It wasn't a bug, it was an animation error because they accidentally animated SSJ Goku for that scene but he wasn't supposed to use it yet. Because how animation schedules work, it was easier to just change the hair from gold to black to meet time constraints.
From what I see in dragon ball heroes ssj4 is around ssjb level so I’d say when he’s a kid in dragon ball daima he will use the form or something like it since his kid form will probably not be able to use god ko

Due to not fully awakening it, he partially transformed for like a few seconds. 2nd use was most likely during Future-Son Kamehameha where we see Goku's image with his super saiyan hair. 3rd use in hell against ginyu force and more villains was actually an accidental use due to animators not wanting to use SSJ first but couldn't change it in time. Other uses were in GT and DBS: Broly Movie where you can see Vegeta with a yellow aura face against Base form Broly if you were ever wondering why he had a yellow aura.
Super Kaio is theoretically still possible but only really while he’s dead for the same reason he was able to use SSJ3 better while dead, it’s WAY to draining. Ofc now he’s able to use it with blue.

These forms are canon, just in different timelines because of Future Trunks. (Thanks to him, we don’t even know if Goku getting Super Saiyan 5 is canon because it happened in another timeline) Even if the forms and all were fanmade, they’re still Dragon Ball related and MOST LIKELY part of a different timeline, if you look at it right.
Nah, that's an entirely different thing called Ikari or "Wrathful", where you can use the Oozaru power without becoming a giant monkey. It's really much more closer to SSJ4 in concept.

No, the original look for Ssj is what Beast Gohan, spiky silver hair with red eyes, looks like! But it was hard to put in manga so they went with the look they have.. Beast Gohan is a tribute to Toriyamas original SSJ look!!

Super Kaio is theoretically still possible but only really while he’s dead for the same reason he was able to use SSJ3 better while dead, it’s WAY to draining. Ofc now he’s able to use it with blue.

These forms are canon, just in different timelines because of Future Trunks. (Thanks to him, we don’t even know if Goku getting Super Saiyan 5 is canon because it happened in another timeline) Even if the forms and all were fanmade, they’re still Dragon Ball related and MOST LIKELY part of a different timeline, if you look at it right.
Fanmade=surely not canon, either way, Super confirms how many alternate timelines exist, nothing about GT and etc.

Nah, that's an entirely different thing called Ikari or "Wrathful", where you can use the Oozaru power without becoming a giant monkey. It's really much more closer to SSJ4 in concept.

Filler episode forms are canon… & ‘super Saiyan partial manifestation’ isn’t a form.. the team literally admitted to having incorrect shading for Gokus hair in that episode. It was supposed to be gold, it was just a mistake by production

That's not how Toriyama was going to make Super Saiyan look. The true story is that, Toei wanted Goku to use Super Saiyan against Slug but they didn't know what it looked like.
The False Super Saiyan design is something Toei made.
No, the original look for Ssj is what Beast Gohan, spiky silver hair with red eyes, looks like! But it was hard to put in manga so they went with the look they have.. Beast Gohan is a tribute to Toriyamas original SSJ look!!
