Possibly have a repeat biopsy, especially if your was rated "Indeterminate".
my question is I'm a male 36 year's old in my first ultrasound impression was likely benign diffuse thyroid disease with colloid nodules involving right lobe what does it mean. ? and in second ultrasound after a month later findings are likely suggestive of benign etiology and my thyroglubin is 507 what dose it mean? can any one answer or help me to come out of this god will bless you
Thyroglobulin is produced by the thyroid gland. This is normal, but I don't know offhand what the reference range is. You can look that up on a diagnostic lab website. However, a rising thyroglobulin level is used as a cancer marker in people who have had their thyroid removed. As for the benign etiology of your ultrasound, that is good. If anything is identified on the ultrasound that is suspicious, it would be recommended for fine needle aspiration biopsy (FNA). Because yours says benign diffuse thyroid disease, I would want to know what disease that is, so I could do everything I could to improve my situation. For example it could be Hashimoto's thyroiditis, an autoimmune condition that is fairly common.
What is the current status of various prognostic criteria i e Mayo Clinic’s or Lahey clinic criteria described in Eighties and nineties. Is there any evidence that two percent of your parents who had second surgery in the neck did benefit from it ??
I had a what was a kind of benign slash malignant tumor. How that happens is the tumor calcifies and stops you from going through puberty till 40. Also it cause my tonsil surgery to bleed out in my throat because it was just hard and sharp enough to poke through my throat causing bleeding. The funny is I am growing shark teeth. I think my dentist attempted to kill the root and make the tooth dull but instead it rolled down my chin and took out the sharp thyroid tumor instead making my hair grow back so I now might have full head of hair.
Yeah.According to present situation we must need to know about thyroid cancer. Its a very good video. We all have must know about the symptoms as well as treatments for thyroid cancer. goo.gl/FheQHy
Thank you all very much for this thorough and very informative video. God bless you and yours in the mighty name of Jesus. Amen and Amen and Amen and Amen and Amen and Amen.
Right lobectomy done Biopsy come out with Neoplasm follicular and suggestive of welll differentiated tumor of uncertain malignanant potential. Please advise further
21 days back done the surgery of PTC.right thyroid removed.stage pt1mno....doctor asked me to remove other nobe immediately.is it recommended.after how many days pet city scan is recommended
I had my whole thyroid out due to huge nodes, is it normal for the skin to heal to the wind pipe? When I swallow a flap forms and it is hard to swallow. I been back to the surgeon 3 times and he said it's scar tissue and rub it. Is this normal? I looked on youtube and I don't see this problem with others. I'm looking for help and advice.
If it were I, I would have it biopsied by FNA. Vascularity is one of the features that makes it suspicious for malignancy. However, that is not a specific sign because more than 50% of hypervascular solid thyroid lesions are benign.
I’ve had a 3 cm nodule on the Left lobe, biopsy outcome benign, although surgeon suspicious of result. I have a very small nodule on the right side. I’ve had a partial left thyroidectomy now and the histology revealed that this was initially a benign nodule but on one edge had started to infiltrate its capsule, because of this it now has to be called a cancer. Treatment options are 1. Surveillance 2. Total thyroidectomy and RAI (which I regarded as cracking a nut with a sledgehammer) I’ve chosen surveillance, I’m just not convinced that this ‘cancer’ is severe enough at this stage to chose option 2. All comments welcome.
He said iodine deficiency and being exposed to radiation when you were a child. Also family history of thyroid cancer. I personally believe vitamin D3 could affect thyroid hormone too I read it in an article. Also he mentioned more women are at risk of getting nodules than men(75% of women have thyroid nodules and most of them are benign)
Can someone help me please. My nodule was 6x3x5 mm couple years ago....then in october 2017 was homeogenous hypoechoic 8x6x6 mm .......NOW it’s heterogeneous 3.9x2.5x7.3 cm. Can someone tell me what that means? My apt is not for another month. Thanks
my english is not good enough. please help me translate them into chinese. thank specimen: US FNA left thyroid nodule. clinical: left thyroid nodule. PRC:solitary 23mm left interpolar thyroid nodule. on today's examination this appears predominantly solid and markedly hypoechoic with possible micro calcification within it. macroscopic: six air-dried smears labelled left thyroid received. microscopic: the smears are moderately cellular and contain uniform thyroid epithelium occurring in microfollicles, flat sheets and as stripped nuclei. in two smears there are some scattered clusters of cells with more abundant dense cytoplasm and larger round to oval or occasionally pleomorphic nuclei.A rare mitotic figure is identified.A single atypical cell shows an intranuclear clearing raising the possibility of an inclusion. There is some thick colloid ,haemosiderin-laden macrophages and an occasional multinucleated giant cell in the background. There are no psammoma bodies. conclusion: US FNA left nodule: sugestive of a cystic neoplasm (atypical),a cystic lesion with a mixture of uniform thyroid follicular epithelial cells and atypical epithelium with some papillary like features (see comment). comment: 1. it is noted that an aspiration has been done on 26.09.16(Q16c28352x) that revealed a non-diagnostic results with predominantly cystic material only. the changes is considered. no cell block is submitted for ancillary testing.

