Matt, one important thing being neglected is ... TEstosterone is being converted by type 1 and type 2 5 AR to DHT in other areas of the body such as skin, prostate and liver. The vast majority of DHT made in these other areas, not the scalp. Also I think it’s about 10% of DHT is made without the conversion of testosterone. Ultimately what this means is that if you’re using a topical a shitload of DHT is still making its way to the hair follicles from blood circulation.
I’m on Fin. I went on it for years and decided to give it a try going off of it and just using minoxidil and maybe saw palmetto but after a few years it started to go. I’m back on it now but I had a hormonal change due to supplements and adding muscle and go through sheds. I’m going to stay on fin cause I don’t get super bad sides but I’m going to add topical dutasteride and take it rarely like once a week with microneedling also added and see how that goes

If we think carefully, 30% less of systemic Dht it's just a big amount... Bear in mind an important aspect: The long term accumulation of the drug. So in the first time it can be only 30%, but what's will be the amount after one years?
It would be interesting if this guy took two different test, the second one after one years of therapy, in order to compare the last one with the first one. I agree with you with the 5aris if you don't want to have any suppression

Does dutasteride work by binding to 5-alpha reductase rendering it ineffective? If that is the case, there will be wide reaching effects because other hormones (e.g., progesterone) will not be reduced into their dihydro version (e.g., dihydroprogeterone). Or, does dutasteride work by binding to testosterone thereby blocking the ability of 5-alpha reductase from reducing the testosterone to DHT? If so, does it bind to any hormones other than testosterone?
It binds to the enzyme, not the hormone. And the idea of topical is that it stays local so it doesn't matter if other substrates are affected as long as it's not systemic
Dermatologist numbs your scalp, does the biopsy and sends the sample to the lab for DHT level evaluation. Can cost couple 100's of $ and insurance doesn't usually cover it.
I personally believe that dutasteride should be applied with limited exposure times. Nizoral, which has is a higher molecular weight than Dut must be washed out after a few minutes and still exerts its effect on the hair follicle. Why let Dut sit on your head for hours absorbing into the blood stream? I dont see the point in that. As Dut lasts so long in the system, an exposure of 3-5 minutes once per week seems to be more than enough to gain significant scalp reduction without systemic absorption.
A Problem is that washing Out can lead to Higher systemic Absorption, through the eyes, mouth, nose or faceskin). For that reason U shouldnt wash Out Minoxidil, or let your head get wet 4 hours after using it

could you apply dut topically once a month since the Half life is about 4 weeks? mesotherapy applies it once every three months and gets results. not saying topical is the same as mesotherapy but yeah

DHT is the main cause. Why do you think women are not balding? It’s pretty simple. It’s not scalp inflammation or fibrosis because those are things women are not immune from either her they don’t bald like men do. It’s DHT.

of course you could try that, just get concentration right ... if its a normal concentration of dutasteride in the topical lets say 0.05% I would still not use it every day anyway since these things remain in hair follicle for over a month anyway. I'd probably use it twice a week at most, and less if you feel some side effects.

hard to say but in theory it is possible because of how much larger dutas is compared to finas - so they absorption is slower/localized topically

Dut or Fin does not treat inflammation and scal fibrosis
The only thing that treats all the symptoms is nettle root extract but you must consume very hight amount of it.

I have been using fibers for many years even after my hair transplant and haven't noticed increased hair loss myself.
