I think for Penelope and Colin it’s because they have been friends for a long time and Colin doesn’t see Penelope as a potential partner, he sees Penelope more like a sister at first.

Not Penelope and Colin:
- holding hands even though they're not dancing
- writing letters
- being on first name basis
- being alone together in rooms with closed doors
- declining dances (Pen) even though she isn't dancing with anyone else
The bit about the first names....I think that's what adds impact to the moment in season 2 with the accident when Antony is shouting "Ms. Sharma, Ms. Sharma" but as she falls he suddenly screams out "Kate!", kind of betraying his feelings for her.
It also shows how intimate Penelope and Colin's relationship has always been. They've only ever referred to each other by their given names, and in Colin's case he usually refers to her by a nickname which would have been even more presonal.
When she's begging him to marry her sister, Kate calls him 'Anthony' and you can see the look of shock pass across his face. Just beautiful.

Using a proper title and last name shows respect, using first name is for family and close friends only.