The pandemic is hard enough, recently found out I am allergic to substances found in the shot, I've had already a serve shock that lasted for 3 days, was told I was lucky to be alive, what do I do??? Especially, they are encouraging cancer patience to get the shot, please help anyone??? I'm lost?
I am 80 and they bave found nodules on my thyroid but I don’t know the size of them….at my age…what should I do? 20:40
I'm still learning about the difference of cancerous & noncancerous...
Is Proton Radiation ever used for malignant thyroid nodules?
What if you have multiple Calcified lymph nodes n your primary doctor says nothing to worry about, but then years later they grow
in hypo t based on the level of tshor t3t4 level ,we should select the dose of l thyroxine. which is more important in selecting dose
I've enjoyed the discussion it's very informative
my nodule grew a bit and its indifferential now or suspicious cells. the DNA/genetic test said i am high risk for aggressive thyroid cancer. I am to have surgery but is it really necessary or can this wait for a second opinion or even wait another month before surgery.
How are you doing now? What happened to you and hopefully everything is going well for you after your treatment!!
I do have total thyroidectoctomy and biopsy result is medullary thyroid ca,
Will it be needed to do will i chemo or radiation?
Thank u
I told m doctor something was wrong for months she ignored me until I switched doctor's and he had them do an ultrasound sound. In have a very suspicious lesion tumor. In just want this out.
Had a 3mm thyroid nodule that disappeared after a year of daily supplementation with lugols iodine, a couple of drops in filtered water each morning.
I would certainly try a holistic approach with good nutrition and supplements before putting my fate in the hands of medical doctors, no matter how well meaning they are. Unfortunately, holistic medicine is frowned upon by the AMA because it hurts profits.
Mike Arcuri Can I ask if your nodule is solid?
J HS I’m really not sure if nodule was solid. But I have read of other people either shrinking or eliminating nodules with iodine supplementation. Most people in U. S. do not get adequate iodine in their diet.
27 mm nodule cystic need surgery or not.. from.Sri Lanka.
What is a an aden survey
If your gonna use a explanation board make sure the person can see and read and be able to distinguish the explanation the lady is pointing too
My Biobsy came up negative for Cancer, yet, I am being advised to get the oiter removed along with the left side of my thyroid gland...Is there any other way of healing and reducin this goiter. My T3 and T4 s are all good --harmone levels are normal???? Is there something more I should know????
Hi did you found any solution to reduce it?
I had over 4inch tumor, i did Gerson Theraphy, and down to 1.5 in 3 month time! This theraphy require 2 yrs continuous to eliminate all cancerous cells heidden. It is very tough, cannot eat anything you used to eat, a complete different eating for 2 years! Juicing, enemas, all eating per the book. When i cheated, mean ete disallowed food, it immediately stopped reduction...because it is a tumir i can touch, i know... so basically tough, but worth it!!!!
What about thyroid goiter that is papillary cancer? Is it better to take both out the first time if goiter is 10 cm?
Good presentation, I was one of the outliers, male, very physically fit had a nodule that was biopsied 2x and came back normal. It constanttly tickled my so I asked to have the nodule removed. They thought it was benign and surgery would only be a partial thyroidectomy and should take 2 hours or so. Surgery ended up being 6.5 hours long and was in 14 lymph nodes. I was 38 yrs old but history of fluoroscope use in my podiatric surgical work....
How old are you know and how everything is going with that?
What does a prior image showed nodule inferior , today image showed to be superior within the thyroid?
Thank you
have you checked your iodine level?
Do you ladies experience severe pain or any pain at all?
I know someone in a similar situation to you. He has multiple nodules on both sides of the thyroid. He has so far had four ultrasound tests done over a period of several years. The three biopsies did not indicate cancer. He has a goiter which could be seen as a small protuberance on his neck. Similar to you, his nodule has grown and as such finds it difficult to swallow food. He used to feel as if his throat was constricted as if the slight lump inside was impeding the passage of food.
His solution was to get the Triple Iodine complex 12.5mg capsule from Swanson Vitamins, break open the capsule, and take about one-third of the powder inside in the morning and with breakfast and the other third of the capsule
after lunch.
Very impressive presentation. 👏👏👏
Hi, how long the patient can leave after the surgery? What did you mean by 10 years? It means they only live up to 10 years after the surgery?
don't worry
Be happy
They are only looking up to 10 years.
It's for people diagnosed with cancer and who didn't take any treatment. Don't worry :)
Is the reacurrance of thyroid cancer have a lot to do with during surgery, being open to the air?
