I don’t see anyone else commenting about the meaning behind this song. Johnny, just like Trent Resnor, was a heroine addict for a good part of his youth. Johnny is reflecting on that time in his life. He wanted more than anything for people to learn lessons from his life through his music. He wanted his life to be a testimony and example so that people would not fall into the same problems that he had throughout his life. His philandering, his drug abuse, his lack of love and appreciation for his family and where he came from. These lessons he learned later in life and his music reflected these lessons.
He was very ill with diabetes complications during recording of this album. His wife, June, who participated in parts of the video had died 3 mths. before final recording.
He passed away two weeks after recording this song and the video. It's been said that it "sounds like the reaper was in the room with him" when he was recording. This song was originally by Nine Inch Nails and Mr. Cash said that this was one of the songs that inspired and touched him.

I relate more to the NIN version because I first heard it as a teen when it came out it was my soundtrack throw a tough puberty but Cash's version is one of my favorite covers ever but the words change meaning because Cash is talking about past mistakes and how you feel at the end of life but NIN is about self doubt and the guilt and powerlessness that comes with it both Gold in my eyes
You should watch the video of a very aged Johnny Cash nearing the end. Adds such a strong drama to it. As he reflects on his past. It's very powerful and in my opinion better than the NIN original, just like Joe Cocker did with the Beatles With A Little Help From My Friends, which you all should react to if you haven't yet. I believe this was the last song Cash did. You should watch the actual music video, which they started filming when he was alive and finished after his passing? Because of that, they added a great deal of old footage of the legend. :)
You should definitely go back and watch the video he made for it. When he closes the piano cover at the end and runs his hands over it, it just feels like everything in his life is over.

He passed away about 7 months after the video was complete. His wife June passed away about 3 months after the video, and he was barely hanging on.
