The base UI template I used was VacantUI (www.wowinterface.com/downloads/info23460-VacantUI-BecomeaBetterPlayer.html) but I don't think it is being maintained anymore, I have had to make changes to keep mine functional but I'm happy to let you know what my addon list looks like. Here is my full Addon list (according to WoWUp at least) imgur.com/a/Rp386Pu. Like I say a lot of this UI pack is based on VacantUI which I linked, that is a great starting point to work from if you want a UI similar to mine.