What if u have a lot of nodules
hi sir /mam , good work your explanation is very good , please help me , on 4-12-16 i gone through ultrasound scanning give the result there is a nodule in right lobe of size 1.9 cm x 2.2 cm and while left lobe has few small centimeter size hypo echoic nodule
and i gone through FNA and the result is "adequate cellular smears showing hermorrage in background . follicular epithelial cells are arranged in mono layered sheets , vague follicular/acinar pattern and small clusters . cells are medium size , moderate cytoplasam ,eccentric or central nuclei, mild anisonucleosis , chromatin appears fine in most , colloid is scant"ANY DEFINITE PAPILLARY STRUCTURE , NUCLEAR INCLUSION OR GROOVES ARE NOT APPRECIATED "
my doctor suggested me to go for surgery , then my doubt is can i go for surgery immediately or can i take time for it and is it cancer or not please suggest me
Does your pathology report say anywhere the word "Indeterminate"? Or "atypia of undetermined significance"?
I am not a medical professional, just an informed patient, but thyroid malignancy, if present, are typically slow-growing; so I would say you do not have to make a decision immediately, and take your time to study your situation. Get another opinion or two. Possibly have a repeat biopsy, especially if your was rated "Indeterminate".
Excellent presentation, thank you.
From Australia here whish you were my Doctors
my question is I'm a male 36 year's old in my first ultrasound impression was likely benign diffuse thyroid disease with colloid nodules involving right lobe what does it mean. ?
and in second ultrasound after a month later findings are likely suggestive of benign etiology and my thyroglubin is 507 what dose it mean? can any one answer or help me to come out of this god will bless you
Nabila Razaei Why don't you go onto Google and that will give you a better idea, benign is OK though. Good luck.
I'm not getting it dear
help me if possible
Nabila Razaei
I had thyroid noulds. Last February I did baypse. BAYPSE TOLD 80% POSSIBILITY. DOCTORS TOLD ME IMMEDIATELY NEEDS SURGERY
Great presentation... Thank you.
Yeah.According to present situation we must need to know about thyroid cancer. Its a very good video. We all have must know about the symptoms as well as treatments for thyroid cancer.

I have a 19mm TI-RADS 4 solid module on left thyroid lobe is this cancer and serious
What happened?
Please tell about multiple solid n cystic nodules appearances ,, what color on FNAc predict benign or cancerous,,
Can't u shrink a nogarel
Can RF radio frecuente skin treatment cause it? I believe so.
21 days back done the surgery of PTC.right thyroid removed.stage pt1mno....doctor asked me to remove other nobe immediately.is it recommended.after how many days pet city scan is recommended
Does it mean if the thyroid nodule calcified, it is a cancer?
I have a partial thyroidectomy 45+ years ago now I have nodules if they are benign can I do nothing and what is consequence
What are the side effects of total thyroidectomy?
What is the treatment for thyroid mass with trachea invasion?
If your TSH is hi and your T4 is low what does that mean.
What's the best way to find a good thyroid doctor/surgeons in my area i live in springfield pa
I Have Highpo Thyroid having problems with weight gain . NOW IF I WANTED TO GET SURGERY TO REMOVE IT FOR GOOD ?? DO U THINK IS possible To DO SO???
Thanks for reply Dr. livihit and Dr. yeh
I'm looking for help and advice.
wow awesome presentation i must say
It is imperative you have a biopsy if your nodule is hypervascular? I really would rather try supplements first.......what do you folks think?
Treatment options are
1. Surveillance
2. Total thyroidectomy and RAI (which I regarded as cracking a nut with a sledgehammer)
I’ve chosen surveillance, I’m just not convinced that this ‘cancer’ is severe enough at this stage to chose option 2.
All comments welcome.
You never got to the root of the problem! How do thyroid nodules form?! What kind of mineral and vitamin deficiencies are the cause etc.?
Thanks
Thank you so much for this video!
Very helpful. Thank you.

Thank you for these information!
Hi Doc i have multiple nodules and my left thyroid lobe it has 8x4 mm measured. Do i need to do biopsy? Please reply. Thanks
I think that in the first 20-25 minutes of the presentation, she stated that nodules

specimen:
US FNA left thyroid nodule.
clinical:
left thyroid nodule.
PRC:solitary 23mm left interpolar thyroid nodule. on today's examination this appears predominantly solid and markedly hypoechoic with possible micro calcification within it.
macroscopic:
six air-dried smears labelled left thyroid received.
microscopic:
the smears are moderately cellular and contain uniform thyroid epithelium occurring in microfollicles, flat sheets and as stripped nuclei. in two smears there are some scattered clusters of cells with more abundant dense cytoplasm and larger round to oval or occasionally pleomorphic nuclei.A rare mitotic figure is identified.A single atypical cell shows an intranuclear clearing raising the possibility of an inclusion. There is some thick colloid ,haemosiderin-laden macrophages and an occasional multinucleated giant cell in the background. There are no psammoma bodies.
conclusion:
US FNA left nodule: sugestive of a cystic neoplasm (atypical),a cystic lesion with a mixture of uniform thyroid follicular epithelial cells and atypical epithelium with some papillary like features (see comment).
comment:
1. it is noted that an aspiration has been done on 26.09.16(Q16c28352x) that revealed a non-diagnostic results with predominantly cystic material only. the changes is considered. no cell block is submitted for ancillary testing.
Thank you, for this video, one question how whole they know if it has spread to they lymph nodes? Is it from the ultrasound? Or during thyroid surgery
