This plays as you fight Laurence, The First Vicar, who is a significant character in Bloodborne's lore. The guy basically turned into a beast, and you're there to put him out of his misery.
But he screams hides the song a.k.a story, which an irony for him that lost his head both literally and figuratively. My fav music from FromSoftware ever.

The lyrics of the song are a retelling of the story of Laurance. At first, when he starts the Healing Church, Laurance rewards the mysterious Healing Blood to his followers, presenting it as a holy gift to overcome human limitations. Then, Laurance has doubts and starts to question the morality of his actions as he introspects how the tempation to excessively consume the blood causes people to regress into beasts. Then, when the song gets intense, Laurance is struck by the adage of his mentor Master Willem, "You would do well to fear the old blood," and he trembles in fear of the mistake he made in neglecting the saying's wisdom. The violin section is Laurance feeling a deep regret for his hubris, realizing he came to this conclusion too late and is now stuck with the consequences of his actions. At the same time, the beast motif creeps in, showing how him giving into despair is weakening his grip on his humanity, letting the savage beastial desires within him take control. The last section is Laurance's transformation into a Cleric Beast as he elatedly praises the Healing Blood, forgetting his past life and all his regrets to his blinding addiction to the blood.
Laurence is the founder of an organization called the healing church that spread the usage of healing blood throughout the city of yharnam (which you hear in the lyrics throughout the track quite a lot, with sanguine sanctus meaning sacred/holy blood). The healing church was essentially as an offshoot of scholars and theologians from a university called Byrgenwerth, which is attempting to commune and ascend humanity into higher dimensional beings called great ones, which are lovecraftian-esque beings. Byrgenwerth attempts to do this through "insight" which is knowledge about the cosmos and exposing your mind to the eldritch truth. Laurence, one of the students at byrgenwerth, did not believe this would work, so he founded the healing church to attempt to use the blood of the great ones (the holy blood) to ascend humanity instead by using the healing properties to experiment on the people of Yharnam. From the blood spread the curse of beasts throughout the city of yharnam, which transforms people into beasts, the curse brings yharnam into complete and utter chaos. When we find Laurence, we see him as a massive beast that has lost all sense of humanity and sanity, and the once bright scholar fights with no sense of thought nor intelligence. Laurence learnt the folly of his ways too late, which is reflected in the lyrics of the theme, at 2:33-3:07 "Diditus, Timere, Sanguine" (roughly) meaning "Spread the word, be afraid of the blood"
