The stem is basically a vice. So, it will ovalize the handlebars. And if you overtighten the stem it could and will strip the threads. Hope that kinda helped. Peace.
Hard to say without seeing it. Probably the chain line. If the back wheel is sitting at an angle in the back of the frame the chain could be making noise as it comes onto the driver. I would loosen the back wheel. Take the chain off the sprocket and driver, just slide it off and spin the cranks to make sure they feel smooth. Then the rear driver. If it seems fine put the chain back on and tighten it back up. See if it still knocks
